The Madison Creek Demolition Plan will be complete in the following phases.
• Phase I — buildings D, E, F, G, H, I, K, all trees removed and site cleaned up
• Phase II — buildings J & N
Phase III — buildings A, B, C & L
Asbestos removal began on October 14, 2004 and will be completed by October 20, 2004
on Phase I.
We intend to remove all of the trees once the PM -10 plan is approved and begin
demolition of the buildings in the process described above.

On June 29th, 2004 an asbestos inspection and bulk sampling was conducted at the above project
address. Bulk samples were taken of all suspect asbestos containing building materials (ACBM) at the
request of the client named above. A total of (15) bulk samples were submitted to Carolina
Environmental, Inc. located at: 107 New Edition Court in Cary, NC (NVLAP Code 101768-0 and Cal.
Environ. Lab. Cert. #2483) to estimate the percentage of asbestos by volume and determine the type(s)
present .
Carolina Environmental, Inc. determines percentages and type of asbestos by using EPA approved
method 600/M4-82-020: Interim Method for the Determination of Asbestos in Bulk Samples. Bulk
samples are analyzed by PLM (Polarized Light Microscopy). Asbestos is quantified by using the visual
arena estimation technique and can only determine the approximate percentage of asbestos present.
After a thorough search is conducted and no asbestos is detected "none detected" will be noted in the
report. The essence of polarized light microscopy is not to emphatically determine that no asbestos is
present, merely that none was detected or, if it was, it was likely to be less than 1.0% of the sample.
When findings reveal less than or close to 1.0% asbestos contained in a sample, further analysis may be
called for on additional samples to confirm or denounce the initial findings. The California Code of
Regulations (CCR 1529) requires bulk sampling be conducted according to the A.H.E.R.A. protocol
provisions of 40 CFR Part 763.86. The A.H.E.R.A. protocol recommends three (3) or more friable
surfacing samples from a homogeneous area be collected and analyzed before any building material is
determined to be non -asbestos containing. Scott Morrison & Associates conducts asbestos inspection
surveys following the A.H.E.R.A. inspection protocol.

On February 22, 2005 Sam Lutchmiah, CAC #.93-1152, conducted asbestos sampling at the above location. The California Code
of Regulations (CCR 1529) requires bulk sampling be conducted according to the A:H.E.R.A protocol provisions of 40 CFR Part
763.86. The AH.E.R.A. protocol recommends three (3) or more friable surfacing samples from a homogeneous area be collected
and analyzed before any building material Is determined to lie non -asbestos containing. During the site inspection, 20 samples
were collected, 22 were analyzed and PLM analysis detected 2 samples to contain asbestos.
BULK SAMPLE ANALYSIS
All bulk samples were analyzed by Carolina Environmental, Inc. 107 New Edition Ct. Cary, NC 27511 NVLAP Accreditation
Number 9101768-0 and California Environmental Laboratory Certificate #2483: All samples were analyzed by Polarized Light
Microscopy (PLM) with dispersion staining in accordance with EPA Method #600/M4-82-420, The reliable limit of detection for PLM
is 1.00/6. Where "None Detected” appears in the lab report, it should be interpreted as meaning that no asbestos was actually
observed in the sample or if asbestos was actually observed then the amount was less than 1.0%. NOTE: When a sample contains
greater than 1,00/5 asbestos it is considered to be a regulated material by Federal Government Standards.
L1WTA77ON OF SAMPLING
Reasonable effort is made by ENVIRO-TEST to locate and sample suspect materials. The existence of unique or concealed
asbestos containing material and debris is a. possibility for any facility. ENV1RO-TEST does not warrant, guarantee, or profess to
have the ability to locate or identify all asbestos containing materials in arty given facility. The intention of this report Is to be used in
planning for construction or demolition. This report is not intended to be a construction document ENVIRO-TEST is not, and has
no responsibility as a generator, operator, treater, storer, transporter, or disposer of hazardous materials or waste found or
ideri fiad as a result of ENVIRO-TEST's work
SAMPLING METHODS
A visual inspection to evaluate and assess suspect ACM was performed at the subject facility. Suspect materials were assessed
based on physical condition, water damage, accessibility, and friability of materials. The inspection also allowed the Inspector to
develop an inventory of homogeneous areas to determine where samples would be collected or materials assumed to contain
asbestos. A homogeneous area is an area, which appears similar throughout in terms of color, te)dure, and date -of material
application. Suspect ACM was representatively sampled within each homogeneous area, refer to Asbestos Sample Summary for
a summary of sampling data Sample cogection methods depend on t14 material being sampled. In each case, the sample was .
contained in an appropriate sterile container, labeled with a unique sample identification number, and sealed into a plastic bag. A
chain -of -custody record was then prepared to accompany the samples to the laboratory. Sampling equipment was decontaminated
before and after sample collection to avoid cross contamination of samples,
SURVEY FINDINGS
The purpose of this survey is to identify Asbestos Containing Building Materials (ACBM) in all areas of the building. The areas
surveyed are planned for removal. Work consisted of collecting bulk samples of materials suspected of containing asbestos. Ali
samples listed below contained more than .1% asbestos. ENVIRO-TEST recommends that all ACBM removal be performed by a

On June 29th, 2004 an asbestos inspection and bulk sampling was conducted at the above project
address. Bulk samples were taken of all suspect asbestos containing building materials (ACBM) at the
request of the client named above. A total of (02) bulk samples were submitted to Carolina
Environmental, Inc. located at: 107 New Edition Court in Cary, NC (NVLAP Code 101768-0 and Cal.
Environ. Lab. Cert. #2483) to estimate the percentage of asbestos by volume and determine the type(s)
present. Carolina Environmental's telephone number is (919)481-1413.
Carolina Environmental, Inc. determines percentages and type of asbestos by using EPA approved
method 600/M4-82-020: Interim Method for the Determination of Asbestos in Bulk Samples. Bulk
samples are analyzed by PLM (Polarized Light Microscopy). Asbestos is quantified by using the visual
arena estimation technique and can only determine the approximate percentage of asbestos present.
After a thorough search is conducted and no asbestos is detected "none detected" will be noted in the
report. The essence of polarized light microscopy is not to emphatically determine that no asbestos is
present, merely that none was detected or, if it was, it was likely to be less than 1.0% of the sample.
When findings reveal less than or close to 1.0% asbestos contained in a sample, further analysis may be
called for on additional samples to confirm or denounce the initial findings. The California Code of
Regulations (CCR 1529) requires bulk sampling be conducted according to the A.H.E.R.A. protocol
provisions of 40 CFR Part 763.86. The A.H.E.R.A. protocol recommends three (3) or more friable
surfacing samples from a homogeneous area be collected and analyzed before any building material is
determined to be non -asbestos containing. Scott Morrison & Associates conducts asbestos inspection
surreys following the A.H.E.R.A. inspection protocol.

Stereoscopic microscopy and polarized light microscopy coupled with dispersion staining is the analytical technique used
for sample identification. The percentage of each component is visually estimated by volume. These results pertain only
to the samples analyzed. The samples were analyzed as submitted by the client and may not be representative of the larger
material in question. Unless notified in writing to return samples, Carolina Environmental, Inc. will discard all bulk samples
after 30 days.
Many vinyl floor tiles have been manufactured using greater than 1 % asbestos. Often the asbestos was milled to a fiber
size below the detection limit of polarized light microscopy. Therefore, a "None Detected" (ND) reading on vinyl floor tile
does not necessarily exclude the presence of asbestos. Transmission electron microscopy provides a more conclusive
form of analysis for vinyl floor tiles.
It is certified by the signature below that Carolina Environmental, Inc_ is accredited by the National Voluntary Accreditation
Program (NVLAP) for the analysis of asbestos in bulk materials. The accredited test method is EPA 1600 / M4-821020 for
the analysis of asbestos in building materials. Procedures described in EPA / 6001 R-931116 have been incorporated
where applicable. The detection limit for the method is 0.9 % (trace amount). Carolina Environmental, Inc.'s NVLAP
accreditation number is #101768-0. This report is not to be used to claim product endorsement by NVLAP or any agency
of the U. S. Government. This report and its contents are only valid when reproduced in full_ Dust and soil analyses for

On June 29th, 2004 an asbestos inspection and bulk sampling was conducted at the above project
address. Bulk samples were taken of all suspect asbestos containing building materials (ACBM) at the
request of the client named above. A total of (08) bulk samples were submitted to Carolina
Environmental, Inc. located at: 107 New Edition Court in Cary, NC (NVLAP Code 101768-0 and Cal.
Environ. Lab. Cert. #2483) to estimate the percentage of asbestos by volume and determine the type(s)
present. Carolina Environmental's telephone number is (919)481-1413.
Carolina Environmental, Inc. determines percentages and type of asbestos by using EPA approved -
method 600/M4-82-020: Interim Method for the Determination of Asbestos in Bulk Samples. Bulk
samples are analyzed by PLM (Polarized Light Microscopy). Asbestos is quantified by using the visual
arena estimation technique and can only determine the approximate percentage of asbestos present.
After a thorough search is conducted and no asbestos 'is detected "none detected" will be noted in the
report. The essence of polarized light microscopy is not to emphatically determine that no asbestos is
present, merely that none was detected or, if it was, it was likely to be less than 1.0% of the sample.
When findings reveal less than or close to 1.0% asbestos contained in a sample, further analysis may be
called for on additional samples to confirm or denounce the initial findings. The California Code of
Regulations (CCR 1529) requires bulk sampling be conducted according to the A.H.E.R.A. protocol
provisions of 40 CFR Part 763.86. The A.H.E.R.A. protocol recommends three (3) or more friable
surfacing samples from a homogeneous area be collected and analyzed before any building material is
determined to be non -asbestos containing. Scott Morrison & Associates conducts asbestos inspection
surveys following the A.H.E.R.A. inspection protocol.

Stereoscopic microscopy and polarized light microscopy coupled with dispersion staining is the analytical technique used
for sample identification. The percentage of each component is visually estimated by volume. These results pertain only
to the samples analyzed. The samples were analyzed as submitted by the client and may not be representative of the larger
material in question. Unless notified in writing to return samples, Carolina Environmental, Inc. will discard all bulk samples
after 30 days.
Many vinyl floor tiles have been manufactured using greater than 1 % asbestos. Often the asbestos.was milled to a fiber
size below the detection limit of polarized light microscopy. Therefore, a "None Detected" (ND) reading on vinyl floor tile
does not necessarily exclude the presence of asbestos. Transmission electron microscopy provides a more conclusive
form of analysis for vinyl floor tiles.
It is certified by the signature below that Carolina Environmental, Inc. is accredited by the National Voluntary Accreditation
Program (NVLAP) for the analysis of asbestos in bulk materials. The accredited test method is EPA / 600 / M4-82 / 020 for
the analysis of asbestos in building materials. Procedures described in EPA / 600 / R-93 / 116 have been incorporated
where applicable. The detection limit for the method is 0.1 % (trace amount). Carolina Environmental, Inc.'s NVLAP
accreditation number is #101768-0. This report is not to be used to claim product endorsement by NVLAP or any agency
of the U. S. Government. This report and its contents are only valid when reproduced in full. Dust and soil analyses for
asbestos using PLM are not covered under NVLAP accreditation.

The Madison Creek Demolition Plan will be complete in the following phases.
• Phase I — buildings D, E; F, G, H, I, K, all trees removed and site cleaned up
• Phase II — buildings J &.N�
• Phase III — buildings A, B, C & L
Asbestos removal began on October 14, 2004 and will be completed by October 20, 2004
on Phase I.
We intend to remove all of the trees once the PM -10 plan is approved and begin
demolition of the buildings in the process described above.
