On March 06, 2007 an asbestos inspection and bulk sampling was conducted at the above project
address. Bulk samples were taken of all suspect asbestos containing building materials (ACBM) from
the areas to be remodeled only at the request of the client named above. A total of (08) bulk samples
were submitted to Carolina Environmental, Inc. located at: 107 New Edition Court in Cary, NC (NVLAP
Code 101768-0 and Cal. Environ. Lab. Cert. #2483) to estimate the percentage of asbestos by volume
and determine the type(s) present .
Carolina Environmental, Inc. determines percentages and type of asbestos by using EPA approved
method 600/M4-82-020: Interim Method for the Determination of Asbestos in Bulk Samples. Bulk
samples are analyzed by PLM (Polarized Light Microscopy). Asbestos is quantified by using the visual
arena estimation technique and can only determine the approximate percentage of asbestos present.
After a thorough search is conducted and no asbestos is detected "none detected" will be noted in the
report. The essence of polarized light microscopy is not to emphatically determine that no asbestos is
present, merely that none was detected or, if it was, it was likely to be less than 1.0% of the sample.
When findings reveal less than or close to 1.0% asbestos contained in a sample, further analysis may be
called for on additional samples to confirm or denounce the initial findings. The California Code of
Regulations (CCR 1529) requires bulk sampling be conducted according to the A.H.E.R.A. protocol
provisions of 40 CFR Part 763.86. The A.H.E.R.A. protocol recommends three (3) or more friable
surfacing samples from a homogeneous area be collected and analyzed before any building material is
determined to be non -asbestos containing. Scott Morrison & Associates conducts asbestos inspection
surveys following the A.H.E.R.A. inspection protocol.

Stereoscopic microscopy and polarized light microscopy coupled with dispersion staining is the analytical technique used
for sample identification. The percentage of each component is visually estimated by volume. These results pertain only
to the samples analyzed. The samples were analyzed as submitted by the client and may not be representative of the larger
material in question. Unless notified in writing to return samples, Carolina Environmental, Inc. will discard all bulk samples
after 30 days:
Many vinyl floor tiles have been manufactured using greater than 1% asbestos. Often the asbestos was milled to a fiber
size below the detection limit of polarized light microscopy. Therefore, a "None Detected" (ND) reading on vinyl floor the
does not necessarily exclude the presence of asbestos. Transmission electron microscopy provides a more conclusive
form of analysis for vinyl floor tiles.
It is certified by the signature below that Carolina Environmental, Inc. is accredited by the National Voluntary Accreditation
Program (NVLAP) for the analysis of asbestos in bulk materials. The accredited test method is EPA / 600 / M4-82 / 020 for
the analysis of asbestos in building materials. Procedures described in EPA / 600 / R-93 / 116 have been incorporated
where applicable. The detection limit for the method is 0.1 % (trace amount). Carolina Environmental, Inc.'s NVLAP
accreditation number is #101768-0. This report is not to be used to claim product endorsement by 'NVLAP or any agency
of the U.S. Government. This report and its contents are only valid when reproduced in full. Dust and soil analyses for

In compliance with 1997 UBC Section 1702 (which the work was designed to comply with), a structural observation was performed
on the structure located at 77-849 Calle Arroba, La Quinta, CA 92253. The Observation was performed on this building on a regular
basis ending on this date (May 14, 2008). An inspection was performed to address modification required to the design following the
discovery of Termite damage in February of 2008. As per the 97 Uniform Building Code, Observation was also provided prior to
framing inspection (and roof inspection) to confirm that all structural details, including modifications and detail changes, have been
performed in compliance with the structural design and with the approval of the Engineer of Record noted in the title block of this
report. All load paths, blocking, sheathing and modifications noted herein have been observed and this report is issued as notice of
compliance and/or indications of non-compliance that must be corrected (see below).
REVISIONS:
In addition to the observation report, modification to the original cantilevered columns (6x6 DF Posts) in the front of the entry porch
due to the discovery of bedrock and decomposed granite boulders. The wood columns embedded in the flag pole foundations noted on
the original drawings have been replaced with two tube steel columns — TS 6x6xl/4" (HSS 6x6xl/4") (Fy--46 ksi) welded to alt -inch
square by '/a -inch thick base plate and secured to a new grade beam foundation ("U" shaped) as shown in the attached details R-01 and
R-02. The steel columns were fabricated by White Steel who issued shop drawings to the General Contractor — Hartman Construction
and approved by the EOR who has wet sealed this report. All welding was done in the shop of an approved fabricator under the
supervision of a certified welder licensed in the City of La Quinta. A copy of the shop drawings is to be submitted to the city official
prior to issuance of a certificate of occupancy. All welds are minimum '/4 -inch fillet welds. The base plate is secured to the top of
grade beam upon 1-1/2" minimum dry pack grout for level. Please note that in order to accommodate the slope of the landscape from
the front of the home, the porch is constructed as shown in the above mentioned details and the steel columns are embedded above the
grade beam in a 22" minimum (24" max) square concrete pier that is set with the top of concrete equal to the top of the interior
finished slab on grade. The mass of the concrete, plus the anchorage of the column to the grade beam dowelled to the existing or new
northeast elevation of the residence (front door entry) provides more than sufficient mass to control the bending in the column and to
create fixity of the base plate and anchor bolts to the grade beam below. Furthermore, the change from a 6x6 DF post originally
embedded into a TS6x6x1/4" sleeve has been replaced with a much more rigid tube steel column — full height to the bent plate
connections securing the roof drag truss (double truss) originally designed for an R=2.2 that increased the actual lateral load by more
than 100% overly conservative.
The only other revision required in this home was the replacement of plywood shear wall located on sheet S2.1 in Grid Line 2 due to
the damage discovered caused by termites. It was not possible to obtain the capacity required in the remaining stud walls attached to
.the existing foundations for a required equivalent of 12 -feet of 430-plf plywood shear or 5160 -pounds of shear required to be collected
in grid line 2. As an alternative, the plywood shear wall was replaced with a Hardy Frame HF78x24 1-1/8" dia. X 10" minimum
embedded anchor bolt at each end which was installed as shown in attached detail R-03 along Grid Line 2 in the new closet extension
to the master bedroom. The extension included a new foundation and slab on grade that was dowelled into the existing concrete slab
on grade (as shown on the original plans and details), but the width of the tum down edge of the slab was widened to allow for proper
embedment of the 1-1/8" diameter SAE 1018 threaded rod w/ a 2-1/2" square x'/a-inch thick plate washer and two nuts at the ,
embedded end of the bolt. The anchor bolts and shear anchors that were supplied with the Hardy Frame were installed using the
manufacturers templates to tie the anchors in place prior to placement of concrete. The capacity of the new Hardy Frame as noted in
the Hard Catalog for an R of 5.5 similar to the original 1 ood and in compliance with Hard 's ICBO report for a frame laced on

the concrete rather than wood is found on page 11 of the product catalog number 6-06A. The allowable shear for this product is 5,326 -
pounds which exceeds the 5,160 -pounds required on the original plan.

All other work was done in compliance with the original drawings. In some locations, the method of installing continuous blocking
was revised from manufactured truss blocking to 2x top and bottom plates with a plywood web nailed to the plates with 8d common
nails at 6" o.c. maximum (as noted on the drawings). Photographs of all connections have been recorded and are available upon
request if any questions arise that are not covered by the attached details.
Excerpt from 1994 Uniform Building Code Section 1702 — Structural Observation:
Structural Observation shall be provided in Seismic Zone 3 or 4 when one of the following conditions exists:
1. The structure is defined in Table 16-K as Occupancy Category I, IF or III or,
2. The structure is required to comply with section 403, or
3. When so designated by the architect or engineer of record, or
4. When such observation is specifically required by the building official.
The owner shall employ the engineer or architect responsible for the structural design, or another engineer or
architect designated by the engineer or architect responsible for the structural design, to perform structural
observation as defined in Section 220. Observed deficiencies shall be reported in writing to the owner's
representative, special inspector, contractor and the building official. The structural observer shall submit to the
building official a written statement that the site visits have been made and identifying any reported deficiencies
which, to the best of the structural observer's knowledge, have not been resolved.
® The structure was found to be in conformance with the design details, framing plans and structural notes. The engineer
responsible for the structural design has certified to the best of his knowledge that the building complies with the design,
drawings and details for the lateral resisting system. Structural revisions have been made and confirmed by the engineer
responsible for the structural design on this day (May 14, 2008). This letter shall be submitted to the building official to
certify that compliance with Section 1702 and Section 220 of the 1997 Uniform Building Code to confirm revisions made.
The engineer certifies to the best of his knowledge that the structural revisions have been resolved and is in conformance with
the intentions of the structural design, drawings and details.
All other work was inspected and found to be in compliance with the structural engineering plans. At the engineer's and owner's
discretion, some connections were improved to protect the connection from lateral motion. It was not necessary to detail these as the
corrections were done to improve the connection where mechanical straps would not work (sandwiched 2x studs were used and nailed
in place to improve the connections where deemed necessary.
