Resolution 2012=01 1 approving Conditional Use Permit 2012-141, subject to the
attached Condition's.of Approval,

GENERAL
-1. The applicant agrees to defend, indemnify and hold harmless the City of La
Quinta ("City"), its agents, officers and employees from any claim, action or
• proceeding to attack, set aside, void, or annul the approval of this. Village
Use Permit. The City shall have sole discretion in selecting its defense
counsel.

proceeding and shall cooperate fully in the defense..
.2.. This Conditional Use Permit approval shall expire on May 22, 2014, two
years after its. effective date, pursuant to Section 9.200.060 (C) of the
Zoning' Code, unless extended pursuant, to the provisions of Section
9'.200.080:
3.. Sharps and other bio or medical waste shall be properly disposed and hauled
off-site by. a .third party vendor and shall be in accordance with all applicable
state laws.
4. A,vinyl fence enclosure is permitted along the south side of the building. The
enclosure shall not exceed -a height of eight feet ' measured from finished
• grade. The fence material shall be of a compatible color .to the main building.
A trash receptacle shall be provided within the enclosed area. The enclosure
shall be cleaned daily, with all feces and waste properly disposed of'. . Use of
the enclosure Is limited to business hours.
5. Animals are not to be left unattended in the outdoor pet play area
6. All outdoor areas, including the adjacent parking lot, adjacent open lawn
areas and public sidewalk areas in front of the building shall be monitored for
waste and shall be cleaned daily. A trash receptacle alongwith waste clean-
up bags shall be provided in front of the building.
7. The applicant shall. obtain a building permit from the Building and Safety
Department prior to installation of the fence.

RE: TENANT IMPROVEMENT PLAN CHECK -Non Structural

You have been issued a release for a tenant improvement on an existing building. THIS IS NOT
AN OCCUPANCY PERMIT.
It is prohibited to use/process or store any materials in this occupancy that would classify it as an
"H" occupancy per Sec. 307 of the 2010 CBC.
THE FOLLOWING CONDITIONS MUST BE MET PRIOR TO INSPECTION:
Install door hardware and exit signs as per Chapter 10 of the CBC.
A minimum 2AIOBC Fire Extinguisher, (State Fire Marshal Approved) must be mounted in a
visible location within 75' walking distance from any point in your building or suite. Fire
extinguishers can be installed by a licensed extinguisher company with a State Fire Marshal
service tag attached to the extinguisher, or purchased from a retail store with a sales receipt
attached. A licensed fire extinguisher company must service extinguisher yearly.
All breakers must be labeled and a clearance of 36 inches must be maintained around the panel
at all times.
A durable sign stating "This door to remain unlocked during business hours" shall be placed on
or adjacent to the front exit door. The sign shall be in letters not less than one inch high on a
contrasting background.
Install Knox Key Lock box, mounted per recommended standard of the Knox Company. Special
forms are available from this office for the ordering of the Key Switch. This form must be
authorized and signed by this office for the correctly coded system to be purchased.
Provide key(s) to the tenant space for inclusion in the main building Knox Box. Key(s) shall
have durable and legible tags affixed for identification of the correlating tenant space. Key(s)
shall be provided at time of final inspection.
Electrical room door shall be posted "ELECTRICAL ROOM" on the outside of door.
Applicant/installer shall be responsible to contact the Fire Department to schedule inspections.
A re -inspection fee will be required if more than one (1) inspection is necessary. Requests for
inspections are to be made at least 72 hours in advance and may be arranged by calling (760)
863-8886.

Department Planning & Engineering Staff at (760) 863-8886.
