responsibilities and possible risk you may incur by having this permit issued in your name as the
Owner -Builder. We will not issue a building permit until you have read, initialed your understanding of each provision,
signed, and returned this form to us at our official address indicated. An agent of the .owner cannot execute this notice
unless you, the property owner, obtain the prior approval of the permitting authority.
OWNER'S ACKNOWLEDGMENT AND VERIFICATION OF INFORMATION _
DIRECTIONS: Read and initial each statement below to signify you understand or verify this informa!ion.
Pt L I understand a' frequent practice of unlicensed persons is to have the property owner obtain an "Owner -Builder"
building permit that erroneously implies that the property owner is providing his or her own labor and material personally. I, as
an Owner -Builder, may be held liable and subject to serious financial- risk for any injuries sustained by an unlicensed person
and his or her employees while working on my property. My homeowner's insurance may not provide coverage for those
injuries. I am willfully acting as an Owner -Builder and am aware of the limits of my insurance coverge for injuries to workers
on my property.
2. I understand building permits are not required to be signed by property owners unless they are responsible for the
construction and are not hiring a licensed Contractor to assume this responsibility.
3. I understand as an "Owner -Builder" I am the responsible party of record on the permit. I understand that I may protect
myself from potential financial risk by hiring a licensed"Contractor and having the permit filed in his or her name instead of my
own.

permits and contracts.

MASONRY:
1. Block units shall be grade N-1 normal weight conforming to ASTM C90-75 standard
specifications for hollow 1 ad -bearing concrete masonry units. Minimum compressive strength
of block shall be 1500psid
2. Mortar shall have a mum compressive strength of 2000psi at 28 days.. Type S mortar
per ASTM C 270 shall be used.
3. Level B quality assurance per TMS402/ACI 530 Table 1.15.2 is required.
Minimum tests and submittals are as follows:
• Certificates for m terials used in masonry construction indicating compliance with the
contract documenis.
• Verification of compressive strength of masonry prior to construction except where
specifically exempted by the Code.
Minimum inspections are as follows:

4. All grout shall havea minimum compressive strength of 2000psi at 28 days and shall
conform to ASTM C 476.
5. No pipes or ducts sh 11 be placed in masonry units unless specifically detailed or noted on
plans.
6. Aggregates for mortar and grout shall be natural sand and rock conforming to ASTM C-144
and C-404. ,
7. All cells containing leinforcement and all cells below grade shall be solie. grouted. Masonry
units shall be laid to a maximum height 5 feet. Grouf pour in vertical cells of masonry units
-shall conform to TMS6 2/AC1530.1 3.5 and Table 7.
8. Head joints at retaining walls shall be omitted at 24"oc in the first course or a 2" diameter
hole at 24"oc shall be p vided for drainage. y
9. All soils used for back fill shall be granular type unless specifically approved by the
engineer. "
10. Backfilling shall not be accomplished until a minimum of 7 days after wall is completed.
Note that backfill isnot to be compacted with the use of heavy equipment unless specifically
approved by the engineer.
FOUNDATIONS/EXCA ATIONS:
1. All soils work shah comply with the soils report as prepared by:

2. Prior to any excav ition, contractor shall consult with the governing jurisdiction and with
all applicable utilities as to the location of all underground lines, pipes, conduits, etc., and he
shall be responsible for the protection of same. '
3. Excavation of all tings shall be as noted on plans, and shall be inspected and approved
by the Building Departme it and the soils engineer prior to placement of concrete.
4. Design soil bearin is 1500psf.
5.. Allowable increas s in soils bearing values shall be per soils report or CBC Table 1804.2
when no soils report is pr vided.
6. The required depth of all footings shall be into firm, undisturbed natural soil or fully
compacted soil as require i by the soils report or building department.
7. The bottom of foc tings shall be level. Prior to placing 'concrete clean area of all debris,
loose dirt, or water. Alleinforcing shall be thoroughly cleaned prior to placing concrete. r

All fill and backfi 1 material shall be compacted in accordance with ASTM D 15 57 to a
minimum relative compa tion of 90%. All fill and backfill material shall be approved by the
Building Department. Backfill material for retaining wall shall be granular free draining
material.
9. See architectural, mechanical, plumbing, and electrical drawings for additional
requirements. Verify all anchors and other embedded items, depressions, fin.shes, dowels for
masonry and/or concrete walls curbs, etc.
10.' Concrete forms shall all be constructed and braced sufficiently as to restrain the concrete in
the required position of tie concrete during and after the pour.
11. All hold-down bc Its, anchor bolts, dowels, sleeves, reinforcing steel and other embedded
items shall be securely sitioned and inspected prior to pouring concrete.
12. Foundation plats or sill shall be bolted to the foundation with not less than 5/8' x 10"
steel anchor bolts (stand d J bolts) embedded a minimum of 7" into the first pour of concrete or ,
reinforced masonry.
13. Foundation bolts shall not be spaced more than 6'oc and there shall te a minimum of
two bolts.per piece with one bolt located not more than 12" of each end of each piece.
14. Where a cold joi t occurs between the concrete slab and the foundatic-n the minimum
required embedment del th of anchor bolts and holddowns shall be into the foundation concrete.
15. 'All interior part tion sills to, be fastened to foundation with Hilti Powder Actuated DS
studs at 48"oc, in accor ance with ICBO RR #1290 and manufacturer's recommendations, .
unless otherwise noted. Sill plates to have a minimum of two fasteners per piece located within
12" of each end.

REINFORCEMENT-.
1. Unless noted otherwise on plans, all steel reinforcement shall be deformed bars and shall
comply with A-706 grade
(40 for #4 bars and smaller, grade 60 for #5 bars and larger.

CBC section 2104.5 for

` ASTM C-330 for lightwe ght concrete.

RESPONSIBILITY'OF ONTRACTOR:
1. Prior to proceeding,with the work described in these documents, the contractor shall°
inspect the site and shall examine the drawings and specifications. Any discrepancies shall be
brought to the attention of the architect/engineer.
2. During •constructs n the contractor shall have complete responsibility for all work as
described herein (includ' that performed by subcontractors) unless specifically noted
otherwise. The contractc r shall provide personal supervision of the work at all times, or, in the
event of his absence shall designate a competent supervisor to act in his behalf: ,
3. The contractor-shE 11 be responsible for the method of construction to be: used and for
providing safe working conditions-at the job site. He shall provide temporary shoring and/or
bracing as needed and provide for cleanup and removal of debris as the workp_-ogresses. Upon
completion of the work, 1 debris shall be removed from the site. All materia:. stored on the site
shall be protected and sh 1 not be placed on the partially completed work in such a manner as to
cause overload or damage thereto.
4. ' ' It shall be the resf onsibility of the contractor to provide and maintain workmen's_
compensation insurance for all his employees during the course of this work and to determine '
that all other workers in this project are similarly protected.
5. The work as desc 'bed herein shall be done in a competent manner in accordance with
good construction pra&ices and shall proceed expeditiously and without unwarranted delays.
6. The contractor shall be responsible for compliance with all federal, state, and local laws
and regulations applicabl to this work. Unless noted otherwise the building code for the local
governing agency shall b� the 2012 Edition of the IBC and the 2013 Edition of the CBC,
including all amendment thereto. Herein after referred to as the CBC or IBC_
7. Any support services performed by the architect/engineer during construction shall be
distinguished from continuous and detailed inspection services which are furnished by others.
These support services p rformed by the architect/engineer, whether of materiae or work, and
whether performed prior to, during or after completion of construction are performed solely for
the purpose of assisting in quality control and in achieving conformance with the contract
drawings and specifications, but they do not guarantee contractor's performanc-. and shall not be
construed as supervisio of construction.
