The plans are now approved subject to the conditions listed below and the attached compliance sheet
1) Make up air must be provided back to the building that is equal to the amount being exhausted by the
ventilation hood. (1200 dm) Make up air must be inner -connected with the exhaust hood by 1 common
switch.
2) Provide a certified air balance report for the exhaust hood and make-up air prior to final approval.
3) Ventilation must be provided inside of janitor closet. Mechanical ventilation or louvered doors.
4) Provide a chemical shelf and mop hangers inside of janitor closet.
5) Extend FRP to a height of 8' around mop sink. --If water heater sits above mop sink, FRP will be required
from the floor to the underside of the water heater.
6) Install a 12" stainless steel splashguard between prep sink and the 3 -compartment sink.
7) A faucet must be installed on the 3 -compartment sink that reaches all compartments. Spray arm may
be added above the faucet,

CONSTRUCTION INSPECTIONS: Contact the Plan Checker for a Preliminary Inspection when construction is
approximately 80% complete, with plumbing, rough ventilation, and rough equipment installed. Request for
inspection should be made at least five (5) working days in advance.
A FINAL INSPECTION MUST be made upon completion of ALL work including finished details. APPROVAL to
operate shall not be granted, or remodeled areas approved to operate, until the facility has passed the FINAL
INSPECTION, and "APPLICATION TO OPERATE" has been completed and PERMIT FEES have been paid.

of 4". No.rubber top set is allowed.
9) All gypsum board walls inside of prep room (N & E walls) must be smooth and painted with a light
colored, semi/ gloss paint.
10) Provide smooth washable ceiling panels above the janitor sink.
11) Provide an outside area for a trash dumpster. Interior walls must be smooth and the flooring and walls
must be sealed in an approved manner.
12) Concrete floor inside of demo kitchen must be sealed with an approved 2 part water-based epoxy. .
Provide spec's to this Department prior to sealing.
13) Minimum size water heater must be 50 gallons and produce 69,000 btu's (gas) or 15 kw's (electric): This
size will accommodate an undercounter dishwasher at a later date.

This form is to be used by th designer and attached to the plans. Listed below are all the acceptance tests for lighting systems. The
designer is required to check the boxes by all acceptance tests that apply and list all equipment that ,require an acceptance test. If all
equipment of a certain type requires a test, list the equipment description and the number of systems to be tested in parentheses. The
NJ number designates the Section in the Appendix.of the Nonresidential ACM Manual that describes the test. Also indicate the person
responsible for performing the tests (i.e. the installing contractor, design professional or an agent selected by the owner). Since this
form will be part of the plans, completion of this section will allow the responsible party to budget for the scope of work appropriately.
