Dear Mr. Wentworth:
As requested, Applied Materials & Engineering, Inc. (AME) has completed load -testing QMTMH Tile
Mount with Unirac angle for shear and tensile strength. The purpose of our testing was to evaluate the
tensile and shear load capacity of QMTMH Tile Mount Hardware in Oriented Strand Board (OSB).
SAMPLE DESCRIPTION
Six (6) 24"x 12"x4" thick samples with 1/2" thick OSB were delivered to our laboratory. Photographs
showing sample makeup are provided in Appendix A.
PROCEDURES & RESULTS
1. Tensile Strength
Three samples were tested for tensile strength on May 18, 2009 using a United Universal testing
machine. Samples were rigidly attached to the testing machine and a tensile load was applied to the
Unirac angle at n displacement rate 0.1 inches per minute without shock until failure. Detailed results are
provided in Table I. Based on these results, the average tensile strength of the tile mount in OSB was
detennined to be 1016 Ibf.
2. Shear Strength
Three samples were tested for shear strength on May 18, 2009 using a United Universal testing machine.
Samples were rigidly attached to the testing machine and a shear load was applied to the Unirac angle at
a displacement rate 0.1 inches per minute without shock until failure. Detailed results are provided in
Table It. Based on these results, the average shear strength of the tile mount in OSB was determined to
be 322 Ibf.

INSTRUCTIONS TO APPLICANT:
1) Provide a written response to each comment on the following pages, noting specifically where the correction
can be found. Responses such as, "Sheet A-3," or, "Handrails shall be per CBC Section 1003," are not
acceptable. Show or note specifically how compliance with a code requirement is achieved. Responses
such as, "Added note 16 on sheet A-3" are appropriate and will help expedite your back check.
2) Corrections may not be made by handwriting on existing drawings. Revise originals and reprint Plans and/or
Calculations as necessary.
3) Return all red -marked Plans and/or Calculations with your resubmittal.
4) Each sheet of resubmitted Plans and/or Calculations shall include the preparer's name and telephone number
and shall be wet -signed by the preparer. If the preparer is a licensed architect or engineer, all documents
prepared by that licensed individual shall also bear his/her stamp as prescribed by California Business and
Professions Code Section 5536. Resubmittals will not be accepted with signatures missing.

SECTION A - GENERAL
1) Provide on each sheet of resubmitted Plans and cover sheet of bound Calculations the preparer's name and
telephone number and wet -signature. If the preparer is a licensed architect or engineer, all documents
prepared by that licensed individual shall also bear his/her stamp as prescribed by California Business and
Professions Code'5536. Resubmittals will not be accepted with signatures missing.
2) Note on Plans: "All construction shall comply with the 2010 California Residential, Mechanical, Electrical,
Plumbing, Energy, and Green Building Codes and the La Quinta Municipal Code."
SECTION B — SOLAR
I. General
1) Reconcile noted information shown on single line diagram. See P review redlined plan. Please provide a
response for each item, either on returned redlined plan or in letter. Responses not provided to 1st review
comments or redlines - please provide response to each redlined comment from 1" and 2"d reviews and
the items listed on this review.
II. Circuit Requirements

III. Disconnecting Means

IV. Wiring Methods
1) All PV system wiring must be made using one of the specific install ation/materials methods included in the
2010 California Electrical Code. (CEC §690.31, Chapter 3) Conduit run locations not specified on single line
diagram, please show locations on site plan or speck on single line diagram. No response or revision made
to plans identifying the location of conduits. See below for additional corrections.
2) Where direct current photovoltaic source or output circuits of a utility -interactive inverter from a building -
integrated system are run inside a building, they shall be contained in metallic raceways or enclosures. (CEC
§690.31 (E)). Revise plan as necessary.
3) Conduits exposed to sunlight on rooftops, where conductors or cables are installed, the adjustments shown in
Table 310.15(B)(2)(c) shall be added to the outdoor temperature to determine the applicable ambient
temperature for application of the correction factors in Table 310.16. (CEC §310.15(B)(2)(c)) Adjust
conductor sizes according to the proposed conduit locations.
V. Grounding
1) No corrections apply.
VI. Marking, Connection to Other Sources, Storage Batteries and Systems Over 600 Volts
1) The output of a utility -interactive inverter shall be permitted to be connected to the load side of the service
disconnecting means of the other source at any distribution equipment on premises. Where such a connection
is made, the interconnection provisions of CEC Section 690.64(B)(1) through (13)(7) shall apply. Verb main

service de -rating complies with Article 220 and provide load calculation on plans for justification.
Additionally the sum of the ampere ratings of overcurrent devices in circuits supplyingpower to a busbar
shall not exceed 120 percent of its rating —110A (50A + (3) 20A) > 120% of 75A (90A). See redlined load
calculation. Additionally 58-295 Carmona is not proposing to derate Main Breaker.
SECTION C - STRUCTURAL

SECTION D - MISCELLANEOUS COMMENTS
1) Red marks on Plans, even if not specifically mentioned in this list, indicate items needing correction. Revise
Plans as necessary and provide written response, noting where correction can be found.
2) Provide one copy each of the Site Plan and Floor Plan for the County Assessor's office. While not required
for resubmittal, this administrative item must be addressed prior to issuance of building permit.
3) Provide a current Certificate of Workers Compensation Insurance covering this project.
4) Provide proof of current City business license.
