Subject: TI for Dental Office = 78080 La Fonda (old LQAF building)
The subject building plans have been reviewed by CDD. We have signed the green sheet
to release the building permit. However, the following items will have to be fully satisfied
by the applicant prior to any temporary or final occupancy:
1. The existing landscaping has deteriorated and is in violation of the City's
requirements for property maintenance. Landscape plans for site remedial work
shall have been submitted to and approved by CDD prior to any final .inspection(s)
being called for, and installed and verified prior to release of any temporary or
final CO.
2. Applicant shall provide a minimum of 2 parking stalls at the rear of the property,
as were required by the original Plot Plan 87-375 approval. A site plan for all
landscape and parking improvements shall be provided. Applicant shall prepare
any required civil drawings for the parking stalls and joins to the north alley, as
may be . required by the Public Works Department. Plans shall have been
submitted to and approved by CDD and Public Works prior to any final
inspection(s) being called for, and installed and verified prior to release of any
temporary or final CO.
3. All roof mounted mechanical equipment must be installed below the horizontal
plane of the roof parapets, so as to not be visible above a horizontal line of sight,
as required under Section 9.100.050.6, LQMC.
Please ensure that the applicant receives a copy of these requirements with the permit.

Fire Department personnel have reviewed and approved the plans you submitted for the above
referenced project. Please be advised the following conditions apply as a part of the conditions for the
issuance of a building permit.
11) As may be necessary to maintain proper fire sprinkler protection due to constructions changes, fire
sprinkler system plans for the tenant improvement area may be required to be submitted to the Fire
Department for review, along with a plan/inspection fee. The approved plans, with Fire Department
Job card must be at the job site for all inspections.
2) Install Knox Key Lock box, Model 3200, mounted per recommended standard of the Knox
Company. If the building/facility is protected with a fire alarm system or burglar alarm system, the
lock boxes will require "tamper" monitoring. Special forms are available from this office for the
ordering of the Key Switch. This form must be authorized and signed by this office for the correctly
coded system to be purchased.
3) Install portable fire extinguishers per Tale 19, but not less than 2A1 OBC in rating. Contact certified
extinguisher company for proper placement of equipment.
4) Approved building address shall be placed in such a position as to be plainly visible and legible from
the street and rear access if applicable. Building address numbers shall be a minimum of 12" for
building(s) up to 25' in height, and 24" in height for building(s) exceeding 25' in height. All
addressing must be legible and of a contrasting color with the background.
Please contact the Fire Department Planning & Engineering staff for final inspection prior to occupancy.
Applicantrinstaller shall be responsible to contact the Fire Department to schedule inspections.
