Dear Larry:
On October 27, 2003, a Certified Asbestos Consultant from J&H Environmental, Inc. conducted a
limited inspection for asbestos -containing materials at the above referenced location.
The inspector is certified by the California Department of Health Services. The inspector extracted
three bulk samples from assumed asbestos -containing materials and forwarded them to Forensic
Analytical to be tested for asbestos -content. The following is a recap of this inspection and our
findings:
I. Executive Summary
1.01 There was asbestos found in the sampled materials.
1.02 J&H Environmental, Inc. recommends that prior to disturbing any areas that were not sampled,
these areas should be sampled to ascertain the presence of any and all asbestos -containing
materials and that all samples obtained be sent to an accredited laboratory for analysis.
II. Bulk Survey Limits
2.01 This report is not intended to identify all hazards or unsafe conditions or imply that others do
not exist, but is merely an indication of the condition of the suspect asbestos -containing
materials in the residence at the time of the survey.
III. Bulk Samples
3.01 Three samples of suspected asbestos -containing materials were taken and submitted to the
laboratory for analysis. The analyses were performed using the polarized -high microscopy
technique described in the EPA Interim Method of the Determination of Asbestos in Bulk
Insulation Samples (40 CFR - Part 763, Appendix A).
IV. Definition of Asbestos -Containing Building Materials
4.01 Asbestos -containing building materials are those who contain more than one percent (1%)
asbestos by weight. They are classified as follows:
1) Friable materials that may be crumbled, pulverized or reduced to powder by hand
pressure; -
2) Non -friable materials (Class 1), that may potentially be broken, crumbled, pulverized or
reduced to powder in the course of demolition or renovation activities; and
3) Non -friable materials (Class 11), that cannot be crumbled to powder by hand pressure.
V. Work Authorization
5.01 This report, alone, is not intended for asbestos abatement purposes. The survey and sample
analyses were performed and the report was prepared under authorization of Mr. Larry Russell.
Neither J&H Environmental, Inc. nor any of its employees, nor any contractors, subcontractors,
or any of their employees make any warranty, expressed or implied, or assume any legal
liability or responsibility for any third party use of or the result of such use of any information,
apparatus, product or process disclosed in this report, or represent that its use by such third
party would not infringe on private held rights.
5.02 The work completed has been performed on behalf of and exclusively for the use of Mr.
Russell. J&H Environmental, Inc. warrants that our services are performed within the limits
prescribed by the client, with the usual thoroughness and competence of the Environmental
Consulting and Testing profession. No other warranty or representation, either expressed or
implied is included.
5.03 The judgmental considerations necessary for the performance of this work have been made by
trained professionals, in accordance with generally accepted practices of Engineers and
Scientists undertaking similar duties and performing similar projects.
5.04 Although comprehensive in nature, results of this survey should not be extrapolated to include
areas not specifically mentioned.
VI. Friable Building Material
6.01 Friable building materials are those which can be crumbled to powder by hand pressure. Fiber
release is, therefore, more likely under these circumstances. The EPA indicates that the
following factors should be considered when assessing the potential for fiber release:
a) Condition of Material, evidence of physical and water damage, debris or deteriorating
connection to the substrate.
b) Potential for Future Disturbance, erosion or damage -proximity to air stream, visibility
and accessibility, type and intensity of activity in the area, sources of vibration, air
movement and building users.
VII. Non -Friable Building Materials
7.01 Non -friable materials cannot be crumbled to powder by hand pressure. If these materials are
intact and in good condition, fiber release is unlikely. Significant fiber release and asbestos
exposure can occur, however, if the surfaces of these materials are broken, drilled, sanded,
scraped, cut or otherwise abraded. If this occurs, the SCAQMD Board Rule 1403 requires that
asbestos -containing (Class 1) non -friable materials be treated as friable.

9.01 Federal, state and local laws require that building owner(s) and/or their representative(s), prior
to any demolition and/or renovation operation that may disturb any asbestos -containing
material(s) in their building(s), to meet the following requirements:
A. Notification;
B. Removal techniques for asbestos -containing material(s);
C. Clean-up procedures; and
D. Waste storage and disposal requirements.
9.02 Federal and state rules and' regulations required that asbestos removal be carried out only by
licensed and certified asbestos abatement contractors by the State Contractors License Board
and currently registered by the State of California Division of Occupation Safety and Health.

Where plywood panels are applied on both faces of a wall and nail spacing is less than 6" o.c. on either side, panel joints shall be offset from one
face to the other to fall on different framing members, or framing shall be 3" nominal or thicker, and nails on each side shall be staggered.
In seismic zones 3 & 4, where allowable shear values exceed 350 lbs. per foot, foundation sill plates, and all framing members receiving edge
nailing from abutting panels shall not be less than a single 3 -inch nominal member. In shear walls where total wall design shear does not exceed
600 lbs. per foot, a single 2 -inch nominal sill plate may be used, provided anchor bolts are designed for a load capacity of 50% or less of the
allowable capacity and bolts have a minimum of 2 -inch by 2 -inch by 3/16 -inch thick steel plate washers. Plywood joints and sill plate nailing shall
be staggered in all cases when applied on both faces of shear wall.
Products shall be manufactured with exterior glue and shall be identified with the words "exterior glue" following the product grade designation.
Framing at adjoining panel edges shall be 3" nominal or wider and nails shall be staggered where 10d nails, having penetration into framing of
more than 1-5/8", are spaced 3" or less on center.

Dear Skip,
Please be advised that the added window at the side of the house has no
adverse effect upon the structural integrity of the structure. The shear values of
the panels installed are well within the allowable maximum value for shear panel
designation. Also, the deletion of glu-lam beam #4 and substituting it with a girder
truss as calculated by the truss manufacturer is acceptable. The Building
Inspector may release the house for completion at the single -car garage and rear
addition. The front house addition was not inspected and is not released.

HERS Required Verification
These features must be confirmed and/or tested by a certified HERS rater under the supervision of a CEC approved HERS
provider. The HERS rater must document the field verification and diagnostic testing of these measures on a form CF -6R. Plan Field

JAI1. All ducts and plenums installed, sealed and insulated to meet the requirements of the 1998 CMC Sections 601, 603, 604 and
Standard 6-3; ducts insulated to a minimum installed level of R-4.2 or enclosed in conditioned space. Openings shall be sealed
with mastic, tape aerosol sealant or other duct- closure system that meets the applicable requirements of UL181, UL181A, or
UL181 B. If mastic or tape is used to seal openings greater than 1/4 inch, the combination of mastic and either mesh tape or
tape shall be used. Building cavities shall not be used for conveying conditioned air. Joints and seams of duct systems and
their components shall not be sealed with cloth back rubber adhesive duct tapes unless such tape is used in combination with
mastic and drawbands.
2. Exhaust fans systems have back draft or automatic dampers.
3. Gravity ventilating systems serving conditioned space have either automatic or readily accessible, manually operated dampers.
X1 §114: Pool and Spa Heating Systems and Equipment
1. Certified with 78% thermal efficiency, on-off switch, weatherproof instructions, no electric resistance heating, no pilot.
2. System is installed with at least 36" of pipe between filter and heater for future solar, cover for outdoor pools or spas.
3. Pool system has directional inlets and a circulation pump time switch.
FX §115: Gas fired central furnaces, pool heaters, spa heaters or household cooking appliances have no continuously burning pilot
light. (Exception: Non -electrical cooking appliances with pilot < 150 Btu/hr)
Lighting Measures
§150(k)l: Luminaires for general lighting in kitchens shall have lamps with an efficacy 40 lumens/watt or greater for general
lighting in kitchens. This general lighting shall be controlled by a switch on a readily accessible lighting control panel at an
entrance to the kitchen.
150(k)2: Rooms with a shower or bathtub must have either at least one luminaire with lamps with an efficacy of 40 lumens/watt
or greater switched at the entrance to the room or one of the alternative to this requirement allowed in Section 150(k)2.; and
recessed ceiling fixtures are IC (insulation cover) approved.
