March.7, 2011 Mr. & Mrs. Wade Gibbs 55-605 Pebble Beach La Quinta, CA 92253 RE: 55-605 Pebble Beach -Custom Home -Exterior. Renovation Page 1 of 2 Q 002 Dear Mr. & Mrs. Gibbs, The Architectural Committee has reviewed your Architectural Variance request. The proposed exterior Modification/Renovation as shown on the drawings and specifications prepared by Cutforth Technologists Limited are acceptable. The proposed scope of work is acceptable in, accordance with the Architectural Guidelines and the Homeowner's Association CC&R's. As a condition to this work, the following comments and/or conditions are applicatle: 1. Provide a copy of the City of La Quints building department permit prior to. starting any work. 2. Any damage to HOA property to be repaired and/or replaced to the satisfaction of the HOA prior to filing a Notice of Completion' form. 3. Upon completion, the homeowner is required to provide a signed copy, of -the city, inspection card and the Contractor Certification letter (enclosed). 4. The contractor cannot use the street for stockpiling construction material or debris. 5. The street gutters are not to be used'for concrete and construction material clean up. The street is to be broom -washed, as needed each day during construction. Neither debris nor dirt is to be washed down the gutter. 6. The street and work area are to be kept clean daily. 7. Contractor to observe all Association rules and regulations. All contractor vehicles to park on one side of the street only. 8. Contractor must provide traffic control supervised during activities in the street. 9. The color of the exterior stucco to be the same as existing per the sample on file. 10. The golf course access is not provided by the HOA. 11. All contractors to have clearance with the Master Association's gate security system. 12. HOA must be in receipt of contractors' current insurance certificates prior to starting any work. As of today, the HOA, has not received proof of auto coverage for D&R Repair. 54-320 Southern Hills, La Quinta. It is the understanding of the HOA that D&R Repair has no employees; as such, D& R Repair purports to be exempt from workers compensation coverage. Please provide a list of the sub -contractors and proof of their workers compensation insurance and their auto insurance, or D&R Repair's auto policy must insure &any auto' and non -owned autos. The obvious concern of the HOA is that all parties working on the job are properly insured to protect you, the homeowners and the Association. It is mandatory that the Association receive proof of insurance as required (see enclosed) prior to the start of work. Our insurance agent suggested that you consider adding `Owners and Contractors Protective' to your policy so that your general liability insurance covers. your sub- contractors. In the alternative, certificates of General Liability Coverage for each sub- contractor are required. We reserve the right to make a final inspection of this change to make sure that it matches the original request. An approval is based only on the aesthetics of your proposed pian and should not be taken as any certification as to the construction worthiness or structural integrity of the proposed change. We appreciate your cooperation in submitting this request for approval. .If I may be of further assistance, please contact me at the HOA office, (760) 771-1234, Extension 15. Storage and Shelf Life Both components should be stored in their original containers and away from excessive heat and moisture, especially after the seals have been broken or some materials have been used. Drums must be stored indoors and jobsite tanks maintained between 50°F and 75°F. Containers should be opened carefully to allow any pressure buildup to be vented safely while wearing full safety protection. Excessive venting of the 'B' component may result in higher density foam and reduced yield. Materials storedat temperatures below 50°F will increase in viscosity and some application equipment may not reach adequate spray temperature set points. Supply pumps and hoses must be sized to provide adequate supply when materials are cold and at a higher viscosity. Shelf Life: Excessive low or high temperatures may decrease shelf life. When stored in the original unopened container at 50°F -75°F, the shelf life of the "Part B" component is six months. Temperature above 75°F decreases the shelf life. The "Part A" component has a shelf life of 6 months in unopened containers when stored at 65°- 85°F. Surface Preparation Surfaces to receive PERMAX 3.0 must be clean and dry, free of dirt, oil, solvent, grease, loose particulates, frost, ice and other foreign matter which could inhibit adhesion. Moisture content and surface conditions of substrate are critical to adhesion of PERMAX 3.0 and need to be verified by installing contractor in small test areas before proceeding with full application. rriminq uDons. Plywood, OSB, and lumber shall not have greater than 15% moisture content. Generally a primer is not required for these surfaces. On substrates where the moisture content cannot be determined or exceeds 15%, a suitable primer is recommended. Adhesion spray tests may be performed with insulating foam and the interface line checked upon cure for good cell structure and adhesion. Warming of these surfaces during winter conditions may increase adhesion. CMU, structural and poured -in-place concrete must have a minimum 28 -day cure and moisture content below 15%. Painted Steel, galvanized steel, and aluminum panels: check surfaces for mill oil used in the manufacturing process and moisture condensate. All oil must be removed and the surface clean and dry before priming. Washed and dry painted steel panels may not require priming. All aluminum and galvanized panels must be primed using Sherwin Williams® DTM Wash Primer or Krylon@ Industrial CoatingsTm Water -Reducible Wash Primer. " Complete cure will depend on temperature, humidity and degree of ventilation. Complete cure usually occurs within 24-72 hours Climatic Conditions and Humidity Moisture in the form of rain, dew or frost can seriously affect the quality and adhesion of the PERMAX 3.0 to the substrate or itself. Henry Company does not recommend the spraying of this system when the relative humidity (RH) exceeds 85%. Application Equipment The proportioning equipment shall be manufactured specifically for heating, mixing, and spray application of polyurethane foam and be able to maintain 1:1 metering with a ±2% variance and adequate main heating capacity to deliver heated and pressurized materials up to 130°F. Heated hose must be able to maintain pre-set temperatures for the full length of the hose. Minimum 2:1 ratio feeder pumps are required to supply stored materials through minimum '/2 inch supply hoses. Pressurized and heated tanks systems may be used if sized appropriately to provide adequate flow at maximum operating capacity and temperatures. Older equipment may be upgraded with supplemental heaters or minimum H-2000 heater / proportioner to adequately pre -heat to process temperatures. Spray guns such as GX-7, GAP Pro Gun and Fusion gun, are well suited for roofing applications where 20 lbs/min or higher volume is desired. These guns may be fitted with smaller output tips (15 — 18 lbs/min.) to perform detail work on pipes, curbs, platforms and parapets etc. Processing Temperatures Recommended processing temperatures 'Part A' Main 100-120°F, 'Part B' Main 110-125°F, Hose 110-120°F are critical settings to achieve viscosity to allow balanced pressure during spraying. Balanced chemical output pressures are important to produce a good mix. Foam output pressures greater than 200 psi differential indicate either improper chemical temperatures, or worn gun/packing parts. Unequal pressures will cause poor chemical mixing through the module and uneven backpressure. Acritical requirement for good spray mixing requires appropriate tip/module sizing to the proportioner and adequate heating capacity. Unequal pressure (>200 psi) can cause excessive pump wear. Do not re -circulate the 'B' component for increased storage temperature as frothing or boil -over may occur at material temperatures above 60°F. Henry Company, 909 N. The foam plastic is RT -2035 2.5-3.0 polyurethane foam having a flame -spread rating of 75 or less when tested in accordance with ASTM E 84 (UL 723) at a maximum thickness of 4 inches (102 mm) and a maximum density of 3 pcf (48 kg /M3). The coatings are designated as Permax 108 and 115. Foam thickness and density, coating information, and roof covering classification for the various systems are shown in Table 1. The coated foam plastic roof coverings described in this report comply with the Resistance to Foot Traffic Test in Section 5.5 of FM 4470. 4.0 INSTALLATION 4.1 Deck Preparation: Roof surfaces must be free of grease, oil, dirt, sediment, moisture, and other foreign materials. All surfaces not to be covered with foam must be masked off or otherwise protected from overspray. All parapets, crickets and valleys must be flashed in accordance with the applicable code. Where precast concrete planks are present, all joints must be taped with minimum 2 -inch. -Wide (51 mm) roofing tape identified as DT -100. New plywood decks must be primed with an Acryprime- Substrate primer. New concreL- decks must be allowed to cure at least 21 days prior to application of foam. The surface is then primed with a penetrating epoxy primer, Urebond V. New metals. decks are primed with a two -component wash primer. Gaps in end or side laps must be sealed with an approved sealant. 4.2 Application of the Foam Plastic: The RT -2035 foam plastic is spray -applied to the prepared substrate in'/2-inch- to 1-inch4hick (12.7 mm to 25.4 mm) passes to the appropriate thickness as noted in Table 1. The wind velocity at time of application must not exceed 12 miles per hour (19.3 km/h), and the ambient temperature must be at least 60°F (330C). The foam plastic must not be applied over wet substrates or if rain or inclement weather is imminent. The foam plastic is applied in a one-to-one ratio with equipment capable of metering each component within t2 percent of the des,.gn ratio. The full specified thickness of foam plastic must be applied at one time. If the foam application is te-minated before the final thickness is attained, additional foam may be applied within 24 hours, provided the Bxisting foam is primed with Acryprime-Substrate primer end a minimum of 1/2 inch (12.7 mm) of foam is applied in a single pass. The foam plastic must be free of bumps, pin holes and ridges. Any such irregularities must be eliminated. The coating must not be applied if rain, fog or inclement weather is imminent or if temperatures below freezing are anticipated within 24 hours of the coating application. An optional top surfacing material may be applied over Systems 4 and 5 of Table 1. The material consists of No. 6 crushed limestone applied at a rate of 64 pounds per 100 square feet (3.1 kg/m2), embedded into the uncured top coating and covered with Permaguard roof mix applied at a rate of one batch per 100 square feet (9.29 m). One batch of Permaguard roofing mix consists of 40 pounds (18.1 kg) of dry mix, 41/2 gallons (17 L) of water, 1/2 gallon of Permaguard Acrylic Resin and 3/4 ounce (21.3 g) of Colloid 60. 4.4 Existing Roofs: The roofing system may be installed over existing roofs, provided the roof deck is as required for the system. The existing roof covering may remain in place if permitted by the building official. Roofing systems utilizing the Permax 108 coating, described in this report, may be installed over built-up, composition, or asphalt -shingle roof coverings and the roof classification will be as noted in Table 1. Prior to foaming, the roof surface must be prepared to assure adequate adhesion. All loose rock, cementitious coatings, peeling paint, dirt and debris must be removed by brooming, power vacuuming or wire brushing. The surface is primed with an Acryprime-Substrate primer at a rate of 200 square feet (18.6 m2) per gallon (3.78 L) and allowed to cure a minimum of one hour prior to foam application. Spray applications are subject to the same conditions as noted in Section 4.2. Where the existing roof covering is removed to the substrate, the deck is prepared as set forth in Section 4.1. 4.5 Wind Resistance: The systems installed in accordance with Sections 4.1 through 4.3 have allowable wind uplift pressures as noted in Table 2. 4.6 Perimeter Flashing: Edge securement for perimeter flashing shall be in accordance with IBC Section 1504.5 and the manufacturer's instructions. 5.0 CONDITIONS OF USE The Permax 108 and 115 Roof Coating Systems described in this report comply with, or are suitable alternatives to what is specified in, those codes listed in Section 1.0 of this report, subject to the following conditions: 5.1 Installation and application of the coated foam plastic roof coverings must comply with the applicable code, the manufacturer's published installation instructions, and this report.
