The Architectural Con-unittee has reviewed your Architectural. Variance request. The proposed
exterior Modification/Renovation as shown on the drawings and specifications prepared by
Cutforth Technologists Limited are acceptable. The proposed scope of work i3 acceptable in
accordance^with the Architectural Guidelines and the Homeowner's Association CC&R's.
As a condition to this work, the following comments and/or conditions are applicable:
1. Provide a copy of the City of La Quinta building department permit prior to- starting any
work.
2. Any damage to HOA property to be repaired and/or replaced to the satisfaction of the
HOA prior to filing a Notice of Completion' form.
3. Upon completion, the homeowner is required to provide a signed copy of the city
inspection card and the Contractor Certification letter (enclosed).
4. The contractor cannot use the street for stockpiling construction material-oa:.debris.
5. The street gutters are not to be used for concrete and construction material clean up. The
street is to be broom -washed, as needed each day during construction. Neither debris
nor dirt. is to be washed down the gutter.
6. The street and work area are to be kept clean daily.
7. Contractor to observe all Association rules and regulations. All contra.Aor vehicles to
park on one side of the street only.
8. Contractor must provide traffic control supervised during activities in the street.
9. The color of the exterior stucco to be the same as existing per the sample on file.
10. The golf course access is notprovided by the HOA.
11. All contractors to have clearance with the Master Association's gate security system.
12. HOA must be in receipt of contractors' current insurance certificates prior to starting any
work. As of today, the HOA has not received proof of auto coverage for D&R Repair.

13. It is the understanding of the HOA that D&R Repair has no employees; as such, D& R
Repair purports to be exempt from workers compensation coverage. Please provide a list
of the sub -contractors and proof of their workers compensation insurance and their auto
insurance, or D&R Repair's auto policy must insure `any auto' and non -owned autos.
The obvious concern of the HOA is that all parties working on the job are properly
insured to protect you, the homeowners and the Association. It is mandatory that the
Association receive proof of insurance as required (see enclosed) prior to the start of
work. Our insurance agent suggested that you consider adding `Owners and Contractors
Protective' to your policy so that your general liability insurance covers your sub-
contractors. In the alternative, certificates of General Liability Coverage for each sub-
contractor are required.
We reserve the right to make a final inspection of this change to make sure that, it matches the
original request. An approval is based only on the aesthetics of your proposed Flan and should
not be taken as any certification as to the construction worthiness or structural integrity of the
proposed change.
