GENERAL PROVISIONS
The following shall always apply:
ENCROACHMENT ON PRIVATE PROPERTY: This permit authorizes wore to be accomplished within City of La Quints right of way
ONLY. Whenever construction extenas within private property. it shall be the resoonsibdity of the oermmes for his contractorsl to
secure permission from abutting property owners. Such authorization must be secured by the permittee Prior to Starnng work.
TRACKLAYING CONSTRUCTION EQUIPMENT: Cleated tracklaving construction eQuitiment shall not be ciermittedtoopgrate an any
Paved surface unless fitted with smooth -faces street Dada. All mechanical outriggers shall be fitted with rubber street shoes to protect
tris paving during excavations. Rubber -tired equipment only shall be used in backfill operations in paved areas. If the existing
pavement is scarred. spelled, or broken during the term of this contract- or d the Pavement is marred City of La Quina shall request
that these Portions of road be resurfaced over their entire vcndth. Resurfacing shall consist of one coat of two inches (2") of A.C.
surfacing plus appropriate seal coat as specified above.
PROTECTION OF TRAFFIC: All excavations and work areas shall be property lighted and barricaded as deemed necessary by the City
Engineer or La Quints City Public Workslnspectors. Suitable detours and detour signs shad be placed and maintained for the dursuon
of the project. The City shall be notified 24 Mrs. in advance of any traffic detours or delinsatrorts.
CARE OF DRAINAGE STRUCTURES: Any drainage structure including corrugated metal pipe, concrete pipe $lest culvert and
concrete structures encountered during excavation which necessitate removal shad be replaced in kind. In the ewent.it becomes
necessary to remove or cut existing drainage structures. City of La Quints shall be ratified prior to commencement of
Drainage structures and open drains shall be kept free Of debt's at ad times for Proper drainage. this work.
RIGHT OF WAY CLEANUP: Any surplus material resulting from excavation and backfill operations shall be removed from the right of
way. All paved surfaces shall be broomed clean of earth and other objectionable materials immediately after backfill and compaction
Existing gutter line and drainage ditches shall be replaced to their original standard or better. All excess material stall be removed
prior to paving. Water tanker shall be used, as required to sprinkle the job site to keep down dust conditions and, shall be used
immediately after baccfill.

of producing an adequately consolidated backfill. Any materiel which the City of La Quinta deems unsuitable (apongyor
saturated material) which Is encountered during excavation shall not be used for backfill, but shall be supplemented or
replaced by an approved sand or gravel. _
R21 BACKFILL SAND: Backfill shall be approved transit -mix sand or equivalent and shall be placed in lifts of notgreaterthan
three feet (31 and vibrated using vibrotemper or eau iva tent equipment. Alternate methods maybe substituted, but in any
case a relative compaction Of 95 percent shall be attained within the c:rurtural section of the roadway.
BACKFILL PLACEMENT: Bacdill shall be applied in lavers of not more than 50 percent of the total depth of the trench
before flooding or a maximum of five-foot (51 lifts where trenches are of excessive depths. Care is to be exercised that the
backfill material is not subjected to exvsms swell by flooding operations. Backfill matenal shall be placed so that the
resulting compaction shall be not 1853 than 900ercem or eouivelentto the surrounding ground. whichever lathe greater
compaction. Where ponakng or flooding is used for a maximum settlement. adequate dikes will be constructed to retain

R28 FOG SEAL: A fog seal coat consisting of an application of asphaltic emulsion shall be applied over all patch areas as
determined by the City Engineer.
R29 SEAL ARMOR COAT: A seal coat consisting of an application binder and screenings shall be applied on all patch areas
where such seal coats existed prior to issuanceof this permit. Seal or armor Coats shall be applied as specified in Section
37, Standard Specifications. State of California. Division of Highways. dated January 1973. Work of pavement repair to
be accomplished by a licensed contractor engaged in the business of pavement repair.
STREET RESTRIANG: Where street striping is still visible on streets to be excavated. such striping shall be replaced upon
completion of permanent repairs.
R31 TREE RELOCATION OR REMOVAL: Tree relocation within the City of La Quinta road right of way shall be axomplished
by a licensed, bonded and insured tree service, and handled safely without interference or hazardto the vi veling public.
It shall be the responsibility of the permittee to maintain the tree in a vigorous growing condition at its new location.
Trees to be removed shall be removed in sectionswhich can be handled safelywithout interference or hazard to highway
traffic. The entire width of the tree stump snail be removed and disposed of so that no debris remains in view of the
. highway. The stump hole shall be backfilled and thoroughly compacted as specified in the following paragraph. Where it
becomes necessary to restrict traffic, the work shall be restricted to a maximum of 500 feet at any one time. Adequate
signs, flagmen and/or barricades shall be provided to protea the traveling public at all times.
Large holes resulting from tree removal shall be backfilled and compacted to not less than 90 percent or equivalent to the
surrounding ground, whichever is the greater compaction as determined by the impact or field method. Compaction
tests shall comply with Section 6.3.01 of the Standard Specifications. Division of Highways. State of California. dated
January 1973.
R32 TREE TRIMMING:
a. Only the necessary trimming shall be accomplished to provide clearance of the facility for one calendar veer.
All broken Or weak sections within the trimming snail be removed.
b. Where it becomes necessary to restrict traffic. the work shall be restricted to a maximum of 500 feet at anyone time.
Adequate signs, flagmen. and/or barricades Shall be provided to Protect the traveling public at all times.
C. If vee trunks are partially, outside of the City of La Quinta road right of way, permittee shall make his own
arrangements with adjoining property owners for consent to the trimming in addition to the authorization provided
by this permit.
d. All tree trimming on EVERGREEN TREES. PALM TREES. and other ORNAMENTAL TREES shall be accomplished to
the satisfaction of the City Public Works Inspector.
e. Abutting prOPWW Owners shall be contected, in all instances prior to trimming or topping trees.

In addition to the standard permit conditions, the following shall apply:
1. All work shall be performed in accordance with the latest edition of the
"Standard Specifications For Public Works Construction" and the plans approved
by the City of La Quinta.
No fill shall be placed which will cover or obscure asphalt concrete, petroleum
product spillage, or other non-native materials.
2. Permittee shall furnish the name(s) and phone number(s) of the individual(s)
responsible for maintaining the project site after hours, weekends, holidays,
etc.
3. Permittee shall.notify the City Engineering Dept. at 564-2246 at least 24 hours
in advance of starting construction.
4. Park Ave and 50th Avenue shall be kept clean of all trackage or spillage from
vehicles using this site. The roads shall be completely cleaned at the end of
each work day, if necessary, and more frequently if required.
5. Operation and maintenance of equipment within one half mile of human occupancy
shall be performed only during the following time periods:
October 1st to April 30: Monday -Friday 7:00 a.m. to 5:30 p.m.
Saturday 8:00 a.m. to 5:00 p.m.
May 1st to September 30: Monday -Friday 6:00 a.m. to 7:00 p.m.
Saturday. 8:00 a.m. to 5:00 p.m.
Equipment may not be operated or maintained on legal holidays including Sundays.
6. Permittee is responsible for repair of any pavement damage or other damage to
access. roads and other public facilities.
7. Should additional work or modifications be required in order -to meet established
City Standards or in order to fit field conditions, the work shall be performed
by the permittee as directed by and to the satisfaction of the City Engineer at
no cost to the City of La Quinta:
8. Any on-site haul road shall be effectively destroyed at the conclusion of the
work to prevent use by unauthorized vehicles.
9. Permittee/contractor shall install stop signs to stop construction traffic prior
to entering Park Ave.
10. A construction schedule along with a traffic control plan showing the element of
work for each day shall be submitted to the City Engineer for approval prior to
any work in the travel way. Field 'adjustments shall be accomplished as directed
by the Engineer to fit actual field conditions.
11. Streets shall be restored to normal traffic use at the end of each work day by
installing traffic plates over the trenches or using temporary A.C.
12. Permanent pavement replacement section on 50th Ave. and Park Ave. shall be 5.5"
of A.C. over 7" of A.B. and 4" of A.C. over 5" of A.B. respectively.
13. When work is within 500' of a signalized intersection , the permittee shall
notify Riverside County Signal Dept. at (714) 275- 6894 or Caltrans Signal Dept
at (619)-688-6845 as appropriate.
14. All traffic signal detector loops shall be protected in place, any damage shall
be the contractor/permittee's responsibility.

1(800)422-4133 and an "inquiry no." shall be provided to the public works
inspector.
16. In advance of excavation, contractor/permittee shall submit to the city for
acceptance a detailed plan , in accordance with labor code section 6705, showing
the design of' shoring, bracing, sloping,. or other provisions to be made for
worker protection from the hazard of caving ground during the excavation of any
trench greater than five feet in depth.
If such plan varies from the shoring system. standards, the plan shall be prepared
by a registered -civil engineer. As part of the plan, a. note shall be included
stating that the registered civil or structural engineer certifies that the plan
complies with the Cal OSHA Construction Safety Orders or that the plan is not
less effective than the Cal OSHA Construction Safety Orders.
17. Permittee shall notify the residents in the adjacent tracts that will be impacted
by the construction/detour a'minimum of 5 days prior to closing any streets.

TRACKLAYING CONSTRUCTION EQUIPMENT: Cleated traeklaving construction eQulpment shall not be permmedtooperate on any
Paved surface unless fitted with smooth -faces street pads. All mechanical outriggers shall be fitted with rubber street shoes to protect
the paving during excavations. Rubber -tired equipment only shall be used in backfill operations in paved areas. If the existing
Pavement is scarred. spelled. or broken during the term of this contract. or if the pavement n marred. City of La Quints shall request
that these portions of road be resurfaced over their entire width. Resurfacing shell consist of one coat of two inches (2") of A.C.
surtaging plus appropriate seal Coat as specified above.
PROTECTION OF TRAFFIC: All excavations and work areas shall be -property lighted and barricaded as deemed necessary by the City
Engineer or La Quinta City Public Worts Inspectors. Suitable detours and detour signs shall be placed and maintained for the duration
of the protea The City shall be notified 24 Hrs. in advance of any traffic detours or delineations
CARE OF DRAINAGE STRUCTURES: Any drainage structure including corrugated metal pipe, concrete pipe. steel Culvert and
concrete structures encountered during excavation which necessitate removal shall be replaced in kind. In the event.it becomes
necessary to remove Or cut existing drainage structures. City of La Quints shall be notified prior to commencement of this work.
Drainage structures and open drains shall be kept free of debris at all times for proper drainage.
RIGHT OF WAY CLEANUP: Any surplus material resulting from excavation and backfill operations shall be removed from the right of
way. All paved surfaces shall be broomed dean of earth and other objectionable materials immediately atter backfill and compaction.
Existing gutter line and drainage ditches shall be replaced to their original standard or better. All excess material shall be removed
prior to paving. Water tanker shall be used. as required, to sprinkle the job site to keep down dust conditions attd shop be used
immediately after backfill.
DE -WATER OPERATIONS: If de -watering operations are required and pumps aro forcing water an City of La Quinta roads, it shall be
the responsibility of the permmee (contractor) to control this water and to provide off-street barricades when neeessan,

R21 BACKFILL SAND: Backfill shall be approved transit-mix sand or equivalent and shall be placed in lifts of notgreaterthan
three feet 131 and vibrated using vibrotamper or ecu iva ient equipment. Alternate methods maybe substituted• but in any
case a relative compaction Of 95 percent shall be attained within the structural section of the roadway,
BACKFILL PLACEMENT: Backfill shall be applied in layers of not more tnan 50 percent of the total depth of the trench
before flooding or a maximum of five-foot 151 lifts where trenches are of excessive depths. Care is to be exercised that the
backfill material is not subjected to extreme swell by flooding operations. Backfill material shall be placed so that the
resulting compaction shall be not less than 90 percem or equivalent to the surrounding ground. whichever es the greater
compaction. Where ponding or flooding is used for a maximum settlement. adequate dikes will be constructed to retain
the water. Where letting is used, the jets shall be of sufficient length to reach the bottom of each :ager and the water
supply shall be cominucus.
COMPACTION TESTS: If so required bythe inspector. compaction tests shall be made at intervals of not more than 1,000
feet and a minimum of one (1) test on each road. One 11) Copy of each test shall be forwarded to the City Engineer for
approval and filed prior to making permanent repairs. Compaction tests shall be made asoutlined in Section 6.3.01 of the
Standard Specifications. Division of Highways. State of California, dated January 1973.
R24 COMPACTION TESTS: If so required by the inspector. Compaction tests shall be made for each crossing or service line.
One (1) copy of each test shall be f orwarded to the City Engineer for approval and filed prior to making permanent repairs.
Compaction tests shall be made asoutlined in Section 6.3.01 ofthe Standard Specifications. Division of Highways. State
of California. dated January 1973.
R25 TEMPORARY PAVEMENT REPAIRS: After completion of backfill and compaction operations. a temporary patch
consisting of 2 inches of SC-800 shall be placed on a prepared subgrade. The SC-800 temporary paving shall be placed
after a maximum of 3,000 lineal feet of trench has been excavated and backfill operations completed. but in no case shall
the placement of the temporary pavement exceed a five (5) day limit.
PERMANENT PAVEMENT REPAIR: After backfill and compaction operations have been completed. a temporary patch
consisting of two inches (2") of SC-800 shall be placed immediately. A permanent patch of
surfacing placed on a inch class base shall be placed no laterthan inches A.C.

R27 PAVEMENT REPAIR: F.A. S.: Upon completion of backf ill and compaction. a temporary Patch consisting Of two inches (2")

be placed on six inches (6") concrete base as directed. but not later than fifteen 0 5) days after compfetton Of temporary
Paving. Concrete base shall be of five-sack mix and shall conform to the City of La Quints Road Improvement Standards
and Specifications, Ordinance #461.
R28 FOG SEAL: A fog seal coat consisting of an application of asphaltic emulsion shall be applied over

R29 SEAL ARMOR COAT: A seal coat consisting of an application bender and screenings shall be applied on all patch areas
where such seal costs existed prior to issuance of this permit. Seal or armor coats shall be applied as specified in Section
37, Standard Specifications. State of California. Division of Highways, dated January 1973. Work of pavement repair to
be accomplished by a licensed contractor engaged in the business of pavement repair.
STREET RESTRIPING: Where street striping is still visible on streets to be excavated. such striping shall be replaced upon
completion of permanent repairs.
R31 TREE RELOCATION OR REMOVAL: Tree relocation within the City of La Quints road right of way shall be accomplished
by a licensed. bonded and insured tree service. and handled safely without mterference or hazard to the traveling public.
It shall be the responsibility of the permittee to maintain the tree in a vigorous growing condition at its new location.
Trees to be removed shall be removed in sections which can be handled safely without interference or hazard to highway
traffic. The entire width of the tree stump snail be removed and disposed of so that no debris remains in view of the
highway. The Stump hole shall be backfilled and thoroughly compacted as specified in thefollowing paragraph. Where it
becomes necessary to restrict traffic. the work shall be restricted to a maximum of 500 feet at any one time. Adequate
signs, flagmen and/or barricades shall be provided to protect the traveling public at all times.
Large holes resulting from tree removal shall be backfilled andcompacted to not less than 90 percent or equivalent to the
surrounding ground, whichever is the greater compaction as determined by the impact or field method. Compaction
tests shall comply with Section 6.3.01 of the Standard Specifications. Division of Highways. State of California. dated
January 1973.
R32 TREE TRIMMING:
a. Only the necessary trimming shall be accomplished to provide clearance of the facility for one calendar year.
All broken or weak sections within the trimming shall be removed.
b. Where it becomes necessary to restrict traffic, the work Shall be restricted to a maximum of 500 feet at anyone time.
Adequate signs, flagmen, and/or barricades shall be provided to protect the traveling public at all times.
c. If tree trunks are partially outside of the City of La Ouinta road right of way, permittee shall make his own
arrangements with adjoining property owners for consent to the trimming in addition to the authorization provided
by this Permit.
d. All tree trimming on EVERGREEN TREES. PALM TREES. and other ORNAMENTAL TREES shall be accomplished to
the satisfaction of the City Public Works Inspector.
e. Abutting property owners shall be contactso. in all instances. prior to trimming or topping trees.
f. Pruning shall. in all instances be at the shoulder of the crotch to insure even• fast healing in all cuts of two inches

In addition to the standard permit conditions, the following shall apply:
1. All work shall be performed in accordance with the latest edition of the
"Standard Specifications For Public Works Construction" and the plans approved
by the City of La Quinta.
No fill shall be placed which will cover or obscure asphalt concrete, petroleum
product spillage, or other non-native materials.
2. Permittee shall furnish the name(s) and phone number(s) of the individual(s)
responsible for maintaining the project site after hours, weekends, holidays,
etc.
3. Permittee shall notify the City Engineering Dept. at 564-2246 at least 24 hours
in advance of starting construction.
4. Park Ave and 50th Avenue shall be kept clean of all trackage or spillage from
vehicles using this site. The roads shall be completely cleaned at the end of
each work day, if necessary, and more frequently if required.
5. Operation and maintenance of equipment within one half mile of human occupancy
shall be performed only during the following time periods:
October 1st to April' 30: Monday -Friday 7:00 a.m. to 5:30 p.m.
Saturday 8:00 a.m. to 5:00 p.m.
May 1st to September 30: Monday -Friday 6:00 a.m. to 7:00 p.m.
Saturday 8:00 a.m. to 5:00 p.m.
Equipment may not be operated or maintained on legal holidays including Sundays.
6. Permittee is responsible for repair of any pavement damage or other damage to
access roads and other public facilities.
7. Should additional work or modifications be required in order to meet established
City Standards or in order to fit field conditions, the work shall be performed
by the permittee as directed by and to the satisfaction of the City Engineer at
no cost to the City.of La Quinta.
8. Any on-site haul road shall be effectively destroyed at the conclusion of the
work to prevent use by unauthorized vehicles.
9. Permittee/contractor shall install stop signs to stop construction traffic prior
to entering Park Ave.
10. A construction schedule along with a traffic control plan showing the element of
work for each day shall be submitted to -the City Engineer for approval prior to
any work in the travel way. Field adjustments shall be accomplished as directed
by the Engineer to fit actual field conditions.
11. Streets shall be restored to normal traffic use at the end of each work day by
installing traffic plates over the trenches or using temporary A.C.
12. Permanent pavement replacement section on 50th Ave. and Park Ave. shall be 5.5"
of A.C. over 7" of A.B. and 4" of A.C. over 5" of A.B. respectively.
13. When work is within 500' of a signalized intersection , the permittee shall
notify Riverside County Signal Dept. at (714) 275- 6894 or Caltrans Signal Dept
at (619)-688-6845 as appropriate.
14. All traffic signal detector loops shall be protected in place, any damage shall
be the contractor/permittee's responsibility.

15. 48 hrs prior to starting work Underground Service Alert shall be contacted at
1(800)422-4133 and an "inquiry no." shall be provided to the public works
inspector.
16. In advance of excavation, contractor/ permittee shall submit to the city for
acceptance a detailed plan , in accordance with labor code section 6705, showing
the design of shoring, bracing, sloping,, or other provisions' to be' made for
worker protection from the hazard of caving ground during the excavation of any
trench greater than five feet in depth.
If such plan varies from the shoring system. standards, the plan shall be prepared
by a registered civil engineer. As part of the plan, a note shall be included
stating that the registered civil or structural engineer certifies that the plan
complies with the Cal OSHA Construction Safety Orders or that the plan is not
less effective than the Cal OSHA Construction Safety Orders.
17. Permittee shall notify the residents in the adjacent tracts that will be impacted
by the construction/detour a minimum of 5 days prior to closing any streets.

I CERTIFY THAT THE DESIGN OF THE WATER SYSTEM IS IN ACCORDANCE WITH
THE REQUIREMENTS PRESCRIBED BY THE RIVERSIDE COUNTY FIRE DEPARTMENT
General District Notes for
Domestic Water
1. All materials and construction shall conform to the Standard Specifications
for the Construction of Domestic Water Systems (Standard Specifications) of
the Coachella Valley Water District (CVWD).
Unless modified by the Standard Specifications, the standards of the
American Water Works Associations shall apply to all materials and
construction methods.
2. All work shall be inspected by CVWD. The charges for inspection shall be
in accordance with CVWD's Regulations and Standard Specifications.
3. Work performed without inspection shall be rejected.
4. The minimum cover for water distribution PIpellnes and appurtenances shall
be 36 inches. Streets shall be brought to subgrade, sewer installed and
curb and gutter installed before pipeline is installed.
5. All connections to existing facilities will be made by CVWD at the owner's
expense. Sixty days notice required.
6. The separation between water and sewer facilities shall be as specified in
CVWD Standard Drawing W1.
7. Contractor shall contact the Chief Inspector, CVWD, at least 48 hours prior "
to start of construction to arrange a preconstruction conference.
8. An approved cleaning tool shall be pulled through the pipe during the
progress of its assembly to remove dirt, rocks or foreign materials.
9. No water or sewer pipelines shall be installed under decorative concrete,

streets shall be paved with asphalt concrete pavement.
10. These plans may be subject to review and/or revision, by CVWD, if
construction has not begun within 12 months from Date of Approval as shown
in CVWD Drawing Approval Blocks.

11. Contractor shall submit to CVWD a list of materials to be used prior to the
preconstruction conference. Only materials on the approved list may be
used. List shall include quantity, manufacturer, model, size, and other
pertinent data.
12. Contractor shall schedule his construction activities with the Chief
Inspector, daily, between 3:30 and 4:30 p.m. for the following working day.
If CVWD is unable to provide an inspector or inspectors, Contractor shall,
reschedule its work for another time.
If Contractor elects to cancel scheduled work without notice to CVWD,
Contractor shall be charged 1 hour of inspection time for each day of
occurrence.
13. Contractor shall call underground Service Alert at 1-800-422-4133 at least
two working days prior to start of construction.
14. Curb and gutter shall be in place before any domestic water pipelines are
constructed.
