Work shall be coordinated with the State or City of La Quints forces to preclude delay or interference with State or City of La Quints projects. 15 SURVEY MONUMENTS: Prior to excavation or beginning of construction, all survey monuments which exist on the centerline of all streets or property lines when involved shall be completely tied out so they maybe readily and correctly be replaced by a licensed civil engineer or surveyor at the exPertas of theDermittee. R18 PAVEMENT CUTTING: Pavement shall be saw cut prior to removal. Prior to final paving operations, any damage to Pavement straight edge shall be corrected. 19 LIMIT OF EXCAVATIONS: Excavations shall be limited to 1.000lineal feet of opentrench before backfill operations must begin. All excavations shall be properly barricaded with lights overnight. on weekends and holidays forthe protection of the traveling public. The Public Works inspector shall determine the suitability of excavation barricading in each case. No excavation shall remain open for a period exceeding five (5) days. No excavation shall be made unless the construction material is actually on the work site. R2 BACKFILL MATERIAL: Backfill shall be free of brush. roots or other organic substance detrimenta _ .;s use for Purposes of producing an adequately consolidated backfill. Any material which the City of La Quints deems unsuitable (spongy or saturated material) which is encountered during excavation shall not be used for backfill, but shall be supplemented or replaced by an approved sand or gravel. R21 BACKFILL SAND: Backfill shall be approved transit -mix sand or equivalent and shall be placed in lifts of notgreaterthan three feet (31 and vibrated using vibrotamper or equiva tent equipment. Alternate methods maybe substituted, but in any case a relative compaction of 95 percent shall be attained within the structural section of the roadway, / R2P BACKFILL PLACEMENT: Baddill shall be applied in layers at not more than 50 percent of the total depth of the trench before flooding or a maximum offivs-toot(51lihawheretrenchesareofexcessivedepths.Careistobeexe►cisedthatthe backfill material is not subjected to extreme swell by flooding operations. Backfill material shall be placed so that the resulting compaction shall be not less than 90 percem or equivalent to the surrounding ground. whichever is the greater compaction. Where jetting is used, the jets shall be of sufficient length to reach the bottom of each :ager and the water supply shall be cominuous. 23 COMPACTION TESTS: If so required bythe inspector. compaction tests shall be made at intervals of not more than 1.000 feet and a minimum of one (1) test on each road One (1) copy of each test shall be forwarded to the City Engineer for approval and filed prior to making permanent repairs. Compaction tests shall be made as outlined in Section 6,3.01 of the Standard Specifications, Division of Highways State of California, dated January 1973. R24 COMPACTION TESTS: If so required by the inspector, compaction tests shall be made for each crossing One (1) copy ofeach test shall beforwarded tothe City Engineer for approvalllbe and edeforeachcto gorservice line. The SC -800 temporary paving shall be placed after a maximum of 3.000 lineal feet of trench has been excavated and backfill operations completed, but in no case shall the placemem of the temporary pavement exceed a five (5) day limit. 26 PERMANENT PAVEMENT REPAIR: After backfill and compaction operations have been comp leted. a tem ora consisting of two inches 12'1 of SC -800 shall be placed immediately. R28 FOG SEAL: A fog seal coat consisting of an application of asphaltic emulsion shall be applied over all patch areas as determined by the City Engineer. R29 SEAL ARMOR COAT: A seal coat consisting of an application binder and screenings shall be applied on all patch areas where such seal coats existed prior to issuanceof this permit. Seal or armor coats shall be appliedas specified in Section 37, Standard Specifications. State of California. Division of Highways, dated January 1973. Work of pavement repair to be accomplished by a licensed contractor engaged in the business of pavement repair. R3 0 STREET RESTRIPING: Where street striping isstill visible onstreets tobeexcavated.suchstripingshallbereplecedupon completion of permanent repairs. R31 TREE RELOCATION OR REMOVAL: Tree relocation within the City of La Quinta road right of way shall be accomplished by a licensed. bonded and insured tree service. and handled safely without interference or hazard to the gaveling public. It shall be the responsibility of the permatee to maintain the tree in a vigorous growing condition at its new location. Trees to be removed shall be removed in sections which can be handled safelywithout interference or hazard to highway traffic. The entire width of the tree stump shall be removed and disposed of so that no debris remains in view of the highway. The stump hole shall be backfilled and thoroughly compacted as specified in the follows ng paragraph. Where it becomes necessary to restrict traffic, the work shall be restricted to a maximum of 500 feet at any one time. Adequate signs. flagmen and/or barricades shall be provided to protea the traveling public at all times. Large holes resulting from tree removal shall be backfilled and compacted to not less than 1 t percent or surrounding ground, whichever ii the greater compaction as determined equivalent to the rineithtests shall comply with Section 6Compaction .3.01 of the Standard Specifications, Division of Highways, State f California, dated January 1973. R32 TREE TRIMMING: a. Only the necessary trimming shall be accomplished to provide clearance of the facility for one calendar year. All broken or weak sections within the trimming shall be removed. It. Where it becomes necessary to restrict traffic, the work shall be restricted to a maximum of 500 feet at anyone time. Adequate signs. flagmen, and/or barricades shall be provided to protect the traveling public at all times. c. If tree trunks are partially outside of the City of La Quinta road right of way, permittee shall make his own arrangements with adjoining property owners for consent to the trimming in addition to the authorization provided by this Permit. d. All tree trimming on EVERGREEN TREES. PALM TREES, and other ORNAMENTAL TREES shall be accomplished to the satisfaction of the City Public Works Inspeao► e. Abutting property owners shall be contacted, in all instances, prior to trimming or topping trees f. Pruning shall, in all instances, be at the shoulder of the crotch to insure even. fast healing in ell cuts of two Inches (21 and above in size. Sealant shall be applied to all cut secttons as directed by the City Public Works in.-- SPECIAL CONDITIONS --PERMIT NO. 1809 ---CVWD--- In addition to the standard permit conditions, the following shall apply: 1. All work shall be performed in accordance with the latest edition of the "Standard Specifications For Public Works Construction" and the plans approved by the City of La Quinta. No fill shall be placed which will cover or obscure asphalt concrete, petroleum product spillage, or other non-native materials. 2. Permittee shall furnish the name(s) and phone number(s) of the individual(s) responsible for maintaining the project site after hours, weekends, holidays, etc. 3. Permittee shall notify the City Engineering Dept.•at 564-2246 at least 24 hours in advance of starting construction. 4. Park Ave and 50th Avenue shall be kept clean of all trackage or spillage from vehicles using this site., The roads shall be completely cleaned at the end of each work day, if necessary, and more frequently if required. 5. Operation and maintenance of equipment within one half mile of human occupancy shall be performed only during the following time periods: October 1st to April 30: Monday -Friday •7:00 a.m. to 5:30 p.m. Saturday 8:00 a.m. to 5:00 p.m. May 1st to September 30: Monday -Friday 6:00 a.m. to 7:00 p.m. Saturday 8:00 a.m. to 5:00 p.m. Equipment may not be operated or maintained on legal holidays including Sundays. 6. Permittee is responsible for repair of any pavement damage or other damage to access roads and other public facilities. 7. Should additional work or modifications be required in order to meet established City Standards or in order to fit field conditions, the work shall be performed by the permittee as directed by and to the satisfaction of the City Engineer at no cost to the City of La Quinta. S. Any on-site haul road shall be effectively destroyed at the conclusion of the work to prevent use by unauthorized vehicles. 9. Permittee/contractor shall install stop signs to stop construction traffic prior to entering Park Ave. 10. A construction schedule along with a traffic control plan showing the element of work for each day shall be submitted to the City Engineer for approval prior to any work in the travel way. Field adjustments shall be accomplished as directed by the Engineer to fit actual field conditions. 11. Streets shall be restored to normal traffic use at the end of each work day by installing traffic plates over the trenches or using temporary A.C. 12. Permanent pavement replacement section on 50th Ave. and Park Ave. shall be 5.5" of A.C. over 7" of A.B. and 4" of A.C. over 5" of A.B. respectively. 13. When work is within 500' of a signalized intersection , the permittee shall notify Riverside County Signal Dept. at (714) 275- 6894 or Caltrans Signal Dept at (619) 688-6845 as appropriate. 14. All traffic signal detector loops shall be protected in place, any damage shall be the contractor/permittee's responsibility. Special Conditions - Permit No. 1809 Page 1 of 2 15. 48 hrs prior to starting work Underground Service Alert shall be contacted at 1(800)422-4133 and an "inquiry no." shall be provided to the public works inspector. 16. In advance of excavation, contractor/permittee shall submit to the city for acceptance a detailed plan , in accordance with labor code section 6705, showing the design of shoring, bracing, sloping, or other provisions to be made for worker protection from the hazard of caving ground during the excavation of any trench greater than five feet in depth. If such plan varies from the shoring system standards, the plan 'shall be prepared by a registered civil engineer. As part of the plan, a note shall be included stating that the registered civil or structural engineer certifies that the plan complies with the Cal OSHA Construction Safety Orders or that the plan is not less effective than the Cal OSHA Construction Safety Orders. 17. If the work is to be performed by forces other than that of the utility company the contracting company shall be required to apply for additional encroachment permit and pay the appropriate fee. All materials and construction shall conform to the Standard Specifications for the Construction of Domestic Water Systems (Standard Specifications) of the Coachella Valley Water District (CVWD). Unless modified by the Standard Specifications, the standards of the F American Water Works Associations shall apply to all materials and construction methods. 2. All work shall be inspected by CVWD. The charges for inspection shall be in accordance with CVWD's Regulations and Standard Specifications. 3. Work performed without inspection shall be rejected. 4. The minimum cover for water distributJor pipelines and appurtenances shall be 36 inches. Streets shall be brought to subgrade, sewer installed and curb and gutter installed before pipeline is installed. 5. All connections to existing facilities will be made by CVWD at the owner's expense. Sixty days notice required. 6. The separation between water and sewer facilities shall be as specified in CVWD Standard Drawing W1. 7. Contractor shall contact the Chief Inspector, CVWD, at least 48 hours prior' to start of construction to arrange a preconstruction conference. 8. An approved cleaning tool shall be pulled through the pipe during the progress of its assembly to remove dirt, rocks or foreign materials. 9. No water or sewer pipelines shall be installed under decorative concrete, concrete, curbs or sidewalks without prior approval of CVWD. All in -tract streets shall be paved with asphalt concrete pavement. 10. These plans may be subject to review and/or revision, by CVWD, if construction has not begun within 12 months from Date of Appreval as shown in CVWD Drawing Approval Blocks. M 11. Contractor shall submit to CVWD a list of materials to be used prior to the preconstruction conference. Only materials on the approved list may be used. List shall include quantity, manufacturer, model, size, and other pertinent data. 12. Contractor shall schedule his construction activities with the Chief Inspector, daily, between. 3:30 and 4:30 p.m. for the following working day. If CVWD is unable to provide an inspector or inspectors, Contractor shall reschedule its work for another time. If Contractor elects to cancel scheduled work without notice to CVWD, Contractor shall be charged 1 hour of inspection time for each day of occurrence. 13. Contractor shall call underground Service Alert at 1 -800 -42? -4133 at least two working days prior to start of construction. 14.
