In addition to the standard permit conditions, the following shall apply:
1. All work shall be performed in accordance with the latest edition of the
"Standard Specifications For Public Works Construction". plans approved by the
City of La Quinta.
No fill shall be placed which will cover or obscure asphalt concrete, petroleum
product spillage, or other non-native materials.
2. Permittee shall submit a fugitive dust prevention plan in accordance with
Ordinance #219. The plan must meet the approval of the City Engineer prior to.
issuance of the permit.
3. Permittee shall furnish the name(s) and phone number(s) of the individual(s)
responsible for maintaining dust control.after hours, weekends, holidays, etc.
4. Permittee shall notify the City Engineering Dept. at 564-2246 at least '24 hours
in advance of starting construction.
5. All streets shall be kept clean of all trackage or spillage from vehicles using
this site. The roads shall be completely cleaned at the end of each work day,
if necessary, and more frequently if required.
6. Operation and maintenance of equipment within one half mile of human occupancy
shall be performed only during the following time periods:
October 1st to April 30: Monday -Friday 7:00 a.m. to 5:30 p.m.
Saturday 8:00 a.m. to 5:00 p.m.
May 1st to September 30: Monday -Friday 6:00 a.m. to 7.:00 p.m.•
Saturday 8:00 a.m. to 5:00 p.m.
Equipment may not be operated or maintained on legal holidays including Sundays.
7. Permittee is responsible for repair of any pavement damage or other damage to
access roads and other public facilities.
8. Should additional work or modifications be required in order to meet established
City Standards or in order to fit field conditions, the work shall be performed
by the permittee as directed by and to the satisfaction of the City Engineer at
no cost to the City of La Quinta.
9. It is the permittee's responsibility to comply with the California Regional Water
Quality Control Board's National Pollutant Discharge Elimination System (NPDES).
10. Any on-site haul road shall be effectively destroyed at the conclusion of the
work to prevent use by unauthorized vehicles.
11. Permittee/contractor shall install stop signs to stop construction traffic prior
to entering any street from the construction site.

for inspection and/or abatement purposes, and 1 agree to hold harmless the City and its
representatives from liability for any actions related to this project. I also agree to
complete the record-keeping requirements included herein as Attachment B; once the
project has been completed.

CONDITIONS FOR FRCP APPLICATION

1., The full amount of security furnished with the approved dust control plan
shall be available to the City for measure (s) deemed necessary by the City to
achieve adequate dust control even though such measures may be other than
those listed by the applicant on this application.
2. Applicant shall submit copies of all records/invoices related to cost accounting
for the dust control measures assumed during grading construction. Said
records shall be submitted within 30 days after completion of all dust control
measures outlined in the plan, including any measures made part of the plan
as a condition to its approval.
3. Paved roadways in areas to be graded shall be kept free of track -out and
flushed with water and/or swept in sufficient frequencies and amounts so as
to prevent blowing dust due to traffic on pavement. No track -out of material
shall be allowed onto streets outside of the areas proposed for grading. Any
track -out into these streets shall be swept or water -flushed by the end of the
work day, and shall be kept damp during hours of operation until such time
as it is removed.
4. Any imported/ exported materials shall be covered and/or wet down prior to
transport. If material is not covered, it shall be sufficiently stabilized
(watered down) so as not to spill or create dust during transport. Minimum
freeboard shall be as established by the Public Works Department.
5. Enforcement of these conditions shall be at the discretion of the assigned field
inspector, who shall verify in the field that the spirit and intent of the FDCP
and its attached conditions are being complied with. Adjustments to
application frequencies, quantities, and other aspects of this FDCP may be
made by the' inspector, at his/her discretion, to insure such compliance.

RE: Request for Dust Control Bond Release; TR 23269, TR 26188, TR 23935-5
In regard to the above requests, submitted under separate cover and dated 9/6/95, 9/7/95 and 9/12/95
respectively, please be advised that approval condition 93 for TR 26188 and TR 23935-5 reads as
follows:
" 3. Applicant shall submit copies of all records/invoices related to cost accounting for the dust
control measures assumed during grading construction. Said records shall be submitted within
30 days after completion of all dust control measures as outlined in the plan, including any
measures made part of the plan as a condition to its approval. Security may be withheld
pending submittal of this information. All such invoices shall be provided, along with the
required completion of the PM10 Program Report Form, which can be obtained from the
Public Works or Community Development Departments.".
You will need to submit cost accounting for dust control efforts for TR 26188 and TR 23935-5 in
order to authorize those bond releases. I have enclosed three copies of the Program Report form for
your use and filing for future use. These forms must be filled out as completely as possible, and be
accompanied by copies of any labor/material invoices pertaining to dust control work. The grading
contractor on these tracts should be able to provide documentation to satisfy this requirement.
TR 23269 has already had cost accounting information submitted to us on this form; therefore this
bond will be released The Public Works Department coordinates release of bonds for grading, dust
control and street improvements. Please contact Mr. Hector Guzman in that department regarding
the process for bond release. He can be reached at 619-777-7047.
