License Law for the following reason (Sec. 7031.5, Business and Professions Code: Any
city or county that requires a permit to construct, alter, improve, demolish, or repair
any structure, prior to its issuance, also requires the applicant for the permit to file a
signed statement that he or she is licensed pursuant to the provisions of the
Contractor's State License Law (Chapter 9 (commencing with Section 7000) of Division
3 of the Business and Professions Code) or that he or she is exempt therefrom and the
basis for the alleged exemption. Any violation of Section 7031.5 by any applicant for a[
permit subjects the applicant to a civil penalty of not more than five hundred dollars

CONTRACT AMOUNT: $156,996.00 (One Hundred Fifty -Six Thousand Nine Hundred Ninety -Six Dollars)
EXCLUSIONS: No items will be supplied nor work to be performed that is not specifically stated herein.
PAYMENT.TERMS: Ten percent ($15,699.60) deposit upon approval of the contract. Invoices to be paid
within 15 days of receipt. No retention is to be held.
WARRANTY
A 10 year Labor and Material warranty will be issued upon completion and final payment is received.
The work will be warranted by Roof Asset Management. Roof Asset Management SHALL NOT BE LIABLE
FOR SPECIAL, INCIDENTAL OR CONSEQUENTIAL DAMAGES. The acceptance of this proposal by the
Customer signifies his/her agreement that this warranty shall be.and is the exclusive remedy against
Roof Asset Management for all defects in workmanship and materials furnished by Rocf Asset
Management.
If you are in accordance with the conditions herein, please execute the agreement below. The contract
amount is subject to change 15 days from the date of this proposal. Thank you for your consideration of
