It shall be the responsibility of the.permittee to maintain the tree in a vigorous growing condition at its new location. Trees to be removed shall be in sections which can be handled safely without interference or hazard to highway traffic. The entire width of the tree stump shall be removed and disposed of so that no debris remains in view of the highway. The stump hole shall be backfilled and thoroughly compacted as specified in the following paragraph. Where is becomes necessary to restrict traffic, the work shall'be restricted to a maximum of 500 feet at any one time. Adequate signs, flagmen and or barricades shall be provided to protect the traveling public at all times. Large holes resulting from tree removal shall be backfilled and compacted to not less than 90 percent or equivalent to the surrounding ground, whichever is the greater compaction as determined by the impact or field method. Compaction tests shall comply with Section 6.3.01 of the Standard Specifications, Division of Highways, State of California, dated January 1973. COORDINATE WORK: The proposed work shall be subordinated to any operation which the State of California or City of La Quinta may conduct in this area during the period of this permit. Work shall be coordinated with the State or City of La Quinta forces to preclude delay or interference with State of City of La Quinta projects. R12 SURVEY MONUMENTS: Prior to excavation or beginning of construction, all survey monuments which exist on the centerline of all streets or property lines when involved shall be completely tied out so they may readily and correctly be replaced by a licensed civil engineer or surveyor at the expense of the permittee. A complete set of notes showing the ties to these monuments shall be furnished to the City Engineer prior to the removal of any monuments. This office shall be notified upon completion or replacement of all survey monuments for proper project clearance. R13 PAVEMENT CUTTING: Pavement will be mechanically saw cut to a straight to a straight edge prior to excavation. Method of pavement cutting shall be approved through the office of the City Engineer. (Under no circumstances shall excavating equipment be used to excavate prior to cutting of pavement.), Excavation material shall be placed in such a position as to best facilitate the general flow traffic. Prior to final paving operations, any damage to pavement straight edges shall'be corrected. �14 LIMIT OF EXCAVATIONS: Excavations shall be limited to 1000 linear feet of open trench before backfill operations must begin. All excavations shall be properly barricaded with lights holidays overnight, on weekends and for the protection of the traveling public. The Public Works Inspector shall determine the suitability of excavation barricading in each case. No excavation shall remain open for a period exceeding five (5) days. No excavation shall be made unless the construction material is actually on the work site. R15 BACKFILL MATERIAL: Backfill shall be free of brush, roots or other organic substance detrimental to its use for purposes of an adequately backfill. Any producing consolidated material which the City of La Quinta deems unsuitable (spongy or saturated material) which is encountered during excavation shall not be used for backfill, but shall be supplemented or replaced by an approved sand or gravel. R16 BACKFILL SAND: Backfill shall be approved transit-mix sand or equivalent and shall be'placed in lifts of not greater than three feet (T) and vibrated using vibrotamper or equivalent equipment. Alternate methods may be substituted, but in any case, a relative compaction of 95 percent shall be attained with the structural section of the roadway. BACCKFIICFE BLL PLACEMENT: Backfill shall be applied in layers of not more than 50 percent of the total depth of the trench before flooding or a maximum of five-foot (57 lifts where trenches are of excessive depths. Care is to be exercised that the backfill material is not subjected to extreme swell by flooding operations. Backfill material shall be placed so that the resulting compaction shall be not less than 90 percent or equivalent to the surrounding ground, whichever is the greater compaction. Where ponding or flooding is used for a maximum settlement, adequate dikes will be constructed to retain the water. Where jetting is used, the jets shall be of sufficient length to reach the bottom of each layer and the water supply shall be continuous. Rig COMPACTION TESTS: If so required by the inspector, compaction tests shall be made at intervals of not more than 1000 feet and a minimum of one (1) test on each road. One (1) copy of each test shall be forwarded to the City Engineer for approval and filed prior to making permanent repairs. Compaction tests shall be made as outlined in Section 6.3.01 of the Standard Specifications, Division of Highways, State of California,dated January 1973. R19 COMPACTION TESTS: If so required by the inspector, compaction tests shall be made for each crossing or service line: One (1) copy of each test shall be forwarded to the City Engineer for approval and filed prior to making permanent repairs. Compaction tests shall be made as outlined in Section 6.3.01 of the Standard Specifications, Division of Highways, State of California, dated January 1973. TEMPORARY PAVEMENT REPAIRS: After completion of backfill and compaction operations, a temporary patch consisting of 2 inches of SC-800 shall be placed on a prepared subgrade. The SC-800 temporary paving shall be placed after a maximum of 3000 linear feet of trench has been excavated and backfill operations completed, but in no case shall the placement of the temporary pavement exceed a five (5) day limit. R21 PERMANENT PAVEMENT REPAIR: After backfill and compaction have been completed, a temporary patch consisting of two inches (2') of SC-800 shall be placed immediately. A permanent patch of inches A.C. surfacing placed on a inch class base shall be placed no later than days after completion of temporary road repair. FOG SEAL: A fog seal coat consisting of an application of asphaltic emulsion shall be applied over all patch areas as determined by the City Engineer. It73/ STREET RFSTRIPING: Where street striping is still visible on streets to be excavated, such striping shall be replaced upon completion of permanent repairs. The following General and Special Provisions are attached to and made a part of Permit No. 0� GENERAL PROVISIONS The following shall always apply: ENROACEMENT ON PRIVATE PROPERTY: This permit authorizes work to be accomplished within City of La Quinta right of way ONLY. Whenever construction extends within private property, it is the responsibility of the permittee for his contractors to secure permission from abutting property owners. Such authorization must be secured by the permittee prior to starting work. TRACKLAYING CONSTRUCTION EQUIPMENT: Cleated tracklaying construction' equipment shall not be permitted to operate on any paved surface unless fitted with smooth -faced street pads. All mechanical outriggers shall be fitted with rubber street shoes to protect the paving during excavations. Rubber -tired equipment only shall be used in backfill operations in paved areas. If the existing pavement is scarred, spelled, or broken during the term of this contract, or if the pavement is marred, City of La Quinta shall request that these portions of road be resurfaced over their entire width. Resurfacing shall consist of one coat of two inches (2") of A.C. surfacing plus appropriate seal cost as specified above. PROTECTION OF TRAFFIC: All excavations and work areas shall be properly lighted and barricaded as deemed necessary by the City Engineer or La Quinta City Public Works inspectors. Suitable detours and detour signs shall be placed and maintained for the duration of the project. The City shall be notified. 24 hours in advance of any traffic detours or delineations. CARE OF DRAINAGE STRUCTURES: Any drainage structure including corrugated metal pipe, concrete pipe, steel culvert and concrete structures encountered during excavation which necessitate removal shall be replaced in kind. In the event it becomes necessary to remove or cut existing drainage structures, City of La Quinta shall be notified prior to commencement of this•work. Drainage structures and open drains shall be kept free of debris at all times for proper drainage. RIGHT OF WAY CLEANUP: Any surplus material resulting from excavation and backfill operations shall be removed from the right of way. All paved surfaces shall be broomed clean of earth and other objectionable materials immediately after backfill and compaction. Existing gutter line and drainage ditches shall be replaced to their original standard or better. All excess material shall be removed prior to paving. Water tanker shall be used, as required, to sprinkle the job site to keep down dust conditions and shall be used immediately after backfill. DE -WATER OPERATIONS: If de -watering operations are required and pumps are forcing water on City of La Quinta roads, it shall be the responsibility of the permittee (contractor) to control this water and to provide off-street barricades when necessary. CLOSING STREEETS: No street shall be closed. A minimum of one lane of traffic shall be maintained at all times to provide limiied access for the adjoining property owners and emergency vehicles. In the event it is felt by the permittee that he must close a street for any length of time, permittee shall contact this office to obtain the necessary permission. SPECIAL PROVISIONS The following shall apply when indicated: Rl NOTIFICATION: Permittee shall notify the City at (619)777-7075 at least 48 hours in advance of starting construction. R2 UTILITY CLEARANCE. (Substructures) Prior to making any excavation within the City of La Quinta right of way authorized by permit, the permittee shall contact all concerned utility companies relative to the location of existing substructures. Damage to existing substructures resulting from operations conducted under this permit shall be the sole responsibility of the permittee. / Ri) U1UJTY CLEARANCE: (Surface Structures) No work shall be done under this permit until all utilities are cher of the proposed work site. The permittee shall notify all concerned utility companies of the proposed work. R4 PAVEMENT WIDENING: Area between the proposed concrete gutter line and the existing road pavement shall be surfaced with inches of A.C. paving placed on inches of class _ aggregate subbase course having an "R" value of not less than and, in conformance with City of La Quints Road Improvement Standards and Specifications, Ordinance #461. RI PARKWAY GRADING: Area between the property line and top of the proposed concrete curb shall be graded to a slope of v l/4 inch to one foot (11. GRADE CHECKING: City of La Quints shall check grades upon receipt of plan and profile and/or grades as established by a licensed engineer. R7 CURB REMOVAL FOR DRIVEWAYS: A portion of the existing concrete curb and/or curb and gutter (_feet) shall be removed. Curb and/or curb and gutter shall be saw cut prior to removal. Depressed curb, matching concrete gutter and concrete driveway approach shall be constructed in conformance with City of La Quints Standard #207. R8 DRIVEWAYS: A.C. driveways shall be constructed as not to alter existing drainage pattern. Surfacing between the property line and the existing road paving shall be 2 1/2 inches of A.C. paving placed on of class aggregate base. Driveway construction shall conform to attached drawing. SIGHT CLEARANCE: Sight clearance of 600 feet in all directions shall be assured and maintained at all times. R10 SOH. STERIIJZ L The area to be surfaced shall be treated with soil sterilizer. Rate of application shall commply with the manufacturer's specifications. Special Conditions - Permit No. 2497 Page 1 of 2 fi SPECIAL CONDITIONS --PERMIT NO. 2497 --- CVWD.--- °In addition to the ,standard permit conditions, the following .shall apply: 1. All work shall be performed in accordance with the latest edition of the Standard Specifications For Public Works Construction. 2. Permittee shall be responsible for providing continuous dust and erosion control. 3. Streets shall be kept clean. They shall be completely cleaned at the end of each working day and more frequently if required. 4. Contractor shall comply with the local noise ordinance: Operation.and maintenance of equipment within. one half mile of human occupancy shall be performed only during the following time periods: October 1st to April 30: Monday -Friday 7:00 a.m. to'5:30p.m.. Saturday 8:00 a.m. to 5:00p.m. May 1st to September,30: , Monday -Friday 6:00 a.m. to 7:00p.m. Saturday -8:00 a.m. to 5:00p.ni. Work shall be prohibited ,on legal Holidays including Sundays. 5. Permittee/contractor shall assume responsibility for repair of any pavement damage to any public or private street and for any damage to. other City. streets or facilities as a result of work performed under this permit. 6. Advance warning signs and traffic control shall be installed and maintained in accordance with Cal Trans Standards or the Work Area Traffic Control Handbook (WATCH. Manual). It shall be the permittee's responsibility to detour and barricade this site. ' 7. Street closures shall not be permitted. A minimum of one travel lane on paved surface shall be maintained with flagmen at all times. .8. Prior to excavating, the permittee shall contact Underground Service Alert at 1(800) 422-4133. _ 9. Should additional work, materials, or modifications of the work be required in order to meet city standards, Safety • requirements, signage requirements, .or to fit actual field conditions, the work shall. be performed by the permittee as directed by and to the satisfaction'of the City Engineer at no cost to the City of La Quinta. All backfill and subgrade soil shall be compacted to 90% relative dry density, all backfill soil within the top 2' of finish pavement surface shall be compacted to 95 % relative dry density, all base material shall be compacted to 95% relative dry density. 11. All excavations at or near the travel way shall'be backfilled and paved at the end of every workday as directed by and to the satisfaction of the City Engineer for the protection of the traveling public. 12. All landscaping, irrigation, decorative rock, decorative concrete, lighting etc. shall be replaced to its original condition. 13.. Access and egress to all local properties shall be maintained at all times. 14. Permanent pavement replacement section shall be one inch greater than existing. 15. Permittee shall notify the City Engineering Department a minimum of 48 hours prior to any work in -the street (i.e. paved areas) at (619) 777-7075 when the work involves lane closure(s) in a signalized intersection or within- 500 feet of a signalized intersection. Permittee may be required to submit a site-specific traffic. control plan for'review and approval prior to commencing work at or near.a signalized intersection. 16. Work hours at signalized intersections or within 500 feet of a signalized intersection are typically from 9am _ 3pm to avoid peak traffic periods but may vary depending on the location of -the intersection. The permittee will be notified by the City of specific work hours (if different) during the 48 hour notice period. 17. All backfill shall be mechanically compacted in lifts of eight (8) inches or less. Jetting or flooding of backfill material will not be allowed. 18. Contractor/permittee shall pay for and provide soils compaction tests. Three (3) random tests shall be performed for every 300' of horizontal trench and three (3) random tests shall be performed for every 3° of vertical fill. 19. The applicant shall furnish proposed mix designs for asphalt concrete and P.C.C. if required for surface restoration and shall -not, begin permanent surface paving until the mix designs are approved by the Public Works Department. 20. A traffic control plan shall be prepared and submitted to the city for review a minimum of 48 hours prior the start of any construction in the street. 21. Contractor/permittee shall schedule a preconstruction meeting and notify all affected utility' companies, districts and agencies. 22. Contractor/permittee shall stabilize any soil that was disturbed (shoulder areas Etc.) as a result of work performed under this permit. 23. The permit fee for the contractor will be based on.a percentage of the contract cost for the portion of work in the City of La Quinta. Please find enclosed herewith plans for three signalized intersections and a storm drain. Signal loops and traffic control markings on the pavement that are destroyed during trenching will need to be replaced in kind in accordance with Caltrans standard specifications and standard plans. Trenches should be backfilled in' accordance with Los Angles County Public Works Standard Drawing 3081-0 (copy enclosed). Jetting or flooding of backfill material will not be allowed; all backfill shall be mechanically compacted in lifts of eight (8) inches or less. The permitte shall pay for, or.provde, two compaction tests performed at random deep and location in every 1000 -foot interval. The back fill material above the "b" bedding shall be 100% aggregate base material, unless soils reports provide evidence the native soil is adequate for certain portions or all of the trench length. If soil testing is performed it needs to be done so at 1.,000400t intervals and should identify the type of soil in accordance with'the Unified Soil Classification table and include such information as: maximum density, optimal moisture content, and existing moisture content. The replacement thickness of asphalt concrete. pavement shall be 1" thicker than the existing pavement; this will be field determined when the trench is open. Regarding proposed placement location for the sewer main, itis recommended that you avoid traffic lanes wherever possible, but if the sewer must go in a traffic lane, every, effort should be made to place it in the center of the ultimate traffic lane. The ultimate traffic lane is determined by applying the dimensions found on the City's adopted cross-section for the street in question with regard to the proposed sewer location. Special Condition No. 1 of the encroachment permit states that all work shall be performed per the Standard Specifications for Public Works Construction, ".Greenbook." The specifications in the "Greenbook" state that all surface improvements removed shill be reconstructed by the contractor to the same dimensions. , CVWD has created a safety hazard by allowing the contractor to construct a raised asphalt lip, creating a very unstable wheel path for vehicles. An additional problem that may occur is water accumulation at the asphalt lip. This will increase the risk of hydroplaning and cause the deterioration of the asphalt road surface. The City of La Quinta requests that the CVWD remove the feathered overlay, and complete. a "butt" joint overlay in the trench created by CVWD's operations. The matter was not resolved. 1/24/97 Field meeting referred to in the letter mentioned below 1/27/97 Letter to CVWD from Petar Artukovich Construction Company F regarding the City of LaQuinta, LaQuinta's encroachment permit, and La Quinta's request for a "buttjoint" for the paving cap in La Quinta. 1/27/97 Steve Robbins continued talking to representatives of LaQuinta. The matter was not resolved. 1/27/97 Gregg Green, CVWD inspector, again asked Hector to put LaQuinta's paving requirements in writing. Again this was not done. 1/27/97 Mr. Tom Levy called the City of LaQuinta to resolve the problem. He was unable to talk to the city manager until 1/28/97. 1/28/97. Washington St. was paved with a 10 foot wide cap starting at I a.m. and ending at 9:30 a.m. r 2/6/97 The City of Palm Desert Encroachment Permit required a butt join as shown in their permanent resurfacing details included in their encroachment permit, see Attachment G. The city of Palm Desert's inspector also notified the contractor on the first day of contraction in Palm Desert that a butt join was required. Therefore, the contractor placed the base paving below the existing surface elevation to achieve a butt join on the inside portion of the trench. The outside portion of the trench was feathered out to the curb and gutter. —> The Green Book reference on "same dimensions" in permanent resurfacing is as follows: "306-1.5.2 Permanent Resurfacing. Unless otherwise shown on the Plans or in the specifications, all surface improvements damaged or removed as a result of the Contractor's operations shall be reconstructed by the Contractor to the same dimensions, except for pavement thickness, and with the same type materials used in the original work. Trench resurfacing shall be I inch greater in thickness than existing pavement." (Attachment I is a copy of the Green Book page containing this reference). The pictures included in Attachment H were taken of January 31., 1997 just north of the intersection of Fred Waring Drive and Washington Street. You.can see that on the inside edge of the overlay the existing street is decomposing. The area of the existing street that is under the overlay was in a similar state of decomposition. The trench width was 4 feet wide. The overlay is 10.5 feet wide. Therefore the city of La Quinta received 6.5 feet of additional street improvements than the contractor would have had to provide. The overlay provided by the contractor meets the requirements of the Green Book section 306-1.5.2 above plus an additional 6.5 feet of improved road surface. A drawing was not included with the City of LaQuinta's Encroachment Permit which was issued to the contractor. The encroachment permit does not require a butt join. The permit does not require a particular type of final paving. The La Quinta City inspector, Hector Guzman , was present on the project site during mainline operations within the city of La Quinta. The dates of mainline construction within La Quinta are shown in Table 2. Hector witnessed the base paving of the trench during mainline operations. Hector did not question the height of the base paving, which was flush with the street. The first time that Hector requested a buttjoin .final A.C. cap was on January 24. If this request had been made earlier the base paving would have been left below the existing surface elevation as was done in Palm Desert. At the time the request for a butt join . was made, the base paving was complete; in order for the final cap to have a butt join the base paving would have to be ground down. Additional work is required to provide a buttjoin after the base paving had been allowed to be installed without being recessed. 2 2/6/97 Table 2. This project will have 3 -encroachment permits involved with the construction of the force main. a) Riverside County; 1) County requires Fees from the contractor and a security bond of $150,000, prior to start of construction. 2) Contractor has Allied.preparing traffic control plan to be submitted to District for County and cities review. 3) Subject of traffic stripping brought for discussion. Initially the county was to replace stripping as a cost to the contractor. But, it was brought to attention that the intersection of Country Club and Washington St. was being re -stripped for the pending construction to expand the Washington bridge over Interstate 10. This information was brought to the -meeting by Frank Petmecky of Riverside County road Dept. Frank also commented that -the stripping for bridge construction will occur in the next week. Frank provided a drawing of the new traffic stripping for the intersection of Country Club and Washington St.. The contractor will only have to replace the stripping in this area'that is damaged due to his construction. Remainder of project will be in'accordance with permit. 4) County inspector requested the trench be backfilled and temporary pavement placed at the end of each work day. Contractor proposed, his operation would be to backfill immediately; including base material and base paving on a daily basis. Inspector reserved resolution based upon visual review of contractors proposed operation. 5) Contractor opened a discussion regarding the bores. 2 -location, One on Country Club at 14+70 and the other on Washington St. at station 31+95. Contractor suggested changing the 30 -diameter casing to 28" diameter. It was recommended the contractor submit his suggestion to the Principal Engineer for review and consideration. 6) Traffic Controlwas briefly discussed for the Intersection of Fred Waring and Washington. One-half of the intersection is within the jurisdiction of the City of La Quinta and without the city being represented information of their needs would not allow discussion of traffic control to continue for this location. 7) Discussion regarding loop replacement of the traffic loops which are to be removed during construction of the force main. Unresolved as to' disposition of these loops. Pending field review by inspector. b) City of Palm Desert 1) City of Palm Desert was not represented at this meeting c) City of La Quinta 1) City.of La Quinta was not represented at this meeting. 2) Contractor brought up subject of Controlled Density Fill (CDF) as he had observed from notes provided in the permit addendum. Utilities/Mark and Locate; Contractor to contact USA and schedule a field meeting for utility location marking. Several utilities did not attend the meeting. A field meeting with the utilities is expected for mark and locate. Requested District inspector be present at -the utility field meet. Brief discussion regarding the relocation of gas main. It was recommended the contractor notify the gas co. prior to construction in this area for their relocation of the line or witness of contractors relocation. Contractor is expected to pothole utilities at least 1,000 feet ahead of construction. Contractor questioned his need to pothole if utilities had been potholed already. Contractor was advised previous pothole operation was for design only and it was required of him to pothole ahead of his operation for potential conflicts and resolution prior to construction. Question of unknown or unmarked utilities brought up. All were advised, there is the potential of unknown or unmarked utilities is there and should this occur each will be handled on a case by case basis. H. Technical Condition Review. , The technical conditions were reviewed for any -questions by the contractor. Items reviewed; 1) Mid-range pipe locators. a) For new pipeline, placement at every 600 -foot and at angle points. b) For existing pipe, placement at every 1200 -foot and at.angle points... Contractor was advised the placement of the mid-range locators must be verified that the locator.is placed directly over the existing pipe. This will require verification with the inspector. Also, the inspector and foreman need to agree on number of installations and as -built the locations. 2. Air and Vacuum Relief Assemblies; 1) Reviewed installation of Air. Vacuum assemblies -at Station 107+83. The new installation and the re -plumbing of the existing A/V to'the right of way and utilizing.the same vault and plumbing of both AN to odor control system. 2) The installation of A/V at station 17+30 is per standard details. 3. Steel Casing and Carrier Pipe;- 1) Refer to note 5) of Encroachment Permit and Dust Control Ordinance. 4. Hyrdostatic testing of Pressure Piping; 1) Contractor questioned testing of the pipeline against existing valves. a) Contractor was requested to submit testing against existing valves to principal engineer for review and consideration. Per principal engineer, at discretion of Senior and resident inspector. 2) Undetermined at this time what the operating pressure of line will be in order test pressure can be established. a) Per principal engineer test pressure = 150 PSI, TC -VII 7.2.1. 8 October 23, 1996 5) Gas Main Relocation; Contractor is advised to verify gas main relocation with gas company and to notify gas company prior to relocation. I. Survey; The contractor provides all the construction staking for the project. Contractor's surveyor is Sigland and Associates. Refer to sign in roster. Requested the surveyor provide cut sheets for inspector prior to construction. Cut.sheets will be Xerox copy of field book. Richard asked about monument documentation, Benchmarks and TBM's used during the design. He was advised to contact Tim Lytsell for benchmark information and Tri-State for benchmark and TBM control used for job layout. Also questioned the stationing layout? Again referred to Tri-State. J. Tie-in connection Points; The project has two tie-in connection points. The tie-in'connection point at the North end of the project requires the contractor to remove thrust block, blind flange and make connection to existing valve. As observed on the drawing construction note, there is a 35-CY thrust block. Size of thrust block is questionable. Contractor needs to verify. The other tie-in is at the South end of the project, just south of the Washington St. and Fred Waring intersection. This tie-in connection point will require scheduling the shutdown of the mains at the lowest flow time in order to provide the contractor the best.available window of construction for cutting the main, disposing of the sewage and installing the wye and gate valves as required per plans. Contractor questioned ifthe tie-in can be by tap -tee or at a different location. The contractor was advis he wanted to change the tie-in connection, he will have to propose/aro submit his ie -in to the District for the principal engineers review.
