In addition to the standard permit conditions, the following shall apply:
1. All work shall be performed in accordance with the latest edition of the Standard Specifications For
Public Works Construction:
2. Permittee shall `be responsible for providing continuous dust and erosion control.
3. Streets shall be kept. clean. They shall be completely cleaned at the end of each working day and
more frequently 'if required.
4: Contractor shall comply with restricted work hours. Operation and maintenance of equipment shall
be performed only during the following.time periods:
A. Work shall be limited between the hours of .9:00 A.M. and 3:.00 P.M. for all work areas
beyond five hundred (500) feet of the signalized intersection of Park Avenue and Avenue 50. `
Traffic control shall be set up and construction activity shall not begin earlier than'9:00 A.M.
Excavated trenches shall -be backfilled and temporarily paved and traffic control shall be
removed no,later than 3:00 P.M.
B. If construction work occurs within five hundred (500) feet of the signalized intersection of
Park Avenue and Avenue 50 when the Truman Elementary and/or the La Quinta Middle
Schools are in session, the work shall be further restricted to the hours of 4:00 P.M. and 12:00
A.M., Monday through "Saturday, or.9:00 A.M. and'3:00 P.M. Sunday, to avoid conflicts with
traffic generated by the schools.
Contractor shall make special arrangements,, including payment of overtime inspection fees,
two (2) working days prior to any work that is performed outside the normal weekeday hours
of 6:00 A.M. to 5:00 P.M..
In addition, the Permittee shall be responsible for issuing a City approved .letter to notify
adjacent residents that construction will occur in their area during late night hours. The letter
shall communicate to residents the purpose of the work period time restriction, the type of
construction to occur, and the actual time period in which residents can expect construction
to occur.
Traffic control shall be set up and construction activity shall not begin earlier than 4:00 P.M.'
Excavated trenches shall be backfilled and temporarily- paved':and traffic control shall be
removed no later than 12;00 A.M.
If the construction occurs in this area during the summer months when schools are not in-
session, work can proceed in this; area during the'restricted hours of 9:00 A.M. and 3:00"P.M.

5. It, shall be the Permittee's responsibility to coordinate with the Riverside County Traffic Signal
Maintenance Department, at (909) 275-6894, to alter the normal operation of the signals within the
intersection of Park Avenue. and Avenue ,50: These signals shall operate on a "flashing 'red" phase
during construction within, five hundred (5 00) feet of the intersection. They. shall operate on a .
manually -set timed phasing program during the interim period from. when the signal system is
affected by the removal of existing traffic loop detectors to the time when those loop detectors are
replaced in kind. The signal's operation shall be returned to its regular automatic phasing program
when the removed traffic loop detectors are replaced. The interim period of signal operation shall
not exceed seven (7). days from the completion of trench backfill work within the La Quinta city'
limits.
6. Permittee shall assume responsibility for repair of any pavement damage to any public or private
street and for any damage -to other City streets or facilities as a result of work performed under this
permit.
T Advance warning signs and traffic control shall be installed and maintained in accordance with Cal ,
Trans Standards•or the Work Area Traffic Control Handbook (WATCH Manual). A traffic control
plan shall be prepared in accordance with the WATCH Manual and submitted to the City for review
and approval one (1) week prior to starting any construction'. It shall be the Permittee's responsibility
to detour and barricade all construction sites. Traffic control set-up shall be inspected and approved
by .the City Inspector each day prior to commencement of construction activity.
8. Street closures shall not be permitted. A minimum of one travel lane on paved surface shall be
maintained with flagmen at all times. a
9. Prior to excavating, the permittee shall contact Underground Service Alert at 1-800-422-4133..
10. Should additional work, materials, ormodifications of the work be required in order to meet City
standards, safety requirements, signage requirements, or to fit actual field conditions, the work shall
be performed by the Permittee as directed by and to the satisfaction of the City Engineer at no' cost
to the City of La Quinta.
11. Open excavation and trenching shall be allowed throughout the length of the proposed 18" pvc sewer
.force main within the La Quint& city limits,, except for the following locations:
A. From the centerline of Avenue 50 (City limits), approx. station 752+35, to station 752+71.37.
B. From -station 771+13 to centerline of Avenue 50 (City limits), approx. station 771+53
C. From station 828+66 to station 830+95.40
D. From station 848+53 to station 848+90

Installation of the 18 pvc sewer force main, shall be performed by bore operation at the locations
specified above. No open excavations shall be, allowed. Existing street pavement shall not be
excavated nor compromised. Existing curb and gutter; sidewalk, median, and median landscaping
or other existing improvements, shall be protected in place to the extent possible. If removal. is
required, approval shall be granted by the City Inspector prior to removal. Improvements removed
shall be replaced in kind by the Permittee as directed by and to the satisfaction of the City Engineer .
no later than. seven (7) days after the completion of the trench backfill work within the La Quinta city
limits.
Allowance for an open excavation and pavement cut shall be allowed from.the centerline of Avenue
50 (City`limits), approxi station'803+50, to station. 804+40 through the Avenue 50defferson Street
intersection where a 45' bend is proposed, provided that adequate traffic control'is furnished, and
backfill and permanent pavement replacement is installed as required herein:
Backfill compaction within street rights-of-way, shall conform with Section 306-1.3 of the latest
edition of the Standard Specifications For Public Works Construction (SSPWC), except as.otherwide
specified herein.
Native material may be used as backfill material provided that minimum compaction, achieved in
the manner prescribed herein, is achieved. Backfill shall be performed by mechanical means; no
water densified compaction via jetting or flooding or other means shall be allowed: It.shall be the
Permittee's responsibility to provide appropriate geotechnical supervision, testing, and inspection,
onsite; at all times during backfill operations.
Backfill compaction shall be achieved by -the following conditions:
A. When a firm foundation is not encountered due to soft, spongy or.other unsuitable material,.
such material shall be removed to the limits directed by the Geotechnical Inspector and/.or the
City Inspector and the resulting excavation backfilled with pipe bedding material.
B. Backfill materials shall be brought to or maintained at an appropriate moisture'content for
compaction. The appropriate moisture content range shall be established at the onset of the
project and ensured prior to use of,the materials throughprogress testing on the stockpiles.
C. The first length of trench backfilled shall be backfilled in the presence: of the City Inspector.
Compaction shall be testedat random depths at. approximate three-foot vertical intervals as
backfill is placed to, ensure the effectiveness of the -compaction methods and to establish the
depth of lift that can, be compacted.
D. If lifts greater than one foot are attempted in this trial length of trench, compaction tests shall
be taken within.the lifts to ensure that the full depth of the lift is"compacted. Upon satisfactory

completion of this portion of trench, the City Inspector shall establish the maximum lift
thickness to be compacted for the remainder'of the project.
F. In subsequent, lengths of trench, the frequency of compaction.tests shall equal one per each lift
of backfill, per 300 linear feet of opentrench, at randomly selected locations within the open
length of trench.
G. The depth of the randomly selected compaction tests shall also be randomly selected except
that where multiple tests are required in a given length of trench, the tests shall be no closer
than three feet (vertically) from each other.
H. If any compaction test fails, previously placed backfill (in the same length of open trench or
previous lengths of trench) which is represented by the failing test (as determined by the City
Inspector), shall be tested for compliance with compaction requirements.
I. All costs incurred due to the conditions above shall be borne -,by the Permittee. Copies of all
test results shall be furnished to the City Inspector.
14. All excavations. within or near the travelway shall.be backfilled and temporarily paved at the end
of every workday as directedsby and to the satisfaction of the City Engineer for the protection of the
traveling public. Lengths of open trench shall not exceed that which can not be backfilled in the
same day. Open trenches shall not be allowed overnight. Steel plating shall not be allowed.
15. All landscaping, irrigation, decorative rock, decorative concrete, lighting, etc., shall be replaced to
its original. condition.
16. Access and egress to all local properties shall be maintained at•all times. .
17. Permanent`. pavement replacement shall be completed no laterathan seven (7) days after. the
completion of the trench backfill work within the La Quinta city limits. Permanent pavement
replacement shall conform to the following conditions, (see Avenue 50 Trench Detail):
A. Existing a.c. pavement shall be sawcut .one (1) foot beyond trench edges. Edges shall be
straight and clean.
B. Permanent pavement replacement structural section, within the trench, shall be one inch
greater than existing a.c. pavement, or 5'/2" minimum, whichever is greater., over V' of
aggregate base;material. The replaced pavement and base material shall be compacted to 95%
relative compaction and respectively tested, for each'300 linear feet of trench.
Note: existing a.c. pavement sections as thick as 14" have been- found within Avenue 50.
Permanent pavement replacement shall be installed -in lifts not to exceed 6" in depth, with each
lift of a.c: pavement compacted to 95% relative compaction and respectively tested, for each

along each edge of the cold -milled area.
D. Throughout the southerlyaone-half (t/2) of the Avenue 50/Jefferson Street intersection a new
one (1) inch a. c. pavement overlayy shall be'applied throughout the coldcmilled area as showri. ".
in Ahe Avenue -50/Jefferson Street Pavement Resurfacing Detail. The "'finished pavement
surface at each edge of this overlay area shall be smooth, consistent, and shall conform to the
surrounding grades.• Existing striping affected by this resurfacing (lane striping, "STOP `
legends and stop bars, etc.) shall be replaced in kind by the Permittee, as directed by and to the
satisfaction'of the City Engineer.
E. From.station 848+90 to 871+54, the existing pavement surface of the affected'lane of travel
of Avenue 50 (from adjacent to the southerly median curb through the lane width of twelve
j (12) feet) shall be cold -milled to a- depth of one (1) inch. The existing lane striping thirteen '
(13) feet from the southerly median curb, shall be preserved and protected in.place. Affected
traffic signal loops shall be removed and replaced in kind. A "butt" joint shall be created -
along the ,southerly edge"of the cold -milled lane width. `

20. The City has designated by ordinance .certain city streets as Truck Routes. The weight limit for
restricted streets (i.e., streets not part of the Truck Route network) is three (3) tons; trucks exceeding
Pe weight limit may use a restricted street if making a delivery or pickup on the subject street, or
if this permit specifically, grants permission to'use the street to deliver street construction materials
and/or equipment.

21. It shall be the Permittee's responsibility to submit to the City a faithful performance'bond, in the
amount of one hundred percent (100%) of the approved engineer's estimate or the awarded contract
amount for that portion of the construction within the La Quinta city limits, including costs
associated with conditions of such construction stipulated herein. Security shall be in a form and
from an institution approved by the City of La Quinta and shall be submitted to the City for approval
and acceptance, one (1) week prior to'starting any, construction. Security .shall be held. until '
successful• completion of the construction within the La Quinta city limits, and acceptance by the _
City Engineer of such.construction.

22. The City'of La,Quinta reserves the right to revoke, suspend or void this permit at any time.
23. This permit isnot for construction. It authorizes the installation of an 18" pvc.sewer force main in
locations and alignment indicated in the approved Coachella Valley Water District plans titled:
"Coachella Valley Water District Mid -Valley Water Reclamation Plant No. 4 (WRP4) Influent Force
Main No. 2, Segment 2:", dated May 15, 1997. This permit further provides: the conditions regulating
construction of this new utility within the La Quinta city limits.

In addition to the standard'permit conditions, the following shall apply:
All work shall be performed in accordance with the latest edition of the Standard Specifications For
Public Works Construction.
Permittee shall be responsible for providing continuous dust and erosion control
Streets shall be kept clean. They shall be completely cleaned at the end of each working day and
more frequently if required. „
Contractor shall comply with restricted work hours. Operation and maintenance of equipment shall
be performed only during the following time periods:
A. Work shall be limited between the hours of,9:00 A.M. and 3:00 P.M. for all work areas
beyond five hundred (500) feet of the signalized intersection of Park Avenue and Avenue 50.
Traffic control shall be set up and construction activity shall not begin earlier than 9:00 A.M.
Excavated trenches shall be backfilled and temporarily paved and traffic control shall be
removed no later than 3:00 P.M.
B. If construction work occurs within five hundred (500) feet of the signalized intersection of
Park Avenue and .Avenue 50 when the Truman Elementary and/or, the La Quinta Middle
Schools are in session, the work shall be further restricted to the hours of 4:00 P.M. and 12:00 .
A.M., Monday through Saturday, or 9:00 A.M. and 3:00 P.M. Sunday, to avoid conflicts with
traffic generated by the schools.
Contractor shall make special arrangements, including payment of overtime inspection fees,
two (2) working days prior to any work that is performed outside the normal weekeday hours
of 6:00 A.M. to 5:00 P.M..
In addition, the Permittee shall be responsible for issuing a City approved letter to notify
adjacent residents that construction will occur in their area during late night hours. The letter
shall communicate to residents the purpose of the work period time restriction, the type of
construction to occur, and the actual time period in which residents can expect construction
to occur.
Traffic control shall be set up and construction activity shall not begin earlier than 4:00 P.M.
Excavated trenches shall be backfilled and temporarily paved and traffic control shall be
removed no later than 12:00 A.M.
If the, construction occurs in this area during the summer months when schools are not in-,
session, work can proceed in this area during the restricted hours of 9:00 A.M. and 3:00 P.M.

5. It shall be the Permittee's responsibility to coordinate with the Riverside County Traffic Signal
Maintenance Department, at (909) 275-6894, to alter the normal operation of the signals within the
intersection of Park Avenue and Avenue 50. These signals shall operate on a "flashing red" phase
during construction within five hundred (500) feet of the intersection. They shall operate on a
manually -set timed phasing program during the interim period from when the signal system is
affected by the removal of existing traffic loop detectors to the time when those loop detectors are
replaced in kind. The signal's. operation shall be returned to its regular automatic phasing program
when the removed traffic loop detectors are replaced. The interim period of signal operation shall
not exceed seven (7) days from the completion of trench backfill work within the La Quinta city
limits.
6. Permittee- shall assume responsibility for repair of any pavement damage to ,any public or private
street and for any damage to other City streets or facilities as a result of work performed under this
permit.
7. Advance warning signs and traffic control shall be installed and maintained in accordance with Cal
Trans Standards or the Work Area Traffic Control Handbook (WATCH Manual): A traffic control
plan shall be prepared in accordance with the WATCH Manual and submitted to the City for review
and approval one (1) week prior to starting any construction. It shall be the Permittee's responsibility
to detour and barricade all construction sites. Traffic' control set-up, shall be inspected and approved
by the City Inspector each day prior to commencement of construction activity.
8. Street closures shall.not be permitted. A minimum of one travel lane on paved surface shall be
maintained with flagmen at all times.
9. Prior to excavating, the permittee shall contact Underground Service Alert at 1-800-422-4133.
10. Should additional work, materials, or modifications of the work be required in order to meet City
standards, safety requirements, signage requirements, or to fit actual field conditions, the work shall
be performed by the Permittee as directed .by and to the satisfaction of the City Engineer at no cost
to the City of La Quinta.
I.I. Open excavation and trenching shall be allowed throughout the length of the proposed 18" pvc sewer
force main within the La Quinta city limits, provided that at locations where trenching across the
existing paved surface of Avenue 50. is required, Permittee shall resurface Avenue 50 as required
herein, (see Avenue 50 Trench Detail #2). Existing curb and gutter, sidewalk, median, and median
landscaping or other existing improvements shall be protected in place to the extent possible. If
removal is required, approval shall be granted by the City Inspector prior to removal. Improvements
removed shall be replaced in kind by the Permittee as directed by and to the satisfaction of the City
Engineer no later than seven (7) days after the completion of the trench backfill work within the La
Quinta city limits.

12. Allowance for an open excavation and pavement cut shall be allowed from.the centerline of Avenue
50 (City limits), approx. station -803+50, to station 804+40 through the Avenue 505efferson Street
intersection wherea 45' bend is proposed, provided that adequate traffic control is furnished, and
backfill and permanent pavement replacement is installed as required herein.
13.' Backfill compaction within street rights-of-way shall conform with Section 306-13 of the latest
edition of the Standard Specifications For, Public Works Construction (SSPWC), except as otherwide
specified herein.
Native material may be used as backfill material provided that minimum compaction, achieved in
the manner prescribed herein, is achieved. Backfill shall be performed by mechanical means; no
water densified compaction via jetting. or flooding or other means shall be allowed. It shall be the
Permittee's responsibility to provide appropriate geotechnical supervision, testing, and inspection,
onsite, at all times during backfill operations.
Backfill compaction shall be.achieved by the following conditions:
A. When a firm foundation is not encountered due to soft, ,spongy or other unsuitable material, .
such material shall be removed to the limits directed by the Geotechnical Inspector and/or the
City Inspector and the resulting excavation backfilled with pipe bedding material.
B. Backfill materials shall be brought to or maintained at an appropriate moisture content for ..
compaction. The appropriate moisture content range shall be established at the onset of the
project and ensured prior to use of the materials through progress testing on the stockpiles.,
C. The first length of.trench backfilled shall be backfilled in the presence of the City Inspector.
Compaction'shall be tested at random depths at approximate three-foot vertical intervals as
backfill is placed to ensure the effectiveness of the compaction methods and to establish the
depth of lift that'can be compacted.
D. If lifts greater than one foot are attempted in this trial length of trench, compaction tests shall
be taken within the lifts to ensure that the full depth of the lift is compacted. Upon satisfactory
completion of this portion of trench, the City Inspector shall establish the maximum lift
thickness to be compacted for the remainder of.the project.
F. In subsequent lengths of trench, :the frequency of compaction tests shall equal one per each lift
of backfill, per 300 linear feet of open trench, at randomly selected locations within the open
length of trench.
G. The depth of the randomly selected compaction tests shall also be randomly selected except
that where. multiple tests are required in a given length of trench, the tests shall be no closer,
than three feet (vertically) from each other.

If any compaction test fails, previously •placed backfill (in the same length of open trench or
previous lengths of trench) which is represented by the failing test (as determined by the City
Inspector), shall. be tested for compliance with compaction requirements.
I. All costs incurred due to the conditions above shall be borne by the Permittee. Copies of all
test results shall be furnished to the City Inspector.
14. All excavations within City of La Quints right-of-way shall be backfilled, and temporarily paved if
within the existing travel way, at the end of every workday as directed by and to the satisfaction of
the City Engineer for the protection of the public. Lengths of open trench shall not greatly exceed
that which can not' be backfilled in the .same day. Excavated trenches shall not be allowed open
overnight, however, contractor may leave a length of excavated trench, not to exceed twenty (20) feet
in length, open overnight at a point where construction will begin the next day; provided that. this
length of trench is completely covered by steel plating.
15. All landscaping, irrigation, decorative rock, decorative concrete, lighting, etc., shall be` replaced to
its original condition:
16.' Access and egress to all local properties. shall be maintained at all times.
17. Permanent pavement replacement shall be completed no later than seven (7) days after the
completion of the trench backfill work within the La Quinta city limits. Permanent pavement
replacement shall conform to the following conditions, (see Avenue 50 Trench Detail):
A. Existing a.c.: pavement shall be sawcut one (1) foot beyond .trench edges. Edges shall be
straight and clean.
B. Permanent pavement replacement structural section, within all trenches, shall be 5'/z" of a.c.
over 6" of crushed aggregate base or crushed miscellaneous base, meeting the -requirements
of Sections 200-2.2 and 2062.4 of the Standard Specifications for Public Works Construction.
The replaced pavement and base material shall be compacted to 95% relative compaction and
respectively tested, for each 300 linear feet of trench.
C. From the centerline of Avenue 50 (City limits), approx. station 803+50, to station 804+40
through the Avenue 50/Jefferson Street intersection, the existing pavement surface of the.
southerly one-half('/2) of the intersection shall be cold -milled to a depth of one (1) inch. (See
Avenue 50/Jefferson Street Pavement Resurfacing Detail). A "butt joint shall be created
along each edge of the cold -'milled area.

D. Throughout the southerly one-half ('/2) of the Avenue 50/Jefferson Street intersection a new
one (1) inch a.c. pavement overlay shall be applied throughout the cold-milled'area as shown
in the Avenue 50/Jefferson Street Pavement Resurfacing Detail.The finished pavement
surface at each edge of this overlay area shall be smooth, consistent, and shall conform to the

surrounding. grades. Existing striping affected by this resurfacing (lane striping, "STOP"
legends and stop bars, etc.) shall be replaced in kind by the Permittee, as directed by and to the
satisfaction of the City Engineer.
E: From station X848+90 to 871+54, the existing pavernent surface of the affected lane of travel
of Avenue 50 (from adjacent to the southerly median curb through the lane width of twelve
(12) feet) shall be cold -milled to a depth of one (1) inch. The existing lane striping, thirteen
(13) feet from the southerly median curb, shall be preserved and protected in place. Affected
traffic signal loops shall be removed and replaced in kind. A "butt" joint shall be created
along the southerly edge of the cold -milled lane width.
F:, From station 848+90 to'871+54,'a new one (1) inch a.c. pavement overlay shall be applied
through the width of the cold -milled lane. Other. existing pavement striping, within the lane'
shall be replaced in kind by the Permittee, as directed by and to the_ satisfaction of the City
Engineer. The finished pavement surface at the southerly edge of this overlay shall be smooth,:
consistent, and shall conform to the surrounding grades.
G. Asphalt concrete mix design shall be approved by the City prior, to its placement.
H. The existing pavement surface, fifteen (15) feet on either side of the excavated. trench, shall
be cold -milled to a depth .of one (1) inch, with a "butt" joint created along each edge of the
cold -milled area, and a new one (1) inch a.c. pavement overlay applied throughout the cold -
milled. areas,, at the following locations:
l:. From the centerline of Avenue 50 (City limits),approx. station 752+35; to station
752+71.37
2. " From station 771+13 to centerline of Avenue 50 (City limits), approx. station 771+53
3. From station 828+66 to station 830+95.40
4. From station 848+53 to station 848+90
5. From station 871+54 to 872+04
Any existing lane striping affected by this resurfacing shall. be ` replaced in kind by the
Permittee, as directed by and to the satisfaction of the City Engineer. The finished_ pavement .
surface at the edges of the new one (1) inch overlay shall be smooth; consistent, and shall
conform to the surrounding grades. (See Avenue 50 Trench Detail 2)

until successful completion of the construction within the La Quinta city limits, and acceptance by
the City Engineer of such construction.
22. The City of La Quinta. reserves the right to revoke, suspend or void this permit at any time.
23. This permit authorizes the construction of an 18" pvc sewer force main within the Avenue 50 right
of-way as shown in the approved Coachella Valley Water District plans titled: "Coachella Valley.
Water District Mid-Valley Water Reclamation Plant No. 4 (WRP4) Influent Force Main No. 2,
Segment 2", dated May 15, 1997.
24. The Coachella_ Valley Water District (CV WD) shall be the sole permitteefor the proposed work.
All, responsibilities identified in the permit shall first accrue as a responsibility of CVWD.
Responsibilities contractually delegated by CVWD to a third party shall not relieve CVWD of its
ultimate responsibility for compliance with respect to this permit.
25. The Permittee shall pay a cash permit fee equal to 3% of the calculated bonding amount required -
in Condition #21:
26. The Permittee shall obtain insurance coverages. in accordance withthe indemnification and
insurance requirements specified in Engineering Bulletin 97-04 and provide an insurance certificate
naming the City of La. Quinta as an additional insured. .
