The Southern California Gas Company would like to inform the City of its
intention of replacing the gas main on Eisenhower Drive due to excessive leakage. During
conversation with Minio Garcia of this office, it was agreed to have our Sub -contractor,
ARB Construction, come into your &ffice and secure a permit , and pay a fee as specified
by City ordination.
This is the official notice, from the Southern California Gas Company, that the Did
price for completing this project will be approximately $41,000.00. We will follow all the
requirements of the City during the digging and the back -filling Process, and have them
inspected per your specifications.

In addition to the standard permit conditions, the following.shall apply:
1.
This permit authorizes the installation of approximately 2,600 linear feet of 2" plastic and steel gas
main, along Eisenhower Drive, from Calle Tampico to 50th Avenue, as shown in the drawings for
WR#273672. Installation shall be made by bore operation whenever possible:

All work shall be performed in accordance with the latest edition of the Standard Specifications For
Public Works Construction (SSPWC).
3..
ARB Inc., "Permittee", shall be responsible for providing continuous dust and erosion control. The
Permittee shall stabilize any soil that is left disturbed as a result of work performed under this permit .
with an approved dust control treatment. The City shall have the right to stop work in progress in
the event, dust control standards are not met. The Permittee shall not proceed with work until dust
levels are reduced and clearance is given by the City Inspector.
4.
Streets.shall be kept;clean. They shall be completely cleaned at the end of each working day and
more frequently if required..
5.
The Permittee.shall comply with the local noise ordinance. Operation and maintenance of equipment
within one half mile of human occupancy shall be performed only during the, following time periods:
October 1 st to April'30 Monday -Friday 7:00 a.m. to 5:30 p.m.
Saturday 8:00'a.m. to 5:00 p.m.
May l st to September 30: Monday -Friday 6:00 a.m to 7:00 P.M.
Saturday 8:00 a.m. to 5:00 p.m.

Work performed within 500 feet.of the signalized intersection of Eisenhower Drive and 50th Avenue
and within the intersection of Eisenhower Drive and Calle Tampico is restricted between the hours
of 9 a.m. - 3 p.m. ;Traffic control shall be set up after 9 a.m. and removed before 3 p.m. The .
Permittee shall contact the Riverside Country Traffic Signal Maintenance Department at (909) 275-
6894 if signal operation at the Eisenhower Drive/50th Avenue intersection is to be altered in any
way.
7.
Work.shall be prohibited on legal holidays including Sundays. Work is allowed on.Saturdays only,
if special arrangements, including payment of overtime inspection fees, have been made two (2) .
working days prior to the desired Saturday. ,
8.
The Permittee shall assume responsibility for repair of any pavement damage to any, public or private
street and for any damage to other City streets or facilities as a result of work performed under this
permit. Upon notification, the Permittee shall respond immediately to correct said damage at its own
expense.

9. Advance warning signs and traffic control shall be installed and maintained in accordancewith Cal
Trans Standards or the Work Area Traffic Control Handbook (WATCH Manual). The Permittee
shall submit a traffic control plan that shall provide for a minimum of one travel lane on paved -
surface with a flagman as necessary. Access and egress to all local properties shall be maintained
at all times. It shall be the Permittee's responsibility to detour and barricade the work site.
10. Prior to excavating, the Permittee shall contact Underground Service Alert at 1-800-422-4133.
11. Should additional work, materials, or modifications of the work be required in order to meet city
standards, safety requirements, signage requirements, or to fit actual field conditions, the work shall
be performed by the Permittee as directed by and to the satisfaction of the City Engineer at no cost
to the City of La Quinta.
12. All excavations within City of La Quinta right-of-way shall be. backfilled, and temporarily paved if
within the existing travel way, at the end of every workday as directed by and to the satisfaction of
the City Engineer for the protection of the public. Lengths of open -trench shall not greatly exceed ,
that which can not be backfilled in the same day. Excavated trenches shall not be allowed open
overnight, however, the Permittee may leave a length of excavated trench not to exceed twenty (20)
feet open overnight at a point where construction will begin the next day, provided that this length
of trench is completely covered by steel plating.
13. Backfill compaction within improved street rights-of-way shall conform with Section 306-1.3 of the
SSPWC. Native material may be used as backfill material provided that all trench backfill up to
subgrade level is compacted to a minimum relative density of 90% with a minimum frequency of
one test per every 300 linear feet per lift. All backfill shall be mechanically compacted in lifts of
eight (8) inches or less: Backfill shall be performed by mechanical means; no water densified ,
compaction viajetting or flooding or other means shall be allowed. It shall be the Permittee's
responsibility to provide appropriate geotechnical supervision, testing, and inspection, onsite, , all
times during backfill operations. The Permittee shall pay all costs for geotechnical testing of backfill
required by the City. All test data shall be submitted to the City.
14. Where trenching is performed in any P.C.C. or A.C. improved area the trench shall be saw cut an
additional 1' on each side of the excavation area and a butt joint created. The edges shall be'saw cut
to straight, stable, and unyielding edges. Feather joints are not acceptable. The finished pavement
surface shall be smooth and consistent, and shall conform to the surrounding grades. Trenches in
the street section shall be permanently paved within seven (7) days after completion of the work
unless otherwise approved by the City.
15. Permanent pavement replacement structural sections within Eisenhower Drive, Calle Tampico, and
50th Avenue shall be one inch greater than existing of asphalt concrete or 5t/z" minimum, whichever
is greater, over 6" of aggregate base material. The asphalt concrete` and base material layers shall
be compacted to a minimum relative density of 95%. The asphalt concrete mix design shall be
approved by the city prior to placement. '

16. All landscaping, irrigation lines, decorative rock, decorative concrete, lighting, etc., shall.be replaced
to its original condition. Existing landscaping adjacent to the La Quinta Hotel frorri the north side
of the CVWD channel to Avenida Mazattan shall be protected. Existing curb and gutter, sidewalk,
medians or other existing improvements shall be protected in place to the extent possible. If removal
is required, approval shall be granted by the City Inspector prior to removal. Improvements removed
shall be replaced in kind by the Permittee as directed by and to the satisfaction of the City Engineer
no later than seven (7) days after the completion of the work authorized. by this permit.
17. The Permittee shall notify the City Engineering Department a minimum of 48 hours prior to start of
any work at (760) 777-7075.
18. The city has designated by ordinance ,certain city streets as Truck Routes. The weight limit for
restricted streets (i.e. streets not part of the Truck Route network) is three,(3) tons; trucks exceeding..
the weight limit may use the restricted street if making a delivery or pickup on the subject street, or
if this permit specifically grants permission to use the street to deliver street construction materials
and/or equipment.
19. The City of La Quinta reserves the right to revoke, suspend or void this permit at any time.

Eisenhower Drive/Avenue SD Intersection
Dear .
Pursuant to our telephone conversation yesterday, this letter is to advise you that as a result of an
oversight by your company, several traffic signal loop lead wires were severed at the Eisenhower
Drive/Avenue 50 intersection. The Riverside County Traffic- Signal Maintenance Department has
been called by City staff to investigate the extent of the damage to the traffic signal facilities and to
identify the location of any other traffic signal facilities or existing conduits in that intersection.
However, those locations may only be approximately located; your company should exercise great
care when excavating within this intersection.
Due to the fact,that your company has, decided to open trench across Eisenhower. Drive, as well as
longitudinally within the right-hand lane; additional trench resurfacing conditions shall be imposed
on ARB, Inc., which. were .not identified in the original permit. Those conditions are as follows:
Pursuant to Section 14.16.3 75 of the La Quinta Municipal Code (Ordinance 10 § 1 (part), 1982),
permanent pavement replacement shall be completed no later than fourteen (14) days after the
completion of the trench backfill work within the La Quinta city limits. Permanent pavement
replacement shall conform to the following conditions:
A. Existing asphalt concrete pavement shall be sawcut one (1) foot beyond trench edges.
Edges shall be straight and clean.
B. Permanent pavement replacement structural section, within all trenches, shall be 5'/2"
of asphalt concrete over 6" of crushed aggregate base or crushed miscellaneous base,
meeting the requirements of Sections 200-2.2 and 200-2.4 of the Standard
Specifications for Public Works Construction. The replaced pavement and base
material shall be compacted to 95% relative compaction and respectively tested, for
each 300 linear feet of trench.
C., The existing pavement surface of the affected lane of travel of Eisenhower Drive in
which the gas main is being installed shall be cold -milled to a depth of one (I).inch _
throughout the entire width of the traffic lane. Affected traffic signal loops shall be, *_12

removed and replaced in kind. A "buttjoint shall be created along -each edge of the
cold= milled lane.
D. Throughout the cold -milled lane described in Paragraph C, a new one (1) inch asphalt
concrete pavement overlay shall be applied. Existing .pavement striping within the
lane shall be replaced in kind by the Permittee, as directed by and to the satisfaction
of the City Engineer. The finished pavement surface at each edge"of this overlay
shall. be' smooth, consistent; and shall conform to the surrounding grades.
E. For trenches that are excavated transverse across Eisenhower Drive, the existing .
pavement surface, fifteen (15) feet -on either side of the excavated trench, shall be
cold -milled to a depth of one (1) inch, with a "butt" joint created along each edge of
the cold -milled area, and a new one' (1) inch asphalt .concrete. pavement overlay
applied throughout the cold -milled area: Affected, traffic signal, loops shall be
removed. and replaced in kind. Existing pavement striping shall be replaced in kind
by the 'Permittee, as directed by and to the satisfaction of the City Engineer. The
finished pavement surface'at the edges of the overlay.shall be smooth, consistent; -and
shall conform to the surrounding grades.
F. Asphalt concrete mix design shall be approved by the City prior to its placement.
Riverside County Traffic Signal Maintenance personnel will identify which of the traffic signal loops
in this intersection have been severed. Once they are identified, it will be`ARB's responsibility to
remove and-re'lace these traffic signal loops as soon as possible: In addition, any service fees
incurred by the City from the Riverside County Traffic Signal Maintenance Department shall be
