ADDENDUM NUMBER 1
The following shall be considered as incorporated into the Request for Proposal (RFP) for the above
referenced services. Portions of RFP not specifically mentioned in this Addendum remain in force.
■ REQUEST FOR PROPOSAL
Question: Insurance and Acknowledgement: Could the City please clarify whether they are requesting
the cost to the consultant or the cost to the City?
Response: We are requesting the cost to the City.
uestion: Insurance and Acknowledgement: Could the City please clarify what is meant by "form"? Is
there an actual form that needs to be completed or does "form" refer to the types and limits of
coverage? If the former, will the form be provided on the City's RFP site? If the latter, please clarify if
an acknowledgement letter from our insurance broker will suffice or do the acknowledgement letters
need to be from each individual insurance company?
Response: The language is referring to the types and limits of coverage and not an actual form.
Question. Due Date and Time: Could the City please clarify if the cover letter, table of contents, and
appendix count toward the 20-page limit?
Response: The cover letter, table of contents and appendix will not count towards the 20-page limit.
uestion: Due Date and Time: Could the City please clarify where in the proposal this subsection
should be placed? Will this subsection count toward the 20-page limit?
Response: The subsection can be placed at the end of the proposal and will not count towards the 20-
page limit.
Question: Non -Collusion Affidavit Form: Should the Non -Collusion Affidavit form be included in the
Appendix or placed in some other proposal section? Will the form count toward the 20-page limit?
Response: The Non -Collusion Affidavit can be placed at the end of the proposal and will not count
towards the 20-page limit.

The Contractor is hereby notified; Addendum No. 1 must be acknowledged as stipulated under the
instructions to Bidders and submitted as part of the Bid. Failure to do so shall result in the City
designating said bid as "Non -Responsive."
