I understand It is the property owner's responsibility to submit copies of weight tickets or receipts to the District
Environmental Coordinator as these hauls occur. I hereby certify that completion, implementation and adherence of the
Debris Management Plan (DMP) for the above named project shall guarantee that at least 65% of the jobsite waste is

We were recently informed during our 2nd Plan check correction (resubmittal) that our
application has become expired and a request for an extension is required. The Clients,
Michael and Amber Townsend, were in the process finishing another home as an income
project, along with the contractor, Michael Tuvell and we all just lost track of the
timeline in the process.
At this point we only have a few corrections and are diligently working to get them
corrected (an (11) item list from AJ Ortega received June 11). A few of the items are just
small paperwork and a few are requiring a bit more work from the structural engineer.
We expect this will only require us to replace a few sheets in the plans and move forward
quickly. We would like to receive an extension while we add the final touches to the
plans, then we can all check this off our list and move forward building this project with
the help of the City of La Quinta.
Our structural engineer has been on Vacation for the past week, but I would assume we
could have this all finished up in a week to 10 days if we could please be granted that
time.
