Dear Mr. McKinney:
United Testing, LLC dba United-Heider Inspection Group (UHIG) is pleased to submit our response to the City of La
Quinta Request for Proposal to provide On-Call Materials Testing Services. UHIG is responding to the Geotechnical
Engineering and Inspection Services categories.
Over the past 35 years, UHIG has set itself apart as a leader in the Materials Testing and Special Inspection industry.
We pride ourselves on providing our clients with the highest level of service and professionalism. In doing this,
we have established long-term relationships with many other clients including; City of Moreno Valley, City of Santa
Monica, City of Santa Ana, City of Fontana and California Department of General Services (DGS). The following are
some highlights to our approach that will benefit the City of La Quinta:
1. Commitment: UHIG is once prepared to work together again with the City of La Quinta’s project team to provide a
seamless execution of specific project goals with the primary focus on: (a) providing unsurpassed quality testing and
inspection services to ensure the products used in construction comply with the approved project documents; and
(b) integrating with the project management team to meet overall project schedules.
2. Cost Savings Approach: Our professional staff will work closely with your project team to find the best approach
when planning and scheduling the required testing and inspection services. We will provide qualified personnel that
are certified in multiple disciplines to help with on-site efficiencies.
3. Experience: Through our Project Management Team, field staff and support team UHIG has consistently provided
high-quality professional services on various projects to the City of La Quinta.
Kevin Ramirez will serve as our Project Manager and the primary point of contact during the evaluation process.

of 7:00 am to 6:00 pm, Monday through Friday. However,

asked to remove any one of our team from a project
and as a company, we have never been terminated or
removed from a project, even those project sites that are
occupied and/or in use. In the past five years, United is
proud to say that we have not had any:
1. Litigation filed by clients against our firm.
2. Litigation filed by our firm against clients.
3. No pending claims.
UHIG also certifies that we have not been barred,
suspended or otherwise declared ineligible to contract
with any other federal, state or local public agency.
RELATED EXPERIENCE
UHIG has extensive experience working with cities,
counties, and public agencies throughout Southern
California. Over the past 35 years, UHIG has demonstrated
an ability to successfully participate in and complete
projects of similar size and scope with this type of
contract. This includes our ability to provide material
sampling/testing services for street, storm drain and
retained wall repairs or maintenance projects, assessing
sinkholes or depressions in public right of way, material

any other City projects during construction and the preparation of required final documents at the completion of

UHIG is currently providing services for numerous other public entities under this same type of Master Service

Our Team goal is simple: to provide the City of La Quinta
with highest-level of professional expertise that utilizes
current technology to provide an economically successful
projects in a timely manner. We believe our proposed

UHIG’s team is comprised of Engineers, Technicians,
and Inspectors that are experienced in all aspects
of Construction Materials Engineering, Testing and
Inspection Services. Our team members operate within

PROJECT ENGINEERING & MANAGEMENT TEAM
We propose Martin B. Lowenthal, Kevin Ramirez,
Param Piratheepan, G.E., Dennis W. Heider, R.C.E., Luis
Mondragon, E.I.T., Jeremy Winscher, and Steven Lindquist
as our core project engineering and management team.
All seven (7) members will bring decades of experience to
the City of La Quinta.

Project Engineers
Mr. Piratheepan and Mr. Heider will be responsible for
the supervision and management of all materials testing
services as required. It will be their responsibility to
ensure compliance with project documents governing
standard codes, as well as all applicable regulations.
They will maintain a productive working relationship
with the City of La Quinta’s engineering and construction
team and review all laboratory testing procedures, test
results, and daily reports prior to distribution. They
will provide engineering consultation, conduct studies
and evaluations as necessary in relation to the services
provided by UHIG. Both Mr. Piratheepan and Mr. Heider
have extensive experience with in providing on-call
services throughout Southern California on projects
ranging from transportation to parks and recreation.
Luis Mondragon, E.I.T. | Project Staff Engineer
Mr. Luis Mondragon will be responsible for reviewing,
and maintaining field soils reports and other submittals
for conformance with project requirements. He will also
assist Mr. Heider, and Mr. Piratheepan with the project
closeout and can produce certification reports, such as
Final Grading Reports.
Martin B. Lowenthal | Project Executive
Mr. Lowenthal, Project Executive, will be responsible for
the overall performance to ensure that the City of La
Quinta receives the level of service and commitment by
our contract management, engineering, and field support
teams. He will periodically meet with representatives
to ensure that all services are delivered in a timely and
cost effective manner. Mr. Lowenthal will direct project
management responsibilities for carrying out the daily
direct support on this project. He will be reviewing all
project specific budgets on a monthly basis and monitor
them on a regular basis to identify cost impact issues
as they arise. Over the years, Martin has overseen the
performance of on-call services to agencies such as; City
of Anaheim, City of San Marcos, City of Brea, and City of
La Quinta to name a few.
Kevin Ramirez| Project Manager
Mr. Ramirez will be assisting Mr. Martin B. Lowenthal with
project management and to ensure that the City of La
Quinta and UHIG reach an agreement that is acceptable
to both parties. He will communicate directly with the City
should any situations beyond our control cause additional
impact on the cost structure of a particular assignment.
He will be the main contact during this proposal process
and will periodically meet with City representatives to
ensure that all services are delivered in a timely and cost
effective manner.
Jeremy Winscher | Field Services Manager
Mr. Jeremy Winscher will work functionally and
operationally to City of La Quinta Representatives. He will
maintain a productive working relationship with the City of
La Quinta’s project team. In addition to the management
duties and responsibilities, Jeremy will coordinate the
scheduling of personnel with City Representatives. He

will communicate directly with the City should situations

Steve Lindquist | Laboratory Services Manager
We have assigned Mr. Steve Lindquist as the Laboratory
Services Manager. He will be responsible for all aspects of
materials testing and inspection services to the City of La
Quinta, in the field and in the laboratory. His duties and
responsibilities include, but are not necessarily limited
to:
• Logging in the samples that are delivered to our
laboratory.
• Assure that all the laboratory tests of soils,
aggregate, concrete, and asphalt concrete samples
are conducted in compliance with the ASTM Test
Methods as recommended in the project plans and
specifications, and as required by the City of La
Quinta.
• Reporting test results in a timely manner.
SPECIAL INSPECTORS/FIELD TECHNICIANS
Work performed by UHIG’s cross trained and multi-
certified Special Inspectors/Field Technicians on this
contract will be in accordance with the requirements of
the specific project plans and specifications and accepted
standard test methods and procedures.
Duties and responsibilities include, but are not limited to:
• Checking engineering plans and specifications and
providing observation pertaining to the materials
testing aspects for City of La Quinta projects
designed by others.
• Testing designated construction materials for
compliance with project plans and specifications.
Reporting all work observed on a daily basis and
reporting materials tests results immediately upon
completion of testing.
• Promptly preparing and submitting written reports
documenting all materials testing and inspection
services performed.
We maintain a full time driver for the delivery of field
testing equipment (such as Nuclear Gauges) to our job
sites and for the collection of samples and/or materials
to be transported to our laboratory for analysis.

PROJECT UNDERSTANDING
We have read and reviewed all the source documents
that are included within the RFQ package. We understand
the geotechnical, testing and inspection services will be
as necessitated by project documents and regulated by
DSA, UBC/IBC/CBC, ICC and Title 24 of the California
Code of Regulations.
We understand all the requirements that are outlined in
the RFQ Requested Services. UHIG has the knowledge,
experience and is capable to perform the expected tasks
and deliverables to the City of La Quinta. UHIG’s services
may vary from nuclear density testing, testing of concrete
cylinders to providing asphalt inspection services during
the construction of various projects. Overall, we are
capable to meet all requirements and utilize our state-of-
the-art laboratory located in Moreno Valley, California to
provide the City of La Quinta with instant communication,
last minute requests, and value engineering.
United-Heider Inspection Group has a proven method
of providing services from the scheduling phone call
through the invoicing of our services.
• Our first step, administratively, is to establish
lines of communication with The City of La Quinta
personnel, including field scheduling requirements,
report distribution, and invoicing.
• Daily field inspection reports will be provided to the
project site team immediately after completion of
the assigned inspection. Copies of these reports are
distributed to the Project Architect, the Structural
Engineer, and City of La Quinta Representative on a
monthly basis. Further, all inspectors’ daily reports
and lab data are available online through our secure
website, with individual client log-in. Laboratory
test data is provided to the engineer as they are
completed for review. Typed copies are then
reviewed by our engineers, stamped and distributed.
Any non-compliance will be reported immediately
to the project team and the United-Heider Project
Team will reach out to the City of La Quinta directly.
Non-Compliances will be reported within 24 hours.
• As needed, our project management team can
attend site meetings and are available for consulting.
• Our approach to meet each project schedule
includes timely reporting, accurate, detailed, and
legible daily reports, and regular communication
between our Project Manager, Field Team, Dispatch,
Lab, and Accounting personnel.
• All field and laboratory testing are performed under
the oversight of a professional engineer experienced
in geotechnical engineering, construction inspection,
and materials testing.
Throughout numerous previous successful projects, we
have the found the most important factor to completing
a project on time and within budget is communication.
Our staff believes in open communication, not only
with our clients, but also with our office. This constant
communication allows us to immediately address any
problems in the field, give immediate access to our
engineers, field technicians and special inspectors for
advice and clarification of field conditions, and improved
response times. This all comes into play in our approach
to completing the tasks assigned and not “wasting time”
In closing, you have our commitment to provide excellent
services with extensively trained field staff for each
project for the City of La Quinta.
Quality Assurance / Quality Control (QA/QC)
To assure the RCCD is getting quality projects with a
quality product, UHIG QA/QC is maintained by essentially
having all field testing and inspection services along
with all laboratory testing performed under the direct
supervision of our Principal Engineer, experienced
in materials testing and inspection. The registered
engineer has full responsibility for making sure that the
requirements of the project plans and specification as
well as all agency requirements are fulfilled with respect
to all inspection and testing requirements. Our team of
Engineers and supervisors will review daily reports as
well as make periodic site visits during the construction
in order to verify the contents of the daily report(s). Our
engineer(s) have direct oversight of our laboratory staff

and facility. Our Laboratory Services Manager works
daily with our team of engineers to make sure all aspects
of testing are being performed in compliance with the
required methods and standards. A hard copy of our QA/
QC manual procedures is available upon request.
TEAM EXPERIENCE, TECHNOLOGIES,
INNOVATIONS AND PROCESSES OF THE FIRM
We have the professional and technical staff to provide
coverage as needed on all planned City projects typical
of this type of contract. In an effort to assist with cost
savings to the City, we try to maximize the use of time on-
site by staffing projects with personnel that are certified
in multiple disciplines. UHIG anticipates providing 100%
of the services under this contract with our own staff that
are capable to perform all services in accordance with
City requirements.
In addition to the familiarity that exists between UHIG
and public entities, we feel we have several additional
strengths that our firm will bring to the City’s proposed
projects. Our team will be led by an experienced
management and supervisory team who have worked
together on similar projects and collectively understand
how contracts/project(s) of this nature need to be
organized and managed. UHIG has a proven track record
of meeting all of the quality assurance requirements
while continually searching for ways to minimize the
owner’s cost and adding value to the project.
Our project management team has been identified in our
Proposed Staffing and Project Organization section. We
have also included full resumes identifying our team’s
qualifications and experience.

materials testing and special inspection services to many
different projects throughout Southern California since
1984. UHIG has continually demonstrated an ability to
successfully participate in and complete construction
projects of all sizes and types.

design and construction team to develop a cost effective
testing and inspection program that will help facilitate

a project in accordance with the contract documents and

construction materials. Our tests are performed to strict
standards that are established by ASTM and AASHTO.
Our reports and results are issued immediately with all
parties notified of results in real time.

Our inspectors are equipped with tablet computers as a
means of reviewing drawings, RFI’s, and interacting with
the design and construction team.

Features of our system are discussed briefly in the
following:
Reporting Method: Our reporting method consists of
iPad-generated field reports left on site daily, so that you
have document of the testing and inspections performed,
or more importantly that you are aware of any non-
compliances or problems day of inspection. Under
normal circumstances, a written report, which has been
reviewed by one of our engineers, is available within one
week following the week of inspection. Laboratory test
reports are available within three days of completion of
the test and can be either e-mailed and/or mailed.
Electronic Reporting: UHIG is implementing a state-of-
the-art electronic reporting system. Our system is not
just an emailed report but a complete library of all the
reports on the project. Therefore, at any given time,
all laboratory reports, on-site reports, off-site reports,
and submittals can be reviewed by anyone who is given
password-protected access to the system. A newly added
feature is the ability to download any and all reports to a
separate file. The system was originally designed in 2004
and was redesigned in 2012. It has had four generations
of improvements installed over the past five years and
UHIG continues to be the leader in electronic reporting.
Budget Accountability: A new feature that has been
added is a client home screen. From this screen, City of
La Quinta representatives will have the ability to view all
project reports including project financials at any instant.
This includes the testing and inspection budget, billed-to-
date, percent, and outstanding invoices with the invoice
number. UHIG will utilize an electronic reporting system
that notifies the entire project team when a new report
is generated. This report is cataloged in our e-Reporting
system and all reports are kept in a library for use by the
design and construction team. UHIG Inspectors are also
utilizing iPads to capture imagery and video on the job
site when relevant to their position or inspection.
Notification of Non-Compliances: Electronic reporting,
however, is not the means and methods utilized to
notify personnel of non-compliance issues. On-site non-
compliance issues will be immediately brought to the
attention of the project team so that those issues can

Escalation in Prevailing Wage hourly rates will be calculated using the percentage of increase issued by the State of California Director of Industrial Relations.
Personnel are billed in four (4) hour and eight (8) hour blocks. Work over eight (8) hours per day, the first twelve (12) hours of work on Saturday, and work over forty (40) hours per
week is billed at 1.5 times the above hourly rates. Any work over twelve (12) hours on Saturdays and work performed on Sundays or holidays is billed at 2.0 times the above hourly
rates. Requested services which are required to be subcontracted by United-Heider Inspection Group will be billed at cost plus fifteen percent (15%). A two (2) hour minimum is
billed for cancellation of inspection personnel less than four (4) hours in advance of the scheduled inspection time. Steel fabrication shop inspection that is required at a distance
greater than fifty (50) miles from our Monterey Park, California office will have billable travel costs including, but not limited to, commercial transportation at cost plus fifteen
percent (15%), vehicle mileage at the above rate, travel time at the inspector's hourly rate, and per diem expenses not to exceed $120.00 per day. Approximately one (1) hour of
engineering (registered geotechnical engineer for soils inspections and certified engineer for all other types of inspections) is billed for every forty (40) hours of field inspection time.
Delinquent invoices may be subject to interest, service charges, and collection expenses, including attorney's fees, at our discretion. Interest is assessed at a rate of 0.0329% per day
(12% annually) for each day the invoice is past due. The date an invoice is due is determined to be the invoice date plus thirty (30) days.

RELEVANT PROJECT EXPERIENCE
Steve Lindquist has over 20 years of experience as Laboratory
Services Manager and Senior Field Technician. He has overseen
the laboratory materials testing for all projects on which our
firm has provided services. He is an experienced Field Techni-
cian, familiar with code requirements as they pertain to testing
and inspection in the laboratory and in the field. He is trained
and certified by Fyfe in the inspection of Fiber Reinforced Poly-
mer (Fiber Wrap) placement. He is also capable of performing
asphalt and concrete batch plant inspection and is an experi-
enced Caltrans Materials Tester.
His laboratory responsibilities include logging of samples, soil
testing including sieve and proctor testing, soil consolidation
testing, field density testing, concrete cylinder fabrication and
testing, asphalt concrete testing. He is responsible for labora-
tory certifications such as OSHPD, AASHTO, City of Los Angeles,
AMRL, City of San Diego and the California Division of the State
Architect (DSA). He is also capable of performing required test-
ing of typical construction materials such as concrete, mason-
ry, structural steel, reinforcing steel, and fireproofing.
