Our experienced reviewers understand the need to provide thorough reviews as well as efficient turn-around time. 2 CITY OF LA QUINTA ON-CALL CONSTRUCTION SURVEYING SERVICES ON-CALL SURVEYING SERVICES INLAND EMPIRE UTILITY AGENCY (IEUA), CHINO, CA (2016-PRESENT) Having been awarded a sole source three-year on-call survey contract for IEUA startfng in January of 2013, two one year extensions, renewed in 2018 and in our seventh year with IEUA. CASC has provided survey services on various water and wastewater projects throughout the Chino and Ontario areas. The majority of the on call surveying requires the use of GPS technology, filling in with conventfonal total statfons and digital levels as needed. All requested design topographic and utflity locatfon surveys are performed, compiled and delivered to the Agency in Autocad Civil 3D format. IEUA has numerous facilitfes throughout the Inland Empire and we can be requested to perform topographic and or constructfon surveying at all of them to facilitate their on going facilitfes improvements. We have provided constructfon survey services for a 5 million gallon recycled water reservoir as well as a 5 mile long water line to supply the reservoir in Chino, CA. Other survey projects include: ON-CALL CONTRACT SURVEY SERVICES - SAN BERNARDINO CITY UNIFIED SCHOOL DISTRICT SAN BERNARDINO, CA (2016-PRESENT) Under a 3-year contract, CASC has provided On-Call Contract Survey Services for SBCUSD schools. CASC surveyors have performed extensive design surveys for the athletfcs fields and paths of travel renovatfon and upgrades for Arroyo Valley High School, Cajon High School, Pacific High School and San Bernardino High School. In additfon we performed major design improvement surveys for 8 elementary schools, Barton ES, Cole ES, Del Rosa ES, Hunt ES, Bonnie Oehl ES, Warm Springs ES, Rio Vista ES and Community Day School. The surveys were performed using a combinatfon of GPS and conventfonal total statfon survey methods. All of the surveys were performed while school was in session. The survey data was compiled into a base map and delivered to the district in AutoCAD format. A minimum of 5 bluff cross sectfons at 25’ intervals is also required. To produce the requested survey we utflized the Orange County Survey Control Network to establish horizontal and vertfcal control in the form of NAD 83 NAVD 88 datum at the project site. Aerial targets were set and located with RTK/GPS methods per our aerial sub consultant flight and aerial target layout. During this tfme we set ground control points for the cross sectfon portfon of the work including survey of any obscured areas not visible from the sky, performed using robotfc total statfon methods. Upon completfon of the field work aerial target survey data was sent to our aerial sub consultant for their use to compile an aerial topographic map with 1 foot contours. With the aerial topographic contour map received from our sub consultant we checked the contours against the ground topo check elevatfons to ensure the topographic map met Natfonal Map Accuracy Standards. ON-CALL SURVEYING - COLTON JOINT UNIFIED SCHOOL DISTRICT COLTON, CA (2009-2012) As on-call surveyor for the District, CASC surveyors performed a variety of services for the school district in support of their Bond improvements: • Alice Birney Elementary School - Topographic and Construction Surveying. CASC surveyors performed conventfonal topographic mapping and constructfon surveying in support of design and constructfon of the new re-configured parking lot that allows for safer loading and unloading of the children on school buses. • Colton High School - Topographic Surveying. CASC surveyors performed a conventfonal topographic site survey bounded by Valley Boulevard, 2nd Street and 3rd Street and the bus garage to the north. Under a 3 year on-call survey contract and renewed for another 3 years in March 2012, CASC surveyors performed constructfon surveying for constructfon of parking lot and ADA pedestrian access walks through out the entfre 10 campus parking lots. CASC surveyors performed constructfon surveying for the constructfon of the two Science Portable buildings along with related grading, storm drain, water and sewer improvements. SURVEYING SERVICES - ENVIRONMENTAL SYSTEMS RESEARCH INSTITUTE REDLANDS, CA (2008-PRESENT) Redland Campus Since 2008 CASC has been providing planning, engineering and surveying GIS pioneer, ESRI (Environmental Systems Research Instftute), at their main campus in Redlands. CASC is providing surveying services for all site improvements as well as for the building constructfon which has required stringent horizontal control for each floor of the four story structure. The project has necessitated extremely tfght survey tolerances for the installatfon of the unique architectural design. Our services are provided in support of ESRI’s unprecedented growth of their Redlands Campus which spans 16 buildings. Since 2006, CASC has reviewed over 200 improvement plans and technical reports for the City. The firm’s services are flexible, with adjustments made periodically to meet the development demand in the City. During the peak of development in the mid-2000s, CASC staff worked directly from City of Redlands offices. As development has slowed, CASC has met review schedules working from our office in Colton. CASC’s experience has shown that working with the Client early on to develop a work flow process results in the most efficient delivery of the project. The firm is skilled at implementing Client specific project approaches, experience gained by working for a wide array of public agency and business Clients. The major milestones that are critical to any project consist of Project Planning, Notice to Proceed, and Project Initiation. A brief description of these milestones are outlined below: PROJECT PLANNING Upon receiving a specific project request from the City, our Director of Survey and Mapping, Rick Furlong will provide project planning, which will include preliminary research and work scoping based upon the City ’s project request. If the City’s project request includes a detailed scope of work and specific project limits, we will prepare a fee estimate and project schedule for submittal to the City. If the scope of work (or project limit) is vague and undefined, our project team will prepare the scope of work and fee schedule as needed for submittal to the City. NOTICE TO PROCEED Once the contract agreement is approved, Mr. Furlong will develop a (1) Survey Management Plan, and (2) Surveying Procedures Plan. The Survey Management Plan will dictate how the project will be managed while the Surveying Procedures Plan will describe how the work will be performed. The project kick-off meeting will be arranged with City staff to formally introduce the project team members to the City, review and discuss data exchange, project work plan and schedule, project issues and opportunities, project communication, Quality Control procedures, and other issues relevant to the project. PROJECT INITIATION Our Project Team will research City and public survey records for survey information such as the closest available control datum as well as record maps, record utilities for the areas to be surveyed or as designated by the City. At this stage, the exact limits of the project will be verified with the City. If needed, our Project Team will prepare a preliminary base map based upon the results of our research of available record data. Construction surveying tasks will include a thorough review of the improvement plans for consistency and constructability. Preparation of field crew staking/survey packages will begin. ADAPTING TO YOUR NEEDS The CASC team provides Clients with ready access to experts in the field of surveying and mapping on an as- needed basis. We provide scalable services that can grow and contract to match the rate at which work is needed. Our survey services team members recognize the importance of maintaining schedules and developing sound and cost-effective solutions for our Clients. CITY OF LA QUINTA ON-CALL CONSTRUCTION SURVEYING SERVICES ASSUMPTIONS/CONSTRAINTS The specific nature of projects are not known at the time of RFP. When determined, the assumptions or constraints will be identified and discussed at the time of project kick-off. The specific limitations and/or assumptions will be tied to a particular project. WORK PLAN AND SCHEDULE THE KEY TO SUCCESS The following is a summary of techniques that CASC uses internally to successfully complete projects in a timely manner, pursuant to our Clients’ requested schedules and deadlines. PROJECT CONTROL AND PROJECT SCHEDULE Our Director of Survey and Mapping, Rick Furlong, will administer the project in a manner that will ensure its accuracy and timely completion. An integral part of Mr. Furlong’s Project Control is the establishment of a close line of communication with City staff and with each key member of the project team. This includes scheduled project review meetings on a regular basis to review daily reports, survey field notes, filing of the notes in field books, and safety procedures. He will track the project budget, cost, and schedule by using a proprietary management computer program called “Deltek” which will allow our staff to electronically input timesheets daily and will provide Mr. Furlong with “real time” cost data and budget information. Use of this important Project Control tool will provide Mr. Furlong with the ability to keep the project on track. QUALITY CONTROL & QUALITY ASSURANCE PLAN Quality control inherently starts with the qualifications and experience of the key personnel. The quality control plan is initiated at the staff level and progresses up. Each team member is required to check their work for completeness and accuracy. Mr. Rick Furlong, our Project Manager, will oversee all survey related work, and using our in-house QC procedures and sound mapping and surveying judgment based upon experience, will continue to monitor and review that work. INNOVATIVE SOLUTIONS– PROJECT EXPERIENCE The Arid Region Non-Vegetated Erosion Control Project Study was initiated by Caltrans to evaluate non- vegetative alternatives for protecting slopes from erosion by wind and rain. In arid regions, vegetation is difficult to establish without the use of supplemental irrigation. As a result, Caltrans initiated this project, located in San Bernardino County near Barstow and Hinkley, to evaluate non-vegetative erosion control practices. In order to demonstrate our approach to the potential work that may be required, the following flowcharts represent typical workflows for requested survey projects. Permits, Applications, and Fees: Includes fees for Notices of Intent (NOI), Notices of Termination (NOT), application fees, submittal fees, permit fees, and other fees required as part of the project and not paid directly by Client. Reproduction Services: Includes blueprinting, copying, printing and plotting. In-house plots will be billed at $6.00 per sheet for each client set and for a final in-house review set. B&W / Color copies: $0.08 / $0.90 for 8.5 X 11 and $0.20 / $1.20 for 11 X 17. Rental Fees: Includes rental fees paid by the firm, including required vehicles, equipment, and tools required to complete the work. Commercial Delivery Services: Including Express Mail, Federal Express, UPS and independent courier services. In-House Pick-Up and Delivery Services: When provided by the firm, these services will be billed at $48.00 per hour plus $0.66 per mile round trip, with no additional markup. Travel Expenses: Includes travel expenses incidental to performance of the work, including airfare, parking, tolls, taxi, lodging, and etc. Vehicle mileage will be billed at a rate of $0.66 per mile with no additional markup. Travel time for professional and administrative staff will be billed per the hourly fee rate schedule with no additional markup and survey crews will be billed at $50 per hour, per 2-man crew. Per Diem: Per diem for meals for overnight stays will be billed at Caltrans current state rates. NOTES: Prevailing Wage: Projects and/or portions thereof designated by Client to be subject to Prevailing Wage shall be billed at the regular staff rate or the Prevailing Wage rate, whichever is higher plus a multiplier of approximately 2.3 to 2.8 for overhead and profit. The Prevailing Wage rate shall be based upon the Wage Rate Determination issued by California’s Director of Industrial Relations for the locality and employee classification at the time the work is performed. Waiver of Subrogation: When a Waiver of Subrogation for Workman’s Compensation Insurance is required by the Client, the Client will be required to pay the additional insurance premium. PRINCIPAL-IN-CHARGE Mr. Richard Sidor, one of CASC’s founding partners, brings over 30 years of civil and environmental engineering experience to the firm. His career in- cludes employment with the California Department of Transportation, large infrastructure contractors and private consulting firms. From his early career to now, he has managed large civil works projects and operations through- out the West. His early career at Caltrans included field and office engineering for projects in urban and rural settings, including the rehabilitation of Highway 91 in Riv- erside and the I-215 Cajalco Expressway interchange in Riverside County. After leaving Caltrans, he worked for a large Infrastructure contractor where he was project engineer on over 120 miles of lining repairs to the Central Arizona Project’s Granite Reef Aqueduct, and project manager for a $7M highway rehabilitation project in southern Idaho. His duties also included estimating and construction engineering at airports, and various military facilities including Kaneohe Marine Corps base in Oahu, Hi and the former George Air Force Base near Victorville. His mid-career experience includes over 15 years in civil design and manage- ment of over $500M in roads, golf courses, parks, storm drain facilities, po- table and reclaimed water systems, sewer systems, and grading on private and government civil works projects in Southern California. At CASC, he has managed the company’s largest contracts for clients includ- ing Boeing, Southern California Edison, and Caltrans, and is responsible for building the company’s Environmental/Water Quality division into a recog- nized leader that competes at the same level as the multi-national, publicly- held firms. He is an accomplished speaker having provided technical training in storm water quality to various agencies, contractors and consulting firms throughout the Southwest.
