License Law for the following reason (Sec. 7031.5, Business and Professions Code: Any
city or county that requires a permit to construct, alter, improve, demolish, or repair any
structure, prior to its issuance, also requires the applicant for the permit to file a signed
statement that he or she is licehsed pursuant to the provisions of the Contractor's State
License Law (Chapter 9 (commencing with Section 7000) of Division 3 of the Business
and Professions Code) or that he or she is exempt therefrom and the basis for the
alleged exemption. Any violation of Section 7031.5 by any applicant for a permit
subjects the applicant to a civil penalty of not more than five hundred dollars ($500).:

THIS CONTRACT ("Contract"), effective as of the date of the last party to sign below, is between

Proposal Limitations: Costs associated with any services or materials not included in this proposal
are NOT included.
Time: CBPP requires 48 hours' advance notice of schedule changes by the owner. If CDPP is delayed
In the progress of the work by owner change orders, fire, labor disputes, acts of God or other causes
beyond CBPP'S control, the work completion schedule or affected parts of the work shall be extended
by the same amount of the time caused by the delay.
Completion and Billing: Final payment is due upon substantial completion of project. Substantial
completion is defined as the point at which your final equipment has been installed. Payments for
work performed may not be withheld under any circumstances. Any disputes will be settled
independently In good faith between the parties,
Changes in the Proposal: Even though we plan each job as carefully and comprehensively as
possible, changes do occur. CBPP shall provide the owner with Change Orders identifying the amount
of additional costs or cost reductions resulting from changes required due to construction
circumstances or ordered by the owner. Change Orders shall be paid in full upon acceptance of
change and shall not alter the proposal payment schedule.
Additional Services: CBPP will perform additional services, maintenance, reprogramming,
customization and other services not covered in the proposal or under the terms of the warranty, per
written Change Order or proposal.
The undersigned customer hereby authorizes CBPP to perform the job outlined in the Scope of Work,
and agrees to the conditions on this Acceptance page.
CONTRACT PRICE: Owner agrees to pay Contractor the sum of $5,147.50 for performing the services
set forth in the attached Proposal.
