9) Fire sprinkler system plans for the tenant improvement area must be submitted to the Fire Department for
review, along with a plan/inspection fee. A licensed C-1 6 contractor will have to submit plans for review
and approval and modify the fire sprinkler system in accordance with NFPA 13, 1999 Edition. A licensed
C- 16 contractor shall do all sprinkler work and certification. The approved plans, with Fire Department Job
card must be at the job site for all inspections.
10) Whenever sprinkler overhead mains of 4" size or larger are provided, the Riverside County Fire Department
requires documentation from a structural engineer that the roof structural members will be capable of
supporting the weight of the water filled mains and attached lines. Provide appropriate detailed
documentation, with a wet stamp and signature, by the project structural engineer.
11) Install an alarm monitoring system for fire sprinkler system(s) with 100 or more heads (20
or more in Group 1, Division 1. 1 and 1-2 occupancies). Valve monitoring, water -flow
alarm and trouble signals shall be automatically transmitted to an approved central
station, remote station or proprietary monitoring station in accordance with 2007 CBC.
An approved audible sprinkler flow alarm shall be provided on the exterior in an approved
location and also in the interior in a normally occupied location. A C-1 0 licensed
contractor must submit plans, designed in accordance with NFPA 72, 1999 Edition, to the
Fire Department for review and approval prior to installation.
12) Instal I a Manual and Automatic Fire Alarm per NFPA 72, which must include initiation by manual pull, any
automatic device and activation of fire sprinkler water flow. System shall include occupant notification by
audio/visual devices, and detectors in all unattended spaces. System plans must be submitted to the Fire
Department for review, along with a plan check/ inspection fee.
13) Install a system to provide interior notification within each unit upon activation of the fre sprinkler system.

purchased.
15) Operation of the Knox key switch shall simultaneously open and control the gates for both directions of
travel.
16) Gate entrances shall be at least 2' feet wider than the width of the traffic lane(s) serving that Pate. All gates
providing access from a road to a driveway shall be located at least 30' feet fi-om the roadway and shall open
to allow a vehicle to stop without obstructing traffic on the road.

control of the Pate in the event of loss of primary power.

21) Provide either a separate Knox Key Lock box, Model 4400, 3200 or 1300, mounted per recommended
standard of the Knox Company or provide key(s) for inclusion in the general building Knox Box. If the
building/facility is protected with a fire alarm system or burglar alarm system, the lock boxes will require
"tamper" monitoring. Special forms are available from this office for the ordering of the Knox Box. This
form must be authorized and signed by this office for the correctly coded system to be purchased.
22) Provide keys to the tenant space for inc ' iusion in the main building Knox Box. Key(s) shal I have durable and
legible tags affixed for identification orthe correlating tenant space.

be an illuminated diagrammatic representation of the actual layout that shows tile name of complex. all
streets, building designators, unit numbers and Fire hydrant locations A& ithin the complex. These directories
shall be a minimum of 4' x 4' in dimension. Addressing of buildings and units shall conform to the Riverside
County Addressing Policy. Additional information and details may be obtai.ned by contacting the Fire
Department Planniag staff.
26) Rolling door in area separation wall must be self-closing. (Fusible link type)
27) Install panic hardware and exit signs as per the 2001 CBC.
28) All egress doors shall comply with 2001 CBC for proper door hardware and as noted on Sht of the
building plans.
29) Prior to final inspection of any building, the applicant shall prepare and submit to the Fire Department for

31) Install portable fire extinguishers per Title 19, but not less than 2AlOBC in rating. Contact a certified

32) If the building is used for high piled/rack storage, the building construction and fire sprinkler system must
meet 1999 NFPA 13 and the 2001 CFC.
33) Shelving, counters, etc., must be in place, however, no merchandise may be placed in the building prior to
inspection..
34) Submit to the Fire Department for approval, a Hazardous Materials Management Plan and a Hazardous
Materials inventory statement as outlined in the 2001 CFC.
35) Comply with Title 19 of the California Administrative Code.
36) The Fire Alarm System plans submitted with this plan are not a part of this plan review.
37) All fire sprinkler systems, fixed fire suppression systems and alarm plans must be submitted separately for
approval prior to construction. Contractors should contact the Planning & Engineering office for submittal
requirements.
38) Fire doors shall be activated by the activation of the Fire Alarm system.
39) Submit to the Fire Department for approval an emergency evacuation plan. The plan shall be conspicuously
posted, and all employees shall be trained in such procedures.
40) Approved building address shall be placed in such a position as to be plainly visible and
legible from the street and rear access if applicable. Building address numbers shall be a
minimum of 12" for building(s) up to 25' in height, and 24" in height for building(s)
exceeding 25' in height. In multi -tenant buildings, businesses shall post the business
name and suite number on back doors as well as the front. Suite numbers or letters must
be a minimum of 6" in height. All addressing must be legible and of a contrasting color
with the background and adequately illuminated to be visible from the street at all hours.
41) Approved building address shall be placed in such a position as to be plainly visible and legible frorn the
street. Said numbers shall contrast with their background.

Fugitive Dust Control PM 10 Plan (< 1 acre per Lot or lnf ill Project)

This Fugitive Dust Control Plan shall take into account applicable SCAQMD Regulations Rule 403, 403.1,
401, 402, 201, 203 and Portable Equipment Registration Program (PERP).
WATERING: Continuous watering is required to prevent dust and must occur a minimum of four (4) times
daily. Water shall be applied from an adequate sized metered water source to dry soils to prevent:
• Visible dust emissions >20% opacity
• Visible dust emissions that travel > 100 feet from the source (within site)
• Visible dust emissions that cross any property line
4- EDUCATION: The responsible dust control Individual and key personnel shall attend SCAQMD PM10 class
and obtain PM10 certificate number prior to construction activity (training information may be obtained by
contacting SCAQMD at (909) 396-2183).
CONSTRUCTION & DEMOLITION ACTIVITIES: A copy of this approved Fugitive Dust Plan must
be kept on site. At least one short-term stabilization method must be used during off hours. At least one
long-term stabilization measure must be used within ten (10) days of ceasing activity. Control
4- WEATHER MONITORING: Wind predictions shall be monitored.
4- SIGNAGE: Approved signs shall be installed at a highly visible location. Please refer to the Coachella
Valley Fugitive Dust Control Plan Handbook for Construction Signage Guidelines.
TRAFFIC: Construction traffic shall not be allowed on the pad, unless absolutely necessary. If vehicles
operate on pad, pad shall be kept firm and moist through hose watering and sprinklers; and a maximum
traffic speed of 15mph will be enforced. Fugitive Dust shall be prevented by fencing off site to prevent
unauthorized traffic on pad.
-.*- PARKING: Parking is not allowed on pad. All vehicles must park on street at designated areas only.
TRACK OUT: Provide 24 hour street cleaning and track out system as approved by City Public Works
inspectors. No dirt on public or private roads. Track out shall be cleaned up within one (1) hour of incident
and at the conclusion of each workday.
DIRT PILES: Dirt piles shall be limited to 50 cubic yards and built per California Building Code grading
requirements. Piles shall be kept moist or covered with tarp material. Larger dirt piles will require stockpile
or grading permit.
FENCING: Provide PM 10 fencing at perimeter of public roads and where applicable. Wood slat fencing
can be installed at rear of property and return 20 feet on either side if HOA restrictions apply. Block walls
can replace PM10 fencing during the construction phase.
-.*- EQUIPMENT: Extra hoses and sprinklers shall be maintained on site.
4- EXCAVATION MEASURES: All areas to be excavated or graded shall be pre -watered. Water shall also
be applied during excavation or grading operations.

Dear Mr. Morris:
As requested this revised letter is being provided with regards to the over -excavation of the building pad
for the EMC Ambulatory Care Facility, in the City of La Quinta, California. Refer to the attached
drawing for approximate limits of over -excavation and fills to date.
This building area has been over -excavated to depths as recommended in the referenced report. The
bottom of this over -excavation was approximately Elevation 92.8. As of the date of this letter the entire
building pad of the pad has been brought to plan grade (Elevation 102.08). Please refer to the attached
Over -Excavation Map.
A final report of testing and observations will be submitted upon completion of grading for the site.
