(CASC) is pleased to submit our proposal to provide On-Call Map Checking Services for the City of La Quinta, including, review of subdivision tract and parcel maps, lot line adjustments, parcel mergers, among other things, as described in the RFP. With a staff consisting of residents across the Inland Empire, and our office located in Palm Desert, we will provide the City with efficient support services eliminating travel time and logistical challenges facing out-of-the-area firms. CASC will provide top-notch local labor whose families live, work, play, and shop locally, ensuring that the project funds are circulated and re-circulated in the area. Our team personnel are highly experienced in field surveying, survey mapping, and improvement plan reviews. CASC has provided review services for cities like yours continuously for almost 30 years. Furthermore, CASC has had a relationship with the City of La Quinta through our Stormwater Inspections of Commercial, Industrial, and Food Facilities in the City itself and throughout the greater Desert Region. Our Director of Survey and Mapping, Rick Furlong, P.L.S., has over 35 years of experience in surveying for capital improvements including pipelines, utilities, infrastructure, building construction, landscaping, and all other development components. He is supported by talented office staff and field crews who bring ample experience working with institutional clients, and their teams of engineers, architects, and contractors. QUALIFICATIONS AND RELATED EXPERIENCE COMPANY OVERVIEW CASC Engineering and Consulting, Inc (CASC), a California S-Corporation, was established in the Inland Empire in 1993 (approaching 30 years). We currently operate from five offices serving Southern California, in the citfes of Colton, Irvine, Glendora, Temecula and Palm Desert. Our standard office hours are Monday-Friday from 8:00am-5:00pm, and our employees have fluid and flexible working hours, making crews available for emergency hours if necessary. We employ over 55 professionals, providing services such as professional planning, engineering, constructfon management, surveying, and technical support to public agencies and private enterprises throughout the region and beyond. CASC is a mid-sized consultfng firm that combines the personal touch of a small firm, with the stability of the large publicly-traded companies. While the Great Recession resulted in painful impacts to many firms including ours, we have been able to sustain a strong base of work in the infrastructure, environmental and energy business sectors. SURVEY MAPPING PLAN CHECKING Jess Becerra, L.S.I.T. DIRECTOR OF SURVEY AND MAPPING Rick Furlong, P.L.S. Rick Furlong, P.L.S.—Mr. Furlong is a California Licensed Land Surveyor and the project manager/day-to-day contact for the City of La Quinta. He is the Director of Survey and Mapping for CASC, and an accomplished professional surveyor with over 30 years of surveying experience in Southern California. Please see Mr. Furlong’s resume in Appendix 1 for a more in-depth view of his experience and accomplishments. Jess Becerra, L.S.I.T.—Mr. Becerra is a Senior Survey Analyst, and will be your survey mapping plan checking contact. He has over 30 years of Survey and Mapping experience and is responsible for overseeing and coordinating all mapping projects, reviewing Title Reports and relevant documents, boundary analysis, writing legal descriptions, and Construction and Survey support. He has a strong understanding of writing a legal description in conjunction with land survey boundary control principals. OFFICE SUPPORT Mike Marangakis, CISEC Noe Millan 7 CITY OF LA QUINTA ON-CALL MAP CHECKING SERVICES 4. WORK PLAN AND TECHNICAL APPROACH A. METHODOLOGY AND OVERVIEW CASC’s experience with Map Checking has provided us with fundamental principles that guide our work in providing Map Checking services successfully for the City. Approach and Methodology: •Arrival: Map checks and documents are brought to the Office by the CASC staff. Upon arrival, packages for Map checking are reviewed to make sure all of the necessary documents are provided, logged in, and the necessary resources are available to return the plan check at the required date. •Departure: Upon the completion of the review, comments and back up information is copied for internal files if needed in the future. A formal transmittal is prepared to the City stating that review has been conducted and states that either there are comments to be addressed, including a summary of the comments, or states that the maps as submitted meet their intended purpose and no further reviews are requested. Upon final mylars, they will stamped and signed technically correct by our Licensed Land Surveyor, Rick Furlong. Mapping Approach: Subdivision Tract and Parcel Maps, Lot line adjustments, Lot mergers, etc., are reviewed by our California Licensed Land Surveyor, Rick Furlong for the procedure of Survey, mathematical closure, and compliance with the Subdivision Map Act, the Land Surveyor’s Act, the approved Tentative Map, approved Conditions of Approval, Riverside County Map Check Standards, current title report and City of La Quinta preferences. Reviews typically include survey documentation, lot and boundary closure calculations, lot line adjustments, dedications and easement provisions, legal descriptions and completeness and accuracy of data notation. The map checking process will be incorporated with Bluebeam Revu software. Efficiency: CASC follows these five basic steps for successful and efficient Map and improvement plan checking: •Be Direct: Comments are clear and concise. •Be Objective: Plan checks are reviews, not critiques. •Be Thorough: Review everything and cross check drawings. •Be Consistent: Use the same staff throughout the project life when feasible. •Be Neat: Handwrite or type comments legibly. B. DETAILED PROPOSED SERVICES PRIOR TO REVIEW •City receives plan review packages from the developer/applicant and fees are paid. •Plan review packages are date stamped and logged into the City maintained database. •City of La Quinta notifies CASC that a submittal is ready for pick up. •CASC arranges for a pickup from City Staff. •All packages are date stamped and logged into a database maintained by CASC. 8 CITY OF LA QUINTA ON-CALL MAP CHECKING SERVICES PROJECT REVIEW STATUS SPREADSHEET •A Project Review status spreadsheet is delivered via email to designated City staff on a weekly basis. These weekly updates provide an overview of all projects currently under review or awaiting review, and their associated turn-around dates. •The commitments needed to meet turn-around timelines are reassessed weekly to ensure that necessary resources are available. MAP CHECKING •CASC reviews submittals for completeness utilizing the City of La Quinta checklist and an internal CASC checklist which verifies that all necessary documents have been provided. (Title report, grant deed, closures, record maps, easement deeds, conditions of approval, tentative maps, etc.). •If a submittal is determined to be incomplete, CASC will notify City staff via email or telephone, and will provide a list of documents to be submitted. If directed by City staff, CASC will notify the Engineer or Surveyor of all missing documents. Upon receipt of all required documentation by the Engineer and/or Surveyor, the submittal package is determined to be complete and re-stamped to indicate the new submittal date. MAP REVIEW PROCEDURES •As stated previously, all map checks (tract maps, parcel maps, lot line adjustments, lot mergers, right of way dedications and easements) are reviewed for the following: compliance with the subdivision Map Act, the Land Surveyor’s Act, the approved Tentative Map, approved Conditions of Approval, mathematical accuracy, City and County map check standards and City preferences. Map reviews typically include: all survey documentation, lot and boundary closure calculations, lot line adjustments, dedications and easement provisions, legal descriptions and completeness and accuracy of data notation. •All corrections and comments are noted on the actual documents being reviewed. A copy of all corrections and comments are maintained with CASC for record keeping. All comments are notated in a transmittal and returned with the submittal package to the City for further processing. ENGINEERING REVIEW PROCEDURES •Engineering plans are reviewed for accuracy and completeness and the Geotechnical Report is reviewed to see that the project is in general conformance with the project description and recommendations. •For all 1st plan checks, field site reviews will take place, as necessary, to provide comprehensive plan reviews. •All corrections and comments are noted directly on the engineering plans and reports. Transmittals with items of concerns listed, are provided to the City when plan set comments are returned to City staff for distribution. MAP AND ENGINEERING REVIEW RECHECK PROCEDURES •City notifies CASC that a submittal is ready for pick up. •CASC arranges for a pickup from City staff. •All packages are date stamped and logged into a database maintained by CASC. •An efficient plan check requires that the engineer or surveyor reply to all comments from previous review submittals. Resubmitted plans are reviewed by confirming the compliance of comments from the previous plan check and noting any changes to the design. Comments are redlined on the plans. •Comments counter-reviewed on the previous plan package are marked with a highlighter pen. •All comments are notated in a transmittal and returned with the submittal package to the City for further processing. •If there are no further comments, the City will is notified and the applicant /owner ’s engineer is informed to submit signed and stamped mylars for further processing. CASC’s Licensed Surveyor will provide stamp and signature stating the map is technically correct on final approved map projects. For interim improvement plans, CASC’s Professional Engineer will provide a stamped and signed letter stating the plan is suitable for acceptance by the City. 9 CITY OF LA QUINTA ON-CALL MAP CHECKING SERVICES ADDITIONAL OPTIONAL SERVICES PROVIDED BY CASC FIELD SURVEY SERVICES FOR CAPITAL IMPROVEMENT PROJECTS: CASC’s approach to providing Surveying services on this project will satisfy the needs and requirements of the City. Project Planning: Upon receiving a specific project request from the City, our Survey Manager will provide project planning, which will include preliminary research and work scoping based upon the City’s project request. Project Initiation: Upon notice to proceed our Project Team will research City and County records for survey information such as the closest available control datum as well as record maps for the areas to be surveyed, etc. At this stage, the exact limits of the project will be verified with the City. If needed, our Project Team will prepare a preliminary base map based upon the results of our research of available record data. We will then have our survey crews establish horizontal and vertical survey control throughout the project. The topographic survey will be performed in accordance with the City ’s project scope. The completed field survey can now be processed and compiled into an AutoCAD file and combined with the record basemap. The topographic survey map will then be reviewed for accuracy and compliance with the project scope and incorporated into Bluebeam Revu software. Field Topographic Survey Work: We plan to utilize a combination of aerial, GPS and conventional topographic mapping methods as needed depending on the City’s project requirements. The survey will include street cross sections as needed and include the location of all other elements usual and customary for a topographic survey including, but not limited to, monumentation and property lines, trees, vegetation, utilities, structures, water courses, fences, driveways, roadways, signs, sidewalks, lights, power poles, drainage structures, sewer manholes and all other visible site improvements as well as all accessible invert elevations. •Sufficient elevations to develop 1-foot contours extending to the centerlines of adjacent streets. •Provide a record data base map depicting boundary, right of way and centerlines of the project, verifying that they are consistent with the current assessor’s maps. •Data shall be delivered to the City in latest AutoCAD format edition. The Survey Team will implement the following methods: 1)Confirm limits of design survey: Project limits will be marked in the field at the beginning of survey. 2)Establish horizontal and vertical control based on City Benchmark systems and City/ County Control: Locate survey monuments and City Benchmarks and distribute survey control throughout the project. 3)Perform design survey (street cross sections) using a combination of GPS, total station and electronic digital level methods: Begin collecting survey shots for the project in a format that will assist in adding symbols and linework for the entire project. 4)Survey all existing features, curb, gutters, utilities, water valves, manholes, structures, inverts, etc.: During this time numerous redundant horizontal and vertical control checks are made during each instrument setup to insure accuracy. Also photos of the project will be taken and can be supplied upon request. 5)Download survey data, automated drafting of survey point data, compile into AutoCAD file for providing survey point, topographic and contour data and deliver to the design engineer: Survey data is downloaded, control points are compared with check shots to verify accuracy, linework is then generating from the point commands which significantly speeds up processing time and requiring only minor editing. Generate contours from the survey points, label and combine with a record base map and final review. In order to demonstrate our approach to the potential work that may be required, the following flowcharts represent typical workflows for requested survey projects. Since 2004, CASC has reviewed over 200 improvement plans and technical reports for the City. The firm’s services are flexible, with adjustments made periodically to meet the development demand in the City. During the peak of development in the mid-2000s, CASC staff worked directly from City of Redlands offices. As development has slowed, CASC has met review schedules working from our office in Colton. Our on-going contract with the City of Redlands demonstrates our intimate knowledge of City standards and our relationships with City staff. CASC also reviewed tract maps, parcel maps, lot line adjustments, and other mapping documents. CASC was responsible for verifying that the City’s requirements were met by applicants, tracking in-coming and out-going plan checks, interfacing with the developer’s engineers, and obtaining approval of the various plans with minimal oversight from the City. Understanding of Development Codes and City Standards along with good record keeping and coordination were key to meeting the City’s review goals. This project involved the preparation of preliminary and final engineering plans, preliminary and final entitlements of implementing tract maps, management and oversight of design guidelines, and mapping for a 1,000-acre golf course/commercial and residential development. The community is centered around the $10 million Yucaipa Valley Golf Club, which features a 6,803 yard 18-hole par 72 course designed by Rainville-Bye. The project also includes the 25-acre Yucaipa Valley Center which includes a variety of retail stores including a Vons supermarket, a Starbucks, McDonalds, and other retail and office space facilities. An open space network encompassing coastal sage scrub, coast life oak woodlands, native grasslands, and miles of multi-use trails was preserved and enhanced as a key community amenity. Arrow Boulevard, Merrill Avenue and Randall Avenue - Aerial Topographic Mapping and Conventional Topographic Surveying: Currently being on the City’s Consultant list, CASC surveyors performed aerial and conventional topographic design surveys to facilitate design of this major road widening project. SOUTH ORANGE COUNTY COMMUNITY COLLEGE DISTRICT SURVEY SERVICES - SADDLEBACK COLLEGE SITE IMPROVEMENT PROJECT CASC surveyors performed a Comprehensive Topographical Survey of the side of the bluff from the SOCCCD property line easterly to the bottom of the bluff at OC Parks trail for 25’ south of the eroded outfall pipe and 75’ north of the eroded outfall pipe. A minimum of 5 bluff cross sections at 25’ intervals is also required. To produce the requested survey we utilized the Orange County Survey Control Network to establish horizontal and vertical control in the form of NAD 83 NAVD 88 datum at the project site. Aerial targets were set and located with RTK/GPS methods per our aerial sub consultant flight and aerial target layout. During this time we set ground control points for the cross section portion of the work including survey of any obscured areas not visible from the sky, performed using robotic total station methods. Upon completion of the field work aerial target survey data was sent to our aerial sub consultant for their use to compile an aerial topographic map with 1 foot contours. With the aerial topographic contour map received from our sub consultant we checked the contours against the ground topo check elevations to ensure the topographic map met National Map Accuracy Standards. CITY OF TEMECULA ON CALL ENGINEERING, SURVEY AND ENVIRONMENTAL SERVICES CASC was recently awarded a 5-year on-call contract with the City of Temecula to provide engineering, survey, and environmental services. CASC’s survey department has provided project planning based on the City’s project request, and our Director of Survey and Mapping, Rick Furlong, PLS, has developed a Survey Management Plan, and a Surveying Procedures plan. The research consisted of public survey records for record maps, record utilities, City limits, and a preliminary base map based upon the survey data. CASC started where the CAP program left off, allowing the cities to maintain compliance with the Riverside County MS4 Permit for the Whitewater River Basin (MS4 Permit). CASC performs stormwater inspections at Permit designated facilities, which include hazardous material and food facility inspections, at frequencies established by the MS4 Permit. CASC’s responsibilities included reviewing the facility lists from the Riverside County Department of Health (DEH) and the City’s business license department, prioritizing facilities to be inspected, preparing inspection schedules, and providing progress reports to the City. CASC also developed an Inspector Procedures Manual for the City’s future use, developed coordination procedures with the City’s Code Enforcement Division, developed digital versions of the City’s inspection forms that can be provided electronically to facilities as inspections occur, and trained City staff on specific requirements of the MS4 Permit inspection program. Travel Expenses: Includes travel expenses incidental to performance of the work, including airfare, parking, tolls, taxi, lodging, etc. Vehicle mileage will be billed at a rate of current IRS mileage reimbursement rate per mile with no additional markup. Travel time for professional and administrative staff will be billed per the hourly fee rate schedule with no additional markup and survey crews will be billed at $60 per hour, per 2-man crew. Per Diem: Per diem for meals for overnight stays will be billed at Caltrans current state rates. NOTES: Prevailing Wage: Projects and/or portions thereof designated by Client to be subject to Prevailing Wage shall be billed at the regular staff rate or the Prevailing Wage rate, whichever is higher plus a multiplier of approximately 2.3 to 2.8 for overhead and profit. The Prevailing Wage rate shall be based upon the Wage Rate Determination issued by California’s Director of Industrial Relations for the locality and employee classification at the time the work is performed. Waiver of Subrogation: When a Waiver of Subrogation for Workman’s Compensation Insurance is required by the Client, the Client will be required to pay the additional insurance premium. The approximate amount for the waiver is $250 per year. Litigation Support and Expert Witness shall be at 2.0 times the above noted hourly rates. INDEMNIFICATION. 6.1 Indemnification. To the fullest extent permitted by law, Contracting Party shall indemnify, protect, defend (with counsel selected approved by City), and hold harmless City and any and all of its officers, employees, agents, and volunteers as set forth in “Exhibit F” (“Indemnification”) which is incorporated herein by this reference and expressly made a part hereof. When the law establishes a professional standard of care for Contracting Party’s Services, to the fullest extent permitted by law, Contracting Party shall indemni- fy, protect, defend (with counsel selected approved by City), and hold harmless City and any and all of its officials, employees, and agents (“Indemnified Parties”) from and against any and all claims, losses, liabilities of every kind, nature, and description, damages, injury (including, without limitation, injury to or death of an employee of Contracting Party or of any subcontractor), costs and expenses of any kind, whether actual, alleged or threatened, including, without limitation, incidental and consequential dam- ages, court costs, attorneys’ fees, litigation expenses, and fees of expert consultants or expert witness- es incurred in connection therewith and costs of investigation, to the extent same are caused in whole or in part by any willfully negligent or wrongful act, error or omission of Contracting Party, its officers, agents, employees or subcontractors (or any entity or individual that Contracting Party shall bear the legal liability thereof) in the performance of professional services under this agreement. With respect to the design of public improvements, the Contracting Party shall not be liable for any injuries or prop- erty damage resulting from the reuse of the design at a location other than that specified in Exhibit A without the written consent of the Contracting Party. b. Indemnification for Other Than Professional Liability. Other than in the performance of professional services and to the full extent permitted by law, Contracting Party shall indemnify, defend (with counsel selected ap- proved by City), and hold harmless the Indemnified Parties from and against any liability (including liability for claims, suits, actions, arbitration proceedings, administrative proceedings, regulatory proceedings, losses, expens- es or costs of any kind, whether actual, alleged or threatened, including, without limitation, incidental and conse- quential damages, court costs, attorneys’ fees, litigation expenses, and fees of expert consultants or expert wit- nesses) incurred in connection therewith and costs of investigation, where the same arise out of, are a conse- quence of, or are in any way attributable to, in whole or in part, the performance of this Agreement by Con- tracting Party or by any individual or entity for which Contracting Party is legally liable, including but not limited to officers, agents, employees, or subcontractors of Contracting Party. 27
