The City has received your request for duplicate plans for the Stable Building located at 55075
Monroe Street (below), dated November 14, 2019, along with the authorization from the
architect on file (below) and the notarized Request to Duplicate Official Plans (attached). You
may pick up a flash drive containing Plan Set No. 02684 at City Hall during our regular business
hours Monday – Thursday, from 7:30 am – 4:30 p.m., and Fridays from 8:00 a.m. to 4:30 p.m.,
let me know when you would like to stop by. Please note we will be closed November 28 & 29,
2019 in observance of Thanksgiving holiday.

Good Morning,
In order to release a copy of the plans, the Clerk's Office must receive the following:
1) Complete the attached affidavit entitled “Request to Duplicate Official Plans,” and have
it notarized
2) Obtain written consent from the Architect/Engineer on record, either via a letter or an
email, authorizing the City to release the plans, listing the property address and who is
authorized to obtain the copies.
The authorization from the Architect has been received so we will just need the Request to
Duplicate Official Plans (attached) completed notarized to process this request.

Is anything more need to send the plans digitally? I’m offsite today and don’t have the specific
plan numbers in this email, but the address is listed and it is for the garage and barn structures,
