Inspection Gerald 03/02/2020 0.25 A search in Trakit reveals an expired STVR permit. (06/30/2019) 1 then conducted a Duchene search in Munirevs and found the STVR to be active. I called Michelle who believed the permit to be expired however she would check with Missy to confirm. I later received a voice message from Missy Mendoza who stated the STVR @ 51715 Avenida Montezuma is expired as of 06-30-2019 and continues to report TOT for the months of Nov, Dec,2019 and Jan 2020, Inspection Gerald 03/11/2020 0.25 Made numerous attempts to reach the property owner. This property remains in Duchene pending status as of 03-02-2020 however, according to Missy the property has not submitted an application and is expired. I will make one final attempt to reach the propedy owner before issuing a citation. Inspection Gerald 03/13/2020 0.25 Conducted a search in Trakit and found no applications have been submitted. Duchene conducted searches in Trakit and in MuniRevs. A citation will be issued for operating an STVR without obtaining an permit. Inspection Gerald 04/22/2020 0.25 Upon arrival of 51715 Avenida MONTEZUMA, I observed no vehicles parked on the Duchene driveway. I conducted a search in Munirevs and found no active STVR permits. I sent an email to Michelle Lopez to confirm the info in Munirevs is accurate. This case will remain open until further notice. Email Gerald 04/23/2020 025 1 received an email from.. Michelle Lopez who stated permit remains on hold do to Duchene missing TOT forms for 2019. Lopez stated she has emailed the property owner multiple time. This case will remain open until further notice. see attachment Inspection Gerald 04/30/2020 0.25 1 contacted property owner Greg Dahlquist and inform him of the missing TOT Duchene payments for May, June, July, August, and Sept. He stated his wife was in each month to pay that I asked that he contact the HUB for clarification. This case will be rechecked for 14 days Inspection Gerald 05/26/2020 0.25 1 attempted to reach property owner Greg Dahlquist, however it went straight to voice Duchene mail. I did not leave a message at the time beucase the voice message states he will not be available until May 30th and to call back at that time. This case will be rechecked for then. I advised she would need to log in to Munirevs and activate her account. I explained she could submit documents through the portal and if she had any question to contact the HUB.
