WORK SCHEDULE
PROPOSED WEEKLY SCHEDULE OF WORK:
Horizon Lighting, Inc. will follow a Monday through Friday weekly schedule to perform lighting inspections and
repairs at the designated bi-weekly locations as per the contract. Please see the attached calendar for "schedule
sample." Lighting maintenance service will commence prior to sunrise each working day to allow our technician to repair
outages and adjust as needed. In addition, this will allow us to notate any large outages, major repairs, etc. to bring to
the assigned City representative's attention.
Our technician will touch base with the assigned City representative to discuss any areas of concern or any
reported areas that may require immediate attention. Our goal is to address any/all lighting issues in a timely fashion to
eliminate and safety/liabilities while still maintaining the ambiance of the City. All outages are deemed a high priority
however we will seek direction from the assigned City representative and move forward based on the City's needs first,
standard repairs second. While lighting maintenance service is performed at the location outlined and listed per the
contract, we will inspect for fixture outages and lighting adjustments needed. We will be attentive to large outages,
broken fixtures, exposed electrical components, etc. As repairs are completed, fixtures will be cleaned of debris and
tested to confirm proper operation prior to departure. In the event a fixture or lighting component is damaged beyond
repair or considered "aged," we will submit our recommendation for replacement. Fixture replacement
recommendations and cost-effective options will be discussed with the assigned City representative prior to
commencement of the initial lighting maintenance. This will allow for a clear understanding of the City's goals in relation
to energy efficiency, consistency and lighting upgrades/improvements.
While service is in progress, our technicians internally provide a detailed scope of the work performed, material
utilized and a marked map of the location in which the work was performed. We utilize custom internal maps to detail
the work performed and materials utilized. A marked map of all the locations in which service is performed is provided
with our electronic lighting maintenance invoice. Our technicians complete all their paperwork and updates
electronically via smart phone through our custom system tailored to our lighting maintenance services. In addition, this
allows them to have any special location notes and lighting control information needed readily available when
dispatched to provide exceptional service on each maintenance location every time.

In compliance with the Advertisement for Proposals, the undersigned Contractor:
Has examined the contract documents, site of work, and being familiar with the conditions to be
met, hereby submits the following Proposal for furnishing the material, equipment, labor, and
everything necessary for the completion of the work listed and agrees to execute the contract
documents and furnish the required bonds and certificates of insurance for the completion of
said work, at the locations and for the prices set forth on the inside pages of this form.
Understands that services of this project shall be in accordance with all applicable specifications
in this document.
Unit prices in each and every case represent the true unit price used in preparing the proposal
schedule totals (Proposal Form). Unit prices listed herein include material, installation and
appurtenant work as is necessary to have the item complete and in place meeting the full intent of
the plans and specifications. We acknowledge that unbalanced unit prices shall be sufficient
cause for the rejection of our proposal.
Proposal items are for labor, equipment, and incidental materials as referenced in Section 3000.

Work shall be completed within one (1) year (twelve [12] months), by June 30, 2025, beginning
on July 1, 2024, as specified in the Notice to Proceed.
The City may elect to exercise an extension to renew this contract for six (6) additional one-year
terms with a ninety (90) day notice, on a year-to-year basis, contingent upon satisfactory work of
the contractor.
