ADDENDUM NUMBER 1
TO: All Prospective Bidders and Plan Holders

SUBJECT: Project No. 2019-14, CALLE TAMPICO AT PARK AVENUE GUARD RAIL
IMPROVEMENTS
The following shall be considered as incorporated into the project plans and specifications
for the above referenced project. Portions of plans and specifications not specifically
mentioned in this Addendum remain in force.
- PROJECT BID DOCUMENTS
Section 4000
Sheet 4000-2, Section 5.0 "Construct Guard Rail", add the following
paragraph after paragraph 1:
The terminal system shall be the latest model terminal system available from an
approved Caltrans manufacturer. At the time of this addendum model MSKT-SP-
MGS8 is the latest model from Road Systems, Inc.
- PROJECT CLARIFICATIONS
Q. Section 1500 7.4 Utility relocation& and Rearrangement. Second paragraph.
Delete all but "Protect" Is this acceptable?
a. The full paragraph says the following: "The Contractor assumes
responsibility for the removal, relocation, or protection of
existing facilities wherein said facilities are identified by the
Plans, field located by a utility company, or as provided for in
the General Requirements. The Contractor shall coordinate
with the owner of utility facilities for the rearrangement of said
facilities. The Contractor assumes responsibility for the

utility company, or as provided for in the General
Requirements. The Contractor shall coordinate with the owner

The plans say to locate and protect in place a water line. The
language in the paragraph is general and points to the project
plans or other specification locations. The paragraph does not
hold the Contractor responsible for anything other than what is
specifically being asked for so it can remain as it is.
Q. Section 1500 7.4 Utility relocation& and Rearrangement. Second paragraph.

a. The full paragraph says the following: "Professional Liability or
Errors and Omissions Insurance as appropriate shall be written
on a policy form coverage specifically designed to protect
against acts, errors or omissions of the Contracting Party and
"Covered Professional Services" as designated in the policy
must specifically include work performed under this
agreement. The policy limit shall be no less than $1,000,000
per claim and in the aggregate. The policy must "pay on behalf
of" the insured and must include a provision establishing the
insurer's duty to defend. The policy retroactive date shall be
on or before the effective date of this agreement. Professional

Since the statement says as appropriate then the paragraph
does not need to be omitted.
Q. Section 3000 4.0 Work Details, 4.1 Traffic Control. Can you omit PE & TE
Signature required? 4.2 Dust Control, an acceptable dust control plan will be
submitted by Contractor. The designated -on site person to implement and
prepare this plan (PM-10) this plan is the designated Safety person also.
(The Contractor) Contractor requests that the "Certified by the SCAQMD" be
omitted?
a. These statements will remain as written.
The Contractor is hereby notified; Addendum No. 1 must be acknowledged as
stipulated under the instructions to Bidders and submitted as part of the Bid.
Failure to do so shall result in the City designating said bid as 'Won -Responsive.
