ADDENDUM NUMBER 1
The following shall be considered as incorporated into the Request for Proposal (RFP) for the above -
referenced services. Portions of RFP not specifically mentioned in this Addendum remain in force.
■ REQUEST FOR PROPOSAL
Question 1: Is there a pricing list that you would like us to use, as I don't see one within the RFP? If
not, are we supposed to provide hourly employee rates and equipment rates only?
Answer 1: Yes, please provide hourly employee rates and equipment rates.
Question 2: Can you please provide a current pricing sheet from your current vendor St. Francis
Electric?
Answer 2: Not available at this time.
Question 3: On attachment 2, 1 don't see a space for a signature. Is it ok to sign anywhere on this
attachment or will the City be providing a revised Attachment 2?
Answer 4: Please fill in the blank sections of Attachment 2 to the RFP.
Question 5: 1 understand that the City is seeking a separate Work Proposal and a separate Cost
Proposal for the hard copy, I would just like to confirm that the Work Proposal and Cost
Proposal is to be included together in the single pdf document? If so, would you like the
Cost Proposal to be after section 2 of the Work Proposal so that it is in chronological
order or at the end of the work proposal?
Answer 5: Please submit one work proposal pdf and one cost proposal pdf as well as one
hard copy of the work proposal and one hard copy of the cost proposal in a
separate envelope.
Question 6: On Page 5 of 12, Section 11. Proposal Format, it states to "Present the proposals in a
format and order that corresponds to the numbering and lettering contained herein..."
can the City please clarify/confirm that Number 3. "Complete Pricing List" and Number 4
"List of Complementary Services Offered by Proposal along with Corresponding Prices"

is not to be included in the Work Proposal and will be part of the Cost Proposal so then
the Work Proposal would be numbered as 1,2,5,7 and so on?
Please include the Complete Pricing List in the Cost proposal. However, the List
of Complementary Services can be included in the Work Proposal.
Question 7: On the previous RFP in 2019, the City provided a Cost Proposal Sheet. Will the City be
providing a cost proposal sheet for this RFP?
Answer 7: Consultant shall provide their own cost proposal sheet.
Question 8: Can the City please confirm that the section for "Firms, Background, Qualifications, and
Experience..." is to be included as part of the Number 1. "Cover letter"? Or should this
be a separate section? Also, letter (d) includes resumes for the project manager and key
personnel; however, two paragraphs above it states, "resumes and brochures may be
added, provided they are located in the appendix at the back of proposal." Can the City
please confirm if the resumes for the Project Manager and key personnel shall be
included in section (d) of "Firms Background, Qualifications, and Experience..." or in the
back with the appendix?
Answer 8: Firms, Background, Qualifications, and Experience is to be included at part of
Number 1. "Cover Letter". Resumes can be included with the appendix.
The Consultant is hereby notified, Addendum No. 1 must be acknowledged as required under
Section 6, Addenda and submitted as part of the RFP. Failure to acknowledge and incorporate
addenda will not relieve the proposer from the responsibility to meet all terms and conditions of
the RFP and any subsequent addenda.
