DECLARATION REGARDING CONSTRUCTION LENDING AGENCY (If
applicable)
A construction lender will be used for completion of the improvements
covered by this permit and the name and information of which is on file for
public inspection during the regular business hours of the public entity.
Failure of the applicant to indicate the construction lender on the
application does not relieve that person of the requirement to give the
construction lender preliminary notice from that duty.
WORKERS’ COMPENSATION DECLARATION (1, 2 or 3)
WARNING: FAILURE TO SECURE WORKERS’ COMPENSATION COVERAGE IS
UNLAWFUL, AND SHALL SUBJECT AN EMPLOYER TO CRIMINAL PENALTIES AND
CIVIL FINES UP TO ONE HUNDRED THOUSAND DOLLARS ($100,000), IN ADDITION
TO THE COST OF COMPENSATION, DAMAGES AS PROVIDED FOR IN SECTION
3706 OF THE LABOR CODE, INTEREST, AND ATTORNEY’S FEES.
1) Has and will maintain a certificate of consent to self-insure for workers’
compensation, issued by the Director of Industrial Relations as provided for
by Section 3700 of the Labor Code, for the performance of the work for
which this permit is issued.
2) Has and will maintain workers’ compensation insurance, as required by
Section 3700 of the Labor Code, for the performance of the work for which
this permit is issued.
3) Certifies that, in the performance of the work for which this permit is
issued, shall not employ any person in any manner so as to become subject
to the workers’ compensation laws of California, and agree that, if should
become subject to the workers’ compensation provisions of Section 3700 of
the Labor Code, shall forthwith comply with those provisions.
NOTICE TO APPLICANT (1, 2 and 3)
1) Each person upon whose behalf this application was made, each person
at whose request and for whose benefit work is being performed under or
pursuant to any permit issued as a result of this application, the owner, and
the applicant, each agrees to, and shall defend, indemnify and hold harmless
the City of La Quinta, its officers, agents, and employees for any act or
omission related to the work being performed under or following issuance of
this permit.
2) Any permit issued as a result of this application becomes null and void if
work is not commenced within 360 days from date of issuance of such
permit, or if cessation of work for 180 days will subject permit to
cancellation. On or after January 1, 2019, every permit issued shall become
invalid unless the work on the site authorized by such permit is commenced
within 12 months after its issuance, or if the work authorized on the site by
such permit is suspended or abandoned for a period of 12 months after the
time the work is commenced.
3) Certified that they read the application and state that the information
provided is correct. Agree to comply with all city and county ordinances and
state laws relating to building construction, and hereby authorize
representatives of this city to enter upon the above-mentioned property for
inspection purposes.
IMPORTANT: An Application for Permit was made to the Building Official,
subject to the conditions and restrictions set forth at the time of application,
which is now hereby an Issued Permit. If any part of the information being

BUILDING PERMIT
Construction or work for which a permit is required shall be subject to inspection by the building official and such construction or work
shall remain visible and able to be accessed for inspection purposes until approved. Approval as a result of an inspection shall not be
construed to be an approval of a violation of the provisions of the applicable code and other ordinances of the jurisdiction. Inspections
presuming to give authority to violate or cancel the provisions of the applicable code and other ordinances of the jurisdiction shall not be
valid. It shall be the duty of the owner or the owner's authorized agent to cause the work to remain visible and able to be accessed for
inspection purposes. Neither the building official nor the jurisdiction shall be liable for expense entailed in the removal or replacement of
any material required to allow inspection.
It shall be the duty of the holder of the building permit or their duly authorized agent to notify the building official when work is ready for
inspection. It shall be the duty of the permit holder to provide access to and means for inspections of such work that are required by this
code.
Inspections
A building or structure shall not be used or occupied in whole or in part, and a change of occupancy of a building or structure or portion
thereof shall not be made, until the building official has issued a certificate of occupancy therefor as provided herein. Issuance of a
certificate of occupancy shall not be construed as an approval of a violation of the provisions of the applicable code and other ordinances
of the jurisdiction. Certificates presuming to give authority to violate or cancel the provisions of this code or other ordinances of the
jurisdiction shall not be valid.
Certificate of Occupancy
A person shall not make connections from a utility, a source of energy, fuel, or power, or a water system or sewer system to any building
or system that is regulated by the applicable code and other ordinances of the jurisdiction for which a permit is required, until approved by
the building official.

EQUIPMENT NOTES/MANDATORY REQUIREMENTS
1. ANY POOL OR SPA HEATING SYSTEM OR EQUIPMENT MAY BE INSTALLED ONLY IF
THE MANUFACTURER HAS CERTIFIED THAT THE SYSTEM OR EQUIPMENT HAS ALL
OF THE FOLLOWING: EFFICIENCY - A THERMAL EFFICIENCY THAT COMPLIES WITH
THE APPLIANCE EFFICIENCY REGULATIONS; AND ON-OFF SWITCH - A READILY
ACCESSIBLE ON-OFF SWITCH, MOUNTED ON THE OUTSIDE OF THE HEATER THAT
ALLOWS SHUTTING OF THE HEATER WITHOUT ADJUSTING THE THERMOSTAT
SETTING; AND INSTRUCTIONS - A PERMANENT, EASILY READABLE , AND
WEATHERPROOF PLATE OR CARD THAT GIVES INSTRUCTION FOR THE ENERGY
EFFICIENT OPERATION OF THE POOL OR SPA HEATER AND FOR THE PROPER CARE
OF POOL OR SPA WATER WHEN A COVER IS USED; AND ELECTRIC RESISTANCE
HEATING - NO ELECTRIC RESISTANCE HEATING; AND
1.1.EXCEPTION 1 TO SECTION 114 (A)4: LISTED PACKAGE UNITS WITH FULLY
INSULATED ENCLOSURE, AND WITH TIGHT-FITTING COVERS THAT ARE
INSULATED TO AT LEAST R-6.
1.2.EXCEPTION 2 TO SECTION 114(A)4: POOLS OR SPAS DERIVING AT LEAST 60
PERCENT OF THE ANNUAL HEATING ENERGY FROM SITE SOLAR ENERGY OR
RECOVERED ENERGY.
2. ANY POOL OR SPA SYSTEM OR EQUIPMENT SHALL BE INSTALLED WITH ALL OF THE

COMPLETED AND SIGNED BY THE INSTALLER OF RECORD AND SUBMITTED TO THE
BUILDING INSPECTOR PRIOR TO FINAL INSPECTION APPROVAL,
4. DIRECTIONAL INLETS AND TIME SWITCHES FOR POOLS. IF THE SYSTEM OR
EQUIPMENT IS FOR A POOL:
4.1.THE POOL SHALL HAVE DIRECTIONAL INLETS THAT ADEQUATELY MIX THE POOL
WATER; AND
4.2.A TIME SWITCH OR SIMILAR CONTROL MECHANISM SHALL BE INSTALLED AS
PART OF THE POOL WATER CIRCULATION CONTROL SYSTEM THAT WILL ALLOW
ALL PUMPS TO BE SET OR PROGRAMMED TO RUN ONLY DURING OFF-PEAK
ELECTRIC DEMAND PERIOD, AND FOR THE MINIMUM TIME NECESSARY TO
MAINTAIN THE WATER IN THE CONDITION REQUIRED BY APPLICABLE PUBLIC
HEALTH STANDARDS.
5. ALL PUMPS AND PUMP MOTORS INSTALLED SHALL BE LISTED IN THE
COMMISSIONS'S DIRECTORY OF CERTIFIED EQUIPMENT AND SHALL COMPLY WITH
THE APPLIANCE EFFICIENCY REGULATIONS.
6. FILTRATION PUMPS SHALL BE SIZED, OR IF PROGRAMMABLE, SHALL BE
PROGRAMMED, SO THAT THE FILTRATION FLOW RATE IS NOT GREATER THAN THE
RATE NEEDED TO TURN OVER THE POOL WATER VOLUME IN 6 HOURS OR 36 GPM,
WHICHEVER IS GREATER.
7. PUMP MOTORS USED FOR FILTRATION WITH A CAPACITY OF 1 HP OR MORE SHALL
BE MULTI SPEED.
8. EACH AUXILIARY POOL LOAD SHALL BE SERVED BY EITHER SEPARATE PUMPS OR
THE SYSTEM SHALL BE SERVED BY A MULTI SPEED PUMP. EXCEPTION - PUMPS
LESS THAN 1 HP MAY BE SINGLE SPEED.
9. MULTI SPEED PUMPS SHALL HAVE CONTROLS WHICH DEFAULT TO THE FILTRATION
FLOW RATE WHEN NO AUXILIARY POOL LOADS ARE OPERATING.
10. FOR MULTI SPEED PUMPS, THE CONTROLS SHALL DEFAULT TO THE FILTRATION
FLOW SETTING WITHIN 24 HOURS AND SHALL HAVE AN OVERRIDE CAPABILITY FOR
SERVICING.
11. A LENGTH OF STRAIGHT PIPE THAT IS GREATER THAN OR EQUAL TO AT LEAST 4
PIPE DIAMETERS SHALL BE INSTALLED BEFORE THE PUMP.
12. POOL PIPING SHALL BE SIZED SO THAT THE VELOCITY OF THE WATER AT MAXIMUM
FLOW FOR AUXILIARY POOL LOADS DOES NOT EXCEED 8 FEET PER SECOND IN THE
RETURN LINE AND 6 FEET PER SECOND IN THE SUCTION LINE.
13. ALL ELBOWS SHALL BE SWEEP ELBOWS OR ELBOW-TYPE THAT HAVE A PRESSURE
DROP OF LESS THAN THE PRESSURE DROP OF STRAIGHT PIPE WITH A LENGTH OF
30 PIPE DIAMETERS.
14. FILTERS SHALL BE AT LEAST THE SIZE SPECIFIED IN NSF/ANSI 50 FOR PUBLIC POOL
INTENDED.
15. MINIMUM DIAMETER OF BACKWASH VALVES SHALL BE 2 INCHES OF THE DIAMETER
OF THE RETURN PIPE, WHICHEVER IS GREATER.
SAFETY STANDARDS
SECTION AV100.3 OF THE 2016 C.R.C. ADDRESSES THE POOL SAFETY STANDARDS FOR
POOLS, SPAS AND HOT TUBS INSTALLED FOR SINGLE FAMILY RESIDENCE.
FOR THE PURPOSE OF THIS CODE THE FOLLOWING DEFINITIONS ARE APPLICABLE:
SWIMMING POOL: ANY STRUCTURE INTENDED FOR SWIMMING OR RECREATIONAL
BATHING THAT CONTAINS WATER OVER 18 INCHES DEEP.
ENCLOSURE: A FENCE, WALL, OR OTHER BARRIER THAT ISOLATES A SWIMMING
POOL FROM ACCESS TO THE HOME
ENCLOSURE REQUIREMENTS:
AN OUTDOOR SWIMMING POOL, INCLUDING HOT TUBS OR SPAS SHALL BE SURROUNDED
BY A BARRIER WHICH SHALL COMPLY WITH THE FOLLOWING:
1.TOP OF THE BARRIER SHALL BE AT LEAST 60 INCHES ABOVE GRADE MEASURED ON
THE SIDE OF THE BARRIER WHICH FACES AWAY FROM THE SWIMMING POOL
1.1.OPENINGS IN TECH BARRIER SHALL NOT ALLOW THE PASSAGE OF A 4 INCH DIA.
SPHERE.
1.2.OPENING BETWEEN THE FINISH GRADE AND THE BOTTOM OF THE BARRIER
SHALL BE MAX. 2 INCHES.
1.3.BARRIERS SHALL BE INSTALLED SO AS NOT HAVE ANY FEATURES WHICH AID IN
CLIMBING.
2.ACCESS GATES SHALL BE A MINIMUM OF 60 INCHES ABOVE GRADE AND SHALL BE
SELF CLOSING AND HAVE SELF LATCHING DEVICE. GATES MUST OPEN OUTWARD
FROM THE POOL. GATES SHALL MEET THE SAME PROVISIONS AS LISTED AS
BARRIERS.
3.OUTSIDE SURFACES SHALL BE FREE OF ANYTHING THAT CAN MAKE THE BARRIER
CLIMBABLE
4.IF USING THE WALL OF THE HOUSE AS AN ENCLOSURE, EXIT ALARMS MUST BE
PLACE ON ALL DOORS THAT PROVIDE DIRECT ACCESS TO THE POOL.
SUCTION OUTLETS:
1.POOLS AND SPAS MUST HAVE AT LEAST TWO CIRCULATION OUTLETS, WHICH ARE
HYDRAULICALLY BALANCED AND SEPARATED BY THREE FEET OR
2.UTILIZATION OF SKIMMERS, PERIMETER OVERFLOW SYSTEMS, OR OTHER
PRE-APPROVED DEVICES IN LIEU OF TRADITIONAL SUCTION DEVICES.
3.THE USE OF OTHER OUTLET DEVICES WHICH MAY MEET ANSI/APSP-7 STANDARDS

SOILS AND FOUNDATIONS
1. THE DESIGN OF THE FOUNDATION SYSTEM IS BASED UPON THE CRITERIA AND RECOMMENDATIONS CONTAINED IN
THE FOLLOWING GEOTECHNICAL INVESTIGATION REPORT:

GEOTECHNICAL INVESTIGATION" BY SLADDEN ENGINEERING (PROJECT No. 544-4810), DATED 01.28.05.
B. "PROPOSED CUSTOM RESIDENCE, 53335 ROSS AVENUE, THE MADISON CLUB LOT 30A, LA QUINTA, CALIFORNIA"
BY SLADDEN ENGINEERING (PROJECT No. 544-20168, DATED 05.28.20.
2. LOCATE AND PROTECT EXISTING UTILITIES TO REMAIN DURING AND/OR AFTER CONSTRUCTION.
3. REMOVE ABANDONED FOOTINGS, UTILITIES, ETC. WHICH INTERFERE WITH NEW CONSTRUCTION, UNLESS

6. REMOVE LOOSE SOIL AND STANDING WATER FROM FOUNDATION EXCAVATIONS PRIOR TO PLACING CONCRETE.
7. BOTTOM OF FOOTINGS SHALL BE LEVEL. FOOTING ELEVATION CHANGES SHALL BE MADE USING FOOTING STEPS.
8. EXCAVATIONS FOR FOUNDATIONS MUST BE ACCEPTED BY THE GEOTECHNICAL ENGINEER OF RECORD (GEOR)
PRIOR TO PLACING REINFORCING AND CONCRETE. NOTIFY THE GEOR WHEN EXCAVATIONS ARE READY FOR
INSPECTION.
9. PLACE BACKFILL BEHIND RETAINING WALLS AFTER CONCRETE HAS ATTAINED FULL DESIGN STRENGTH.
10. MECHANICALLY COMPACT EXCAVATION BACKFILLS IN LAYERS. PROVIDE 90% MINIMUM COMPACTION IN
ACCORDANCE WITH THE ASTM D1557 TEST METHOD AND THE GEOTECHNICAL REPORT.
11. SCARIFY AND RECOMPACT TOP 6 INCHES OF SOIL BENEATH FILL PER ITEM ABOVE.
12. PROVIDE POSITIVE SLOPES, BOTH DURING AND AFTER CONSTRUCTION, TO DIRECT SURFACE AND ROOF RUNOFF A
MINIMUM OF 10'-0” FROM BUILDING FOUNDATIONS.
REINFORCING STEEL
1. REINFORCING SHALL BE FABRICATED ACCORDING TO THE “MANUAL OF STANDARD PRACTICE FOR REINFORCED
CONCRETE CONSTRUCTION” AND SHALL CONFORM TO ASTM A615, GRADE 60.
2. PROVIDE REINFORCING SHOWN OR NOTED CONTINUOUS IN LENGTHS AS LONG AS PRACTICABLE.
3. TERMINATE REINFORCING STEEL IN STANDARD HOOKS UNLESS OTHERWISE SHOWN.
4. ALL REINFORCING STEEL INTERRUPTED BY STRUCTURAL STEEL SHALL TERMINATE WITHIN 1" OF STEEL SURFACE
WITH A 90° STANDARD HOOK UNLESS OTHERWISE NOTED.
5. PROVIDE REINFORCING BAR DOWELS IN FOOTINGS, PILE CAPS AND OTHER SUPPORTING MEMBERS OF THE SAME
SIZE AND SPACING AS VERTICAL REINFORCING UNLESS OTHERWISE NOTED. LAP SPLICE AS INDICATED ON TYPICAL
DETAILS.
6. ANCHOR BOLTS, DOWELS AND OTHER EMBEDDED ITEMS SHALL BE SECURELY TIED IN PLACE BEFORE CONCRETE
IS PLACED.
7. ACCURATELY POSITION, SUPPORT AND SECURE REINFORCEMENT FROM DISPLACING DUE TO FORMWORK,
CONSTRUCTION, OR CONCRETE PLACEMENT OPERATIONS. LOCATE AND SUPPORT REINFORCING BY METAL
CHAIRS, RUNNERS, BOLSTERS, SPACERS AND HANGERS AT A MAXIMUM SPACING OF 3'-0”.
8. FIELD BENDING OF REINFORCING STEEL EMBEDDED IN CONCRETE SHALL NOT BE ALLOWED UNLESS APPROVED BY
THE SEOR.
9. MILL CERTIFICATIONS SHALL BE SUBMITTED FOR REVIEW.

DOCUMENTS.
2. REFERENCE TO CODES, RULES REGULATIONS, STANDARDS, MANUFACTURER'S INSTRUCTIONS OR REQUIREMENTS
OF REGULATORY AGENCIES IS TO THE LATEST EDITION OF EACH IN EFFECT AT THE DATE OF SUBMISSION OF BID
UNLESS THE DOCUMENT DATE IS SHOWN.
3. DRAWINGS INDICATE GENERAL AND TYPICAL DETAILS OF CONSTRUCTION. WHERE CONDITIONS ARE NOT
