public inspection during the regular business hours of the public entity.

WARNING: FAILURE TO SECURE WORKERS' COMPENSATION COVERAGE IS

NOTICE TO APPLICANT (1, 2 and 3)
1) Each person upon whose behalf this application was made, each person
at whose request and for whose benefit work is being performed under or
pursuant to any permit issued as a result of this application, the owner, and
the applicant, each agrees to, and shall defend, indemnify and hold harmless
the City of La Quinta, its officers, agents, and employees for any act or
omission related to the work being performed under or following issuance of
this permit.
2) Any permit issued as a result of this application becomes null and void if
work is not commenced within 360 days from date of issuance of such
permit, or if cessation of work for 180 days will subject permit to
cancellation. On or after January 1, 2019, every permit issued shall become
invalid unless the work on the site authorized by such permit is commenced
within 12 months after its issuance, or if the work authorized on the site by
such permit is suspended or abandoned for a period of 12 months after the
time the work is commenced.
3) Certified that they read the application and state that the information
provided is correct. Agree to comply with all city and county ordinances and
state laws relating to building construction, and hereby authorize
representatives of this city to enter upon the above -mentioned property for
inspection purposes.
IMPORTANT: An Application for Permit was made to the Building Official,
subject to the conditions and restrictions set forth at the time of application,

Inspection
Construction or work for which a permit is required shall be subject to inspection by the building official and such construction or work
shall remain visible and able to be accessed for inspection purposes until approved. Approval as a result of an inspection shall not be
construed to be an approval of a violation of the provisions of the applicable code and other ordinances of the jurisdiction. Inspections
presuming to give authority to violate or cancel the provisions of the applicable code and other ordinances of the jurisdiction shall not be
valid. It shall be the duty of the owner or the owner's authorized agent to cause the work to remain visible and able to be accessed for
inspection purposes. Neither the building official nor the jurisdiction shall be liable for expense entailed in the removal or replacement of
any material required to allow inspection.
It shall be the duty of the holder of the building permit or their duly authorized agent to notify the building official when work is ready for
inspection. It shall be the duty of the permit holder to provide access to and means for inspections of such work that are required by this
code.
A building or structure shall not be used or occupied in whole or in part, and a change of occupancy of a building or structure or portion
thereof shall not be made, until the building official has issued a certificate of occupancy therefor as provided herein. Issuance of a
certificate of occupancy shall not be construed as an approval of a violation of the provisions of the applicable code and other ordinances
of the jurisdiction. Certificates presuming to give authority to violate or cancel the provisions of this code or other ordinances of the
iurisdiction shall not be valid.
A person shall not make connections from a utility, a source of energy, fuel, or power, or a water system or sewer system to any building
or system that is regulated by the applicable code and other ordinances of the jurisdiction for which a permit is required, until approved by
the buildino official.

such as, "Added note 16 on sheet A-3" are appropriate and will help expedite your back check.
2) Corrections may not be made by handwriting on existing drawings. Revise originals and reprint Plans alid/or
Calculations as necessary.
3) Return all red -marked Plans and/or Calculations with your resubmittal.
4) Each sheet of resubmitted Plans and/or Calculations shall include the preparer's name and telephone number
and shall be wet -signed by the preparer. If the preparer is a licensed architect or engineer, all documents
prepared by that licensed individual shall also bear his/her stamp as prescribed by California Business and

SECTION A - GENERAL
1) Specify within scope of work that project is proposed to combine suite addresses 78-570 and 78-560 into one
suite.
2) Provide on each sheet of resubmitted Plans and cover sheet of bound Calculations the preparer's name and
telephone number and wet -signature. If the preparer is a licensed architect or engineer, all documents
prepared by that licensed individual shall also bear his/her stamp as prescribed by California Business and
Professions Code ' 5536. Resubmittals will not be accepted with signatures missing.
3) Provide written evidence of Riverside County Fire Department approval of this Project. Contact the Fire
Department directly at (760) 863-8886.
SECTION B - ARCHITECTURAL
1) Professional offices of health care providers shall comply with Section 1113-805. (11B-223.4) Provide the
following throughout the proposed expansion.
a) Where seating is provided in waiting rooms, at least 5 percent of the seating shall be wheelchair spaces
complying with Section I IB-802.1. (11B-805.3)
b) A 36 inch (914 min) minimum wide clear space shall be provided along the full length of each side of
beds, exam tables, procedure tables, gurneys and lounge chairs. (I IB-805.4.1)
c) Turning space complying with Section I IB-304 shall be provided within the rooms. (I IB-805.4.3)
d) All hand washing fixtures, lavatories and sinks shall comply with Section I IB-606. (I IB-805.6)
e) Built-in cabinets, counters and work surfaces shall be accessible, including: patient wardrobes, nurse's
stations, administrative centers, reception desks, medicine preparation areas, laboratory work stations,
equipment consoles, clean and soiled utility cabinets, and storage areas; and shall comply with Sections
I I B-225 and 11 B-902. (I 1 B-805.7)
2) Handles, pulls, latches, locks, and other operable parts on doors and gates shall be operable with one hand and
shall not require tight grasping, pinching, or twisting of the wrist. The force required to activate operable parts
shall be 5 pounds maximum. Operable parts of such hardware shall be 34 inches minimum and 44 inches
maximum above the finish floor or ground. Where sliding doors are in the fully open position, operating
hardware shall be exposed and usable from both sides. (CBC § 11 B-309.4 and 11 b-404.2.7) Revise plans to
demonstrate compliance at sliding doors and their proposed hardware.
3) Locks and latches shall be permitted to prevent operation of doors in Group A having an occupancy load of
300 or less, Groups B, F, M and S, and in places of religious worship at the main exit door or doors only
provided the locking device is readily distinguishable as locked and the door is posted with a readily visible
sign on the egress side stating: "THIS DOOR TO REMAIN UNLOCKED WHEN BUILDING IS
OCCUPIED" in letters 1 inch in height on contrasting background. (CBC § 1008.1.9.3) Verify that the
existing hardware at the rear egress doors is single action type.
SECTION C — ELECTRICAL
1) All branch circuits serving patient care areas shall be provided with an effective ground -fault current path by
installation in a metal raceway system, or a cable having a metallic armor or sheath assembly. The metal
raceway system, or metallic cable armor, or sheath assembly shall itself qualify as an equipment grounding
conductor in accordance with 250.118. (CEC §517.13(A)) Verify the proposed conductors comply.

SECTION D - PLUMBING
1) In business and mercantile occupancies with a total occupant load of 50 or less including customers and
employees, one toilet facility, designed for use by no more than one person at a time, shall be permitted for
use by both sexes." — Interpretation: You may use single user toilet facilities to satisfy the requirements for
fixtures of both sexes when under the occupant load threshold. Show that the proper number of plumbing
fixtures is provided for this occupancy meeting the requirements of Chapter 4 of the 2013 California
Plumbing Code (CPC), Revise fixture calculation and demonstrate compliance on plans.
SECTION E — ENERGY EFFICIENCY
1) The performance approach may be used for whole building permit applications; or for permit applications that
involve combination of either building envelope and indoor lighting or the mechanical system or for lighting
and mechanical together. The performance method may be used to demonstrate compliance with the envelope
alone or the mechanical system alone, but cannot be used to show lighting compliance alone. (2013 California
Non -Residential Compliance Manual, Section 11.4 — Application Scenarios) Provide LTG forms showing
compliance with lighting fixtures specified on plans using the Complete Building Method, Area
Category Method, or Tailored Method. Due to lack of information in this submittal, a comprehensive
review will be performed at next submittal.
2) Luminaires providing general lighting that are in or are partially in the Skylit Daylit Zones or the Primary
Sidelit Daylit Zones shall be controlled independently by fully functional automatic daylighting controls that
meet the applicable requirements of Section 110.9, and the applicable requirements of Section 130.l(d)2,

SECTION F - MISCELLANEOUS COMMENTS
1) Red marks on Plans, even if not specifically mentioned in this list, indicate items needing correction. Revise
Plans as necessary and provide written response, noting where correction can be found.
2) Provide one copy each of the Site Plan and Floor Plan for the County Assessor's office. While not required
for resubmittal, this administrative item must be addressed prior to issuance of building permit.

controlled space and works with the room controller(s) to maintain the
design light level. WattStopper's exclusive control algorithm uses on/off or
