In accordance with Special Provision Section 1550 2.0 Federal Trainee Program, The City is reimbursing the
Contractor $0.80 per hour of training on this contract under the approved training program.
DESCRIPTION OF CHANGE
This change order provides payment for costs for the Contractor training program at the rate of $0.80
per hour for the agreed number of 7828 hours. Total amount to be reimbursed to the Contractor shall
be $ 6,262.40.
Payment shall be agreed lump sum price of $ 6,262.40.

We, the undersigned Contractor, have given careful consideration to the change proposed and hereby agree, if this
proposal is approved, that we will provide all equipment, furnish all materials, perform all labor, except as may be noted above,
and perform all services necessary to complete the above specified work, and hereby accept as full payment the amount shown
above, which includes all direct and indirect overhead expenses for any delays.
