I/We, Village Park Animal Hospital the undersigned, hereby state that I/We am/are the legal owners
(s) of the property located at 51230 Eisenhower Drive, identified as Assessor's Parcel Number of
773-072-019 (Subject Property).
I/We have been informed by the City of La Quinta and/or its representatives that the City of La
Quinta plans to construct the proposed La Quinta Village Complete Streets - A Road Diet Project
within the downtown La Quinta area (Project). The construction of the Project will require that the
City protect landscaping, tree and tie-in, installation of temporary fencing on the Subject Property
to tie into the proposed Project improvements.
I/We also state that I/we have seen and received a copy of the project drawings for the permit to
enter onto the Subject Property, which propose construction on the Subject Property consisting of:
1. Landscaping and tree protection and tie-in, installation of temporary fencing
2. No permanent City improvements (such as a walkway, etc.) will be constructed on our
property.
I/We hereby give my/our consent and permission to the City of La Quinta and its authorized
contractors; representatives and agents for the Project to enter onto the area of the Subject Property
shown on the project drawings and construct the improvements described above and shown on the
project drawings, 1/we hereby understand that the construction of these improvements on the
Subject Property is expected to take 2 months from the commencement of this construction.
I/We represent and warrant to the City that the party or parties whose signature appear below are
the authorized representatives of the owner of the Subject Property, and have the authority to agree
to the terms of this agreement, and that by entering into this agreement, the owner is not thereby in
breach or violation of any other agreement or contract.

Quinta Village Streets Project. I am assisting him in submitting the final package to the City for City
acceptance. We appreciate all of your assistance on this matter and would like to confirm that we
received your approvals and acknowledgement of the permit via the e-mail scanned photos.
Due to some grammar corrections to the first page, we made those revisions to include that the City is
to make adjustments of grade and landscaping protection on the Subject Property. I am attaching the
final corrected front page of the permit showing these corrections. This is for your records only but if
you could please reply and confirm your receipt and acceptance for the correction I would greatly
appreciate it.

I/We, Palm Desert Lodging, LLC the undersigned, hereby state that I/We am/are the legal owners
(s) of the property located at NW Corner of Desert Club Drive and Calle Tampico, identified as
Assessor's Parcel Number of 770-020-026 (Subject Property).
I/We have been informed by the City of La Quinta and/or its representatives that the City of La
Quinta plans to construct the proposed La Quinta Village Complete Streets - A Road Diet Project
within the downtown La Quinta area (Project). The construction of the Project will require that the
City make adjustments of grade, landscape protection, replanting and maintain irrigation system on
the Subject Property to tie into the proposed Project improvements.
UWe also state that I/we have seen and received a copy of the project drawings for the permit to
enter onto the Subject Property, which propose construction on the Subject Property consisting of:
1. Adjustment of grade, landscape protection, replanting and maintain irrigation system
2. No permanent City improvements (such as a walkway, etc.) will be constructed on our
property.
I/We hereby give my/our consent and permission to the City of La Quinta and its authorized
contractors, representatives and agents for the Project to enter onto the area of the Subject Property
shown on the project drawings and construct the improvements described above and shown on the
project drawings. Uwe hereby understand that the construction of these improvements on the
Subject Property is expected to take 2 months from the commencement of this construction.
I/We represent and warrant to the City that the party or parties whose signature appear below are
the authorized representatives of the owner of the Subject Property, and have the authority to agree
to the terms of this agreement, and that by entering into this agreement, the owner is not thereby in
breach or violation of any other agreement or contract.

Councilmember Pena said he will recuse himself from participating in the
discussion on Closed Session Item No. 2 due to a potential conflict of interest
stemming from the proximity of his residence to some of the real properties
under negotiation.
