Pursuant to your request, we are pleased to present our proposal for providing construction -staking
services for the above -referenced project.
Based upon what we believe to be your goals; we propose to provide the following services on above -
mentioned project:
Task 1: Control
Verify, establish and maintain horizontal and vertical control during the construction
staking process. (Control disturbed or destroyed during construction by any panty
other than NV5, will be reset/replaced on a time and expenses basis.).
Task 2: Sidewalk
Provide office calculations and one set of stakes for on -site sidewalks. Sidewalk will
be stake on one side only at 25' intervals off set to be determined by client and
graded to finish surface. Cut sheets will be provided. Four site visits are included in
our fee.
Task3: Parking Area
Provide office calculations and one set of stakes for Parking area. The edge of the
Parking area will be staked at 25' intervals off set to be determined by client and
graded to finish grade. Cut sheets will be provided. Three site visits are included in
our fee.
Task 4: Site Features
Provide office calculations and one set of stakes for Site Features which include
Crossings over water features and Art Display features. These features will be stake at
a offset to be determined by the client and graded to finish surface. Cut sheets will
be provided. Two site visits are included in our fee.

Task 5: Water
Provide office calculations and one set of stakes for water lines. Stakes will be set on
a pre -determined offset to centerline of water pipe at intervals of 50 feet, fire
hydrants and all appurtenances. Stakes will be set for line only, no grades will be
provided. One site visit is included in our fee.
Task 6: Sewer
Provide office calculations and one set of stakes for sewer lines. Stakes will be set on
a pre -determined offset to centerline of sewer pipe at intervals of 25 feet and all
appurtenances. Grades will be marked to flow line of sewer pipe. Cut sheets will be
provided. One site visit is included in our fee.
Task 7: Block Walls
Provide office calculations and one set of stakes for Garden and Retaining Walls.
Stakes will be set on a pre -determined offset from the face of wall at 25' intervals and
graded to top of footing and top of wall. Cut sheets will be provided. One site visit is
included in our fee.
Task 8: Restroom Building
Provide office calculations and one set of stakes for Restroom Building. Stakes will be
set on a pre -determined offset and graded to finish floor. Cut sheets will be provided.
One site visit is included in our fee.
Task 9: Parking Lot Lights
Provide office calculations and one set of stakes for Parking Lot Lights. A stake will be
set at each location for the lights in the proposed parking area. Cut sheets will be
provided. One site visit is included in our fee.
Task 10: Discharge Line
Provide office calculations and one set of stakes for the Discharge Line. Stakes will be
set on a pre -determined offset and graded to flow line. Cut sheets will be provided.
One site visit is included in our fee.
Task 11: Trench Drain
Provide office calculations and one set of stakes for the Trench Drain. Stakes will be
set on a pre -determined offset and graded to flow line and top of grate. Cut sheets will
be provided. One site visit is included in our fee.
Task 12: Equipment Vaults (per Partell Engineering Plan)
Provide office calculations and one set of stakes for the Equipment Vaults. Stakes will
be set on a pre -determined offset and graded to finish grade. Cut sheets will be
provided. One site visit is included in our fee.
Task 13: Meetings/Survey Management
Meetings, research, revisions and consultations with client and others as requested
or required.

Task 14: Additional Services
Additional services, stakes, or restaking will be provided when requested by the client
on a T&M basis at the applicable Charge Rate Schedule. NV5 will produce the
fieldwork order for approval and signature by the client at the time services are
provided.
PROJECT FEES
We propose to provide the foregoing services for the following fees. All fees listed will be considered
Time and Materials based upon the attached rate schedule. NV5 will invoice monthly by subtotal
groupings based upon time spent on each task.

We propose to provide the foregoing on a time and expense basis with an estimated budget of
$50,990. These services are to be included in the on call agreement between the City of La Quinta
and NV5.
The client will provide a digital copy of the files in AutoCAD format.
Per your direction, this is a prevailing wage project.

NV5 CHARGE RATES
NV5 would like to bill the City on a monthly basis. The billings will include fees earned for the billing period.
Project will be billed on a Time & Materials not to exceed for each project authorization per the rate sheet
included with this proposal. Possible additional charges if requested by the City may include geotechnical testing,
UAV or aerial topography, and mileage if over 40 miles from the City.

Rates are effective one year from issuance. If contract assignment extends beyond that date, NV5 will meet with

Note: Each individual project will be based upon a detailed not -to -exceed fee proposal using a project specific
scope of services.
