DECLARATION REGARDING CONSTRUCTION LENDING AGENCY (If
applicable)
A construction lender will be used for completion of the improvements
covered by this permit and the name and information of which is on file for
public inspection during the regular business hours of the public entity.
Failure of the applicant to indicate the construction lender on the
application does not relieve that person of the requirement to give the
construction lender preliminary notice from that duty.
WORKERS’ COMPENSATION DECLARATION (1, 2 or 3)
WARNING: FAILURE TO SECURE WORKERS’ COMPENSATION COVERAGE IS
UNLAWFUL, AND SHALL SUBJECT AN EMPLOYER TO CRIMINAL PENALTIES AND
CIVIL FINES UP TO ONE HUNDRED THOUSAND DOLLARS ($100,000), IN ADDITION
TO THE COST OF COMPENSATION, DAMAGES AS PROVIDED FOR IN SECTION
3706 OF THE LABOR CODE, INTEREST, AND ATTORNEY’S FEES.
1) Has and will maintain a certificate of consent to self-insure for workers’

Section 3700 of the Labor Code, for the performance of the work for which
this permit is issued.
3) Certifies that, in the performance of the work for which this permit is
issued, shall not employ any person in any manner so as to become subject
to the workers’ compensation laws of California, and agree that, if should
become subject to the workers’ compensation provisions of Section 3700 of
the Labor Code, shall forthwith comply with those provisions.
NOTICE TO APPLICANT (1, 2 and 3)
1) Each person upon whose behalf this application was made, each person
at whose request and for whose benefit work is being performed under or
pursuant to any permit issued as a result of this application, the owner, and
the applicant, each agrees to, and shall defend, indemnify and hold harmless
the City of La Quinta, its officers, agents, and employees for any act or
omission related to the work being performed under or following issuance of
this permit.
2) Any permit issued as a result of this application becomes null and void if
work is not commenced within 360 days from date of issuance of such
permit, or if cessation of work for 180 days will subject permit to
cancellation. On or after January 1, 2019, every permit issued shall become
invalid unless the work on the site authorized by such permit is commenced
within 12 months after its issuance, or if the work authorized on the site by
such permit is suspended or abandoned for a period of 12 months after the
time the work is commenced.
3) Certified that they read the application and state that the information
provided is correct. Agree to comply with all city and county ordinances and
state laws relating to building construction, and hereby authorize
representatives of this city to enter upon the above-mentioned property for
inspection purposes.
IMPORTANT: An Application for Permit was made to the Building Official,
subject to the conditions and restrictions set forth at the time of application,
which is now hereby an Issued Permit. If any part of the information being

Construction or work for which a permit is required shall be subject to inspection by the building official and such construction or work
shall remain visible and able to be accessed for inspection purposes until approved. Approval as a result of an inspection shall not be
construed to be an approval of a violation of the provisions of the applicable code and other ordinances of the jurisdiction. Inspections
presuming to give authority to violate or cancel the provisions of the applicable code and other ordinances of the jurisdiction shall not be
valid. It shall be the duty of the owner or the owner's authorized agent to cause the work to remain visible and able to be accessed for
inspection purposes. Neither the building official nor the jurisdiction shall be liable for expense entailed in the removal or replacement of
any material required to allow inspection.
It shall be the duty of the holder of the building permit or their duly authorized agent to notify the building official when work is ready for
inspection. It shall be the duty of the permit holder to provide access to and means for inspections of such work that are required by this
code.
Inspections
A building or structure shall not be used or occupied in whole or in part, and a change of occupancy of a building or structure or portion
thereof shall not be made, until the building official has issued a certificate of occupancy therefor as provided herein. Issuance of a
certificate of occupancy shall not be construed as an approval of a violation of the provisions of the applicable code and other ordinances
of the jurisdiction. Certificates presuming to give authority to violate or cancel the provisions of this code or other ordinances of the
jurisdiction shall not be valid.
Certificate of Occupancy
A person shall not make connections from a utility, a source of energy, fuel, or power, or a water system or sewer system to any building
or system that is regulated by the applicable code and other ordinances of the jurisdiction for which a permit is required, until approved by
the building official.

The plan needs to be corrected:
1) On the plan, provide the State license number for Pye Barker and remove the license number for
Comtron.
2) Provide the manufacture data sheet and the State Fire Marshal listing sheet for the communicator.
3) Remove the manual pull station. It’s not required. Reference California Fire Code 907.2
4) Fire alarm occupant notification is not required for this building. Two notification appliances are
shown inside one of the apartments. Partial occupant notification should not be provided. If occupant
notification is desired by the owner, then occupant notification shall be provided throughout the
building in accordance with CFC 907.5 and a statement shall be provided on the plans that the owner
has voluntarily chosen to provide occupant notification although occupant notification is not required
for this building by the California Fire Code. CFC 907.2.9.1
5) An automatic smoke detection system is not required for this building inside of the apartments if
single and multiple station smoke alarms are provided. One of the apartments shows a smoke detector
in one of the bedrooms. Explain the intention of this device. According to the floor plan, the
apartment unit with the smoke detector appears to be unit 107, but the enlarged riser room plan
references unit 203 for the location of this smoke detector. Reference CFC 907.2.11

The plan for the above referenced project was reviewed and approved by the Riverside County Fire
Department with the following conditions:
1. The Office of the Fire Marshal staff shall witness the function test.
2. Provide a completed NFPA 72 Record of Completion that the system has been installed in accordance
with the approved plans and specifications as required.
The Fire Department approved plans and conditions letter must be at the job site.
Applicant/installer shall be responsible to contact the Fire Department to schedule inspections. Requests for

1. Sway bracing and hangers will be inspected at the time of rough piping inspection.
2. All required hydrostatic pressure tests shall maintain 200 psi for a 2-hour duration. (EX: Residential Fire Sprinkler
Systems)
3. Fire sprinkler system remodels: if indicated on plans or this card, a hydrostatic pressure test shall be required.
4. All underground joints and thrusts blocks shall be exposed and readily visible during the hydrostatic test.
5. Contractor shall supply all testing equipment, access and operating instructions at time of test.
6. All systems shall be inspected, tested, and approved by a Fire Inspector before any work is started that may cause the
affected system to be covered or hidden.
POLICIES ON FIELD INSPECTIONS
A. INSPECTION REQUESTS: Inspections are made subject to availability. Please have the permit number available when

B. APPROVED PLANS: Shall be kept on the job site at all times that work is in progress. Work shall NOT commence
without approved plans.
C. INSPECTION REQUESTS: As required by the California Fire Code, the appropriate installing contractor shall be required
to schedule the necessary inspections and be present on site for inspections.
D. RE-INSPECTIONS: Will be charged against a permit when an inspection has failed, is not ready, the responsible party not
on site or where previously noted corrections have not been completed.
E. COMMODITIES: Unless otherwise noted on approved plans or letter of conditions, no commodities shall be entered into
the structure until clearance has been granted by the Fire Dept and Building Official.
F. OCCUPANCY: Buildings shall NOT be occupied by persons other than construction workers until a Final Fire Clearance
has been granted by the Fire Dept and a Certificate for Occupancy has been issued by the Building Official.

19.POWER LIMITED CABLE WHICH IS RUN WITHIN 7 FT. OF THE FINISHED FLOOR MUST BE PROTECTED FROM
PHYSICAL DAMAGE ARTICLE 760.130(B)(1).
20.ELECTRICAL POWER CIRCUITS SUPPLYING FIRE ALARM SYSTEMS SHALL BE A DEDICATED BRANCH
CIRCUIT. THE CIRCUITS SHALL BE MECHANICALLY PROTECTED CAPABLE OF BEING LOCKED IN THE ON
POSITION WITH AN APPROVED MECHANICAL CLIP. CIRCUIT DISCONNECT MEANS SHALL HAVE "RED
MARKING", ACCESSIBLE TO ONLY AUTHORIZED PERSONNEL AND BE IDENTIFIED AS "FIRE ALARM
CIRCUIT". THE LOCATION OF THE CIRCUIT DISCONNECT MEANS SHALL BE PERMANENTLY IDENTIFIED AT
FIRE ALARM CONTROL PANEL.
21.THROUGH PENETRATION FIRE STOPPING FOR ALL FIRE WALLS, FLOORS/CEILINGS AND ASSEMBLIES
SHALL HAVE AN F OR T RATING PER THE CBC AND STANDARDS. INSTALLATION OF THE FIRE STOPPING
SHALL COMPLY WITH APPROVED METHODS.
22.THE CIRCUIT DISCONNECT DISCONNECT BREAKER PROVIDING POWER TO THE FIRE ALARM UNIT SHALL
ONLY BE ACCESSIBLE TO AUTHORIZED PERSONNEL AND SHALL IDENTIFIED AS "FIRE ALARM CIRCUIT".
23.WHERE DETECTION IS NOT REQUIRED DURING CONSTRUCTION, DETECTORS SHALL BE INSTALLED UNTIL
AFTER ALL OTHER CONSTRUCTION TRADES HAVE COMPLETED CLEANUP. NFPA 72, 17.7.1.12.3.
24.THE OPERATION OF FIRE EXTINGUISHING SYSTEMS OR SUPPRESSION SYSTEMS SHALL INITIATE AN
ALARM SIGNAL BY ALARM-INITIATING DEVICES INSTALLED IN ACCORDANCE WITH THEIR INDIVIDUAL

MANUFACTURE FOR TYPE OF SMOKE. NFPA 72, 14.4.3.2.
27.THE SECONDARY POWER SUPPLY SHALL HAVE SUFFICIENT CAPACITY SUPPLY TO OPERATE THE SYSTEM
UNDER QUIESCENT LOAD (SYSTEM OPERATING IN NON-ALARM CONDITIONS) FOR A MINIMUM 24 HRS AND
AT THE END OF THAT PERIOD, SHALL BE CAPABLE OF OPERATING ALL ALARM NOTIFICATION APPLIANCES
USED FOR EVACUATION OR TO DIRECT AID TO THE LOCATION OF AN EMERGENCY FOR 5 MINUTES,
UNLESS OTHERWISE PERMITTED OR REQUIRED BY 10.6.7.2.1.1 THROUGH 10.6.7.2.2. NFPA 72, 10.6.7.2.1.
28.CONTRACTOR REQUIRED TO PROVIDE DECIBEL METER DURING FINAL ACCEPTANCE TESTING TO FIELD
