INTRODUCTION / BACKGROUND
USA Notify (FEIN: 81-0814502), a Sole Proprietorship, respectfully submits this proposal to
provide after-hours answering and messaging services for the City of La Quinta,
California.
Established in 2006, USA Notify has grown from a medical transcription provider into a
nationwide communications company specializing in live operator answering services and
proprietary text messaging solutions. We currently serve clients across the United States,
with a strong emphasis on reliability, responsiveness, and long-term client relationships.
All services are performed within the United States; no offshore resources are utilized.
USA Notify has extensive experience supporting organizations where timely and accurate
communication is critical, including medical offices and emergency -related call handling.
Our operators are trained to manage urgent, sensitive situations and ensure accurate,
protocol -driven message delivery.
To support uninterrupted 24/7/365 service, USA Notify partners with Smartphone Secretary
(FEIN: 83-3222630), a New York -based answering service provider with over 30 years of
experience. This partnership, in place for more than 10 years, provides operational
redundancy and consistent service quality.
USA Notify certifies that there are no known conflicts of interest associated with this
proposal. All pricing and terms contained herein shall remain valid for a period of six (6)
months from March 13, 2026.
The designated Contract Manager for this engagement will be Tami Wallace, a 12-year
employee with extensive experience in account management and service oversight.
Executive support and escalation will be provided directly by the undersigned.

PRICING PROPOSAL
USA Notify proposes a firm, fixed monthly rate of $500 to provide after-hours answering
services for the City of La Quinta.
This rate includes:
• Unlimited inbound call handling
• Message intake, documentation, and delivery
• Secure access to recorded calls and activity logs

There are no additional fees or usage -based charges. This pricing supports continuous
staffing and service availability 24 hours per day, 7 days per week, including weekends and
holidays.
SCOPE OF SERVICES
USA Notify will provide comprehensive after-hours answering services, including:
• Live operator call answering using City -approved scripts
• Accurate message capture and prompt delivery to designated City personnel
• Escalation of urgent or emergency calls in accordance with City protocols
• Adherence to all communication workflows and response procedures established
by the City
A secure, web -based client portal will be provided, allowing authorized personnel to:
• Monitor call activity in real time
• Review messages
• Access audio recordings of calls
STAFFING AND OPERATIONS
USA Notifywill assign a dedicated team of trained operators and a supervising manager to
the City of La Quinta account. Services will be provided on a 24/7/365 basis.
All personnel are trained in:
• Professional call handling and customer service
• Confidentiality and data protection
• Emergency and priority call escalation procedures
Operators will strictly follow City -provided scripts and escalation flowcharts to ensure
consistency, compliance, and reliability.

ADDITIONAL (COMPLEMENTARY) SERVICES

INSURANCE AND COMPLIANCE
Upon contract award, USA Notify will procure and maintain Commercial General Liability
Insurance with coverage limits of:
• $1,000,000 per occurrence
• $2,000,000 aggregate
USA Notifywill comply with all applicable federal, state, and local regulations, as well as
any additional insurance or endorsement requirements specified bythe City of La Quinta.
USA NOTIFY is a Sole Proprietor organization and exempt from Workers Compensation
Insurance.
