P. W. Stephens Contractors, Inc., hereinafter designated as PWSCl, proposes to furnish all )abor, material and equipment to complete work as follows:
REMOVE A_ND DISPOSE OF ASBESTOS CONTAINING ACOUSTICAL CEILING SPRAY.
REMOVE AND DISPOSE OF ALL CARPET. (CONTAMINATED), DEMO WALLS, MIRRORS,
PANELLING, ETC. AS NECESSARY.
H.E.P.A. VACUUM ALL FURNITURE AND SET ASIDE FOR OTHERS TO DISPOSE OF.
H.E.P.A. VACUUM AND CLEAN ENTIRE INSIDE OF BUILDING.
NOTE:
IT MAY NOT BE POSSIBLE TO GET THE UPHOLSTERED FURNITURE CLEAN ENOUGH
TO SAVE. IF SO, THERE WILL BE SOME ADDITIONAL DISPOSAL COSTS INVOLVED.
I SUGGEST YOU ALLOW APPROXIMATELY $4,800.00 FOR THIS.
TERMS AND CONDITIONS
The Work: PWSCI will supply all Work as described, in accordance with the documents ("Bid Documents") and time schedule I"Time Schedule") referenced herein
Schedule Change: PWSCl's obligation hereunder is based upon the Schedule, both as to duration and sequence· In the event of any significant change inSchedule,the bid
price and Schedule shag be equitably adjusted· PWSCI shag also receive an equitable adjustment of the bid price if this contract is terminated otherwise than for PWSCI's
material breach
Work Week: Unless otherwise stated, PWSCI will supply labor for the Work on a regular 40-hour Week without overtime, If the Customer requires overtime, PWSCI will be
reimbursed for the premium portion of wages paid (including payrog taxes and insurance) plus a reasonable allowance for loss of productivity caused bv the overtime
Guarantee: PWSCl warrants and guarantees that for a period of one vear from date of comp[etion of the Work, it will repair or replace anv Work which is defective in mate-
rials or workmanship or which does not conform to the specifications set forth in the Bid Documents· PWSCI's liability is Umited to the foregoing and it shah not in any case
be liable for indirect, incidental or consequentia[ damages of any kind, Additionagv, PWSCI does not guarantee the performance of any material for other than those uses for
which the materia~ is intended, in no case shall PWSCI's liability exceed that of the product manufacturer's guarantee·

Bonds: Upon request and at the expense of Customer, PWSCl will furnish performance and payment bonds written by a corporate surety. The cost of any such bond is not
included in the b d price unless spec fically stated otherwise·

of Work performed upon delivery to jobeite.
Changes and Extras Custome may change the Work at any time and from time to time, including changes in scope, methods, scheduling or performance requirement, in
which case the contract price and completion date w I be equitably adjusted·
Whenever the Customer, its employees, contractors, and subcontractors luther than PWSCI), use ladders, scaffolding, tools, vehicles, equipment or prop-
equitable adjustment in price shall be made and the contract time modified accordingly·
Force Majeure; PWSCI shall not be deemed in default nor be liable for damages for any failure or delay in performance of its Work which arise out of causes beyond its

This proposal is subject to change and may be withdrawn if not accepted within 60 days of above date. It is subiect to and includes all terms and con-
ditions listed above.
