ACTIVITY INFORMATION 1)Open to the Public Yes No 5)Alcoholic Beverages Yes No 2)Admission Charged Yes No $6)Attendance: Adults Teens Children 3)Food & Beverage Yes No 4) Describe Activity/Program and any Special Conditions: I, as permittee, and the USER agree to hold the City of La Quinta harmless, defend and indemnify the City of La Quinta from any and all liability for injury to persons or property, including liability related to exposure to communicable diseases, illnesses, or viruses, occurring as a result of the activity sponsored by the permittee, and said person shall be liable to the City of La Quinta for any and all damages to parks, equipment, and buildings owned or controlled by the City which result from the activity of the permittee or is caused by any participant in said activity. All persons exercising any of the privileges authorized does so at his/her own risk without liability on the part of the City of La Quinta for the injuries to persons or property resulting there from. INSURANCE: USER shall procure and maintain general liability/Commercial general liability insurance against any and all losses, costs, expenses, claims, liabilities, actions, or damages, including liability for injuries to any person or persons or damage to property arising at any time during and /or arising out of or in any way connected with USER use or occupancy of the facility and adjoining property, in an amount not less than $1,000,000 per occurrence, $2,000,000 general aggregate. Such insurance shall name CITY as an additional insured prior to the date of the Event. USER shall provide certificates of such insurance with CITY which shall be endorsed to provide such additional insured provisions. If such insurance is not provided, CITY may deny access. If alcohol is sold during the permitted activity, coverage must include full liquor liability. City of La Quinta, its officers, officials, agents, and employees shall be included as additional insureds on the policy. The undersigned has received and understands the rules and regulations for the City of La Quinta facility selected above and agree to abide by such rules and regulations. The undersigned agrees to clearly and accurately convey the rules and regulations to all members in their party. The undersigned further certifies that the information provided above is true. Please allow 7-14 business days for application review and processing. The undersigned agrees to clearly and accurately convey the rules and responsibilities to all members in their party. ___________________________________ Name ___________________________________ Signature ____________________________________ Date LQYSA SPORTS COMPLEX SAFETY PROTOCOL LQYSA has always been an organization aimed at providing a fun and safe environment for our youth to play baseball and softball. Due to the Covid-19 we had to call time on providing such opportunity for LQYSA youth to play ball. We realized the importance of our kids returning to an active lifestyle, as well as the concern of parents that want to make sure we can do so in a safe matter. The LQYSA organization has set a goal to provide a safe and clean environment for players, parents, and staff to return to our park. We have reviewed local and state guidelines and mapped out a way of opening the La Quinta Sports Complex by creating the following return to play consideration. General Public • Face masks will be highly recommended and/or maintain social distance of 3 feet in accordance with CDC guidelines. • Hand sanitizers stations will be made available near the snack bar and restrooms. • Advise the general public not to enter our sports complex if they are having flue like symptoms or have come into contact with a Covid-19 positive person. • They will be advised to follow the Covid-19 Self-assessment at home posted on our website. Coaches/Team/Staff • Limit of 2 coaching staff members will be allowed in the dugout and on the field during games. • Advise all coaches to wear mask if they will be having close contact with their players. • Coaches are responsible for providing hand sanitizer to player during practices and games. • Coaches will and are to advise parent and players to conduct the Covid-19 Self-Assessment at home prior to attending our fields. • Complete a COVID-19 ASSESMENT WAIVER before the start of each game. • It will be the coach’s responsibility to contact an LQYSA organization board member of an incident in which a person and/or team have encountered a person that tested positive for Covid-19 within 2 days of game day or practice; if a player or coach has any of the symptoms listed on the self-assessment questionnaire; or if a player or coach have tested positive for Covid-19 at any time. • Such instances above will be also applied to LQYSA staff. Response to Covid-19 incident • Silvia Emely Mejicanos has been the designated person at the La Quinta Sports Complex to handle a Covid-19 incident and will be responsible for implementing the current CDC, local and state guidelines in handling such incident by o Notify all team parents and coaches of incident and advise to take current CDC guideline protocols. o Team(s) that came into direct contact with such team will also be notified immediately. o If contact to positive person incident applies, coaches and team will not be allowed to return to the fields until a current negative Covid-19 test is turned in. o If coaches or player become Covid-19 positive, they will be asked to quarantine for the 10 days in accordance with the CDC guideline, and will be asked not to return to the fields until a negative Covid-19 test result is turned in. o Will apply any additional guidelines deemed necessary. Opening and use of the La Quinta Sports Complex Snack Bar • LQYSA staff will be trained in accordance with CDC, local and state guideline for the handling and preparation of food. • Only Authorized personnel will be allowed into the snack bar and will limit its capacity to help maintain three feet of social distance. • All La Quinta Sports Complex Snack Bar staff will be also assisting in ensuring and implementing the La Quinta Sports Complex Safety Protocol by posting sign such as the ones below: Self-Assesment Questionaire will be posted on our website made available to everyone Communication and Training • All LQYSA staff and board member will be given a copy of the protocol • LQYSA board member have been given a copy of the protocol and received proper training by LQYSA organization with information provided by https://www.riversidehealth.org as well as https://www.cdc.gov. • The La Quinta Website will be used at a point of reference for all protocol and recommendation and will apply direct links to the above websites.
