CONTRACT CHANGE ORDER NO. 006
Pursuant to the terms of the original Contract Agreement, you are hereby directed to make the herein described
changes or do the following described work not included in the plans and specifications for this Contract. Unless
otherwise stated all work shall conform to the terms, general conditions, and special provisions of the original
Contract.
DESCRIPTION OF CHANGE
Per direction of the Coachella Valley Water District, the preexisting abandoned waterline in the channel
on the east side of Dune Palms Road was removed from abutment 1 to abutment 5. This work was
performed at an agreed bid item price of $ 98/LF for 480 LF length. Total cost of $ 47,040.00
CVWD final tie-in work at the north and south connection point was performed by SRK forces, under
CVWD direction. CVWD required extra thrust blocks to be placed at the blind flange locations.
This work was tracked on a time and material basis and covered as Extra Work at Force Account.
Cost for the three days of work for SRK was $ 17,865.46. Ortiz 5 % markup made this total cost
$ 18,758.73.
Total $ 65,798.73
After review of tentative work agreements and invoices from the force account work, the agreed lump
sum price of $ 65,798.73 was determined. Price total will constitute full and complete compensation,
including markups for construction work and delays incurred.
